Book excerpt: The purpose of this dissertation was to test the theoretical hypothesis purported by writers in the area of incest that time-limited group therapy is an effective treatment to decrease symptoms seen in women incest victims. Increased interest in child sexual abuse has created a growing body of literature since the 1970's that focuses primarily on the prevalence and symptoms of incest. Treatment methods are the least researched, and, while prevalence and symptoms are documented empirically, only a handful of treatment paradigms are described well enough to replicate. Efficacy studies of treatment are virtually non-existent. This study attempted to document the efficacy of time-limited group therapy in six single-case studies. The hypotheses derived from the literature asserted that providing 12-week group therapy designed for this population would reduce symptoms related to depression and self-esteem. The null hypothesis was that no reduction of these symptoms would occur. Each case presented includes history, course of treatment, diagnosis, and formulation. After presenting results of the depression and self-esteem measurements, each case then focuses on the interpretation of the results. Qualitative data posited may contribute collateral evidence concerning the efficacy of the intervention. Summaries of the content and process of each group add to the understanding of incest and its treatment A critique of the design and measurement methods precedes a summary discussion including recommendations for replication of this research. This study found that time-limited group therapy was effective in reducing the target symptoms for some of the clients, but not others. It discusses why the group intervention appeared to be more effective in some cases than others.

Book excerpt: Because of the high prevalence of childhood incest in therapy-seeking populations of women, it is important to determine the treatment approaches that are most effective in resolving the negative consequences of this traumatic event. Although there is a general consensus in the clinical literature that a group treatment approach offers unique therapeutic benefits for this population that are not available in individual therapy, there is little empirical evidence to support this contention. The purpose of the present study was to empirically assess the effectiveness of a group treatment program on intrapersonal symptomatology and interpersonal difficulties in a clinical population of women with a history of incest. Subjects for the study were drawn from participants in a group treatment program conducted by a community-based, mental health agency and developed specifically for women with a history of incest. Thirty-two women who participated in the 20-week group treatment program were compared to thirty-one women who were wait-listed for the program for a similar period of time on measures of intrapersonal symptomatology and interpersonal difficulties. In addition, the effects of the group treatment program across time were examined by assessing intrapersonal symptomatology and interpersonal difficulties in group participants six months following completion of the group treatment program. Results indicate that a time-limited group which focuses on the original trauma is effective in reducing intrapersonal symptomatology for women with a history of incest and that this improvement is stable over time. Although the women who received group treatment felt more support from friends following their experience in the group and appeared to see themselves in a more positive light in their relations with other women over time, interpersonal difficulties, in general, did not show as much improvement as intrapersonal symptomatology. Clinical implications of these findings as well as suggestions for future research are discussed.

Anxiety disorders may develop from a complex set of risk factors, including genetics, brain chemistry, personality, and life events. Generalised Anxiety Disorder (GAD) is characterised by excessive, unrealistic worry that lasts six months or more; in adults, the anxiety may focus on issues such as health, money, or career. In obsessive-Compulsive Disorder (OCD) individuals are plagued by persistent, recurring thoughts (obsessions) that reflect exaggerated anxiety or fears; typical obsessions include worry about being contaminated or fears of behaving improperly or acting violently. Panic Disorder, a third type of anxiety disorder, is when people suffer severe attacks of panic which may make them feel like they are having a heart attack or are going crazy for no apparent reason. Post-Traumatic Stress Disorder (PTSD) can follow an exposure to a traumatic event such as a sexual or physical assault, witnessing a death, the unexpected death of a loved one, or natural disaster. Social Anxiety Disorder (Social Phobia; SAD) is characterised by extreme anxiety about being judged by others or behaving in a way that might cause embarrassment or ridicule. In regards to specific phobias people with specific phobias suffer from an intense fear reaction to a specific object or situation (such as spiders, dogs, or heights); the level of fear is usually inappropriate to the situation, and is recognised by the sufferer as being irrational. Drugs used to treat anxiety disorders include selective serotonin reuptake inhibitors (SSRIs), tricyclic antidepressants, benzodiazepines, beta blockers, and monoamine oxidase inhibitors (MAOIs). He maintains that there is no credible clinical evidence that psychotherapy is effective in handling personal or social problems, or that it is more effective than other modes of treatment. The theories that underpin clinical practice remain speculative and their influence over social policy are more ideological than scientific. A skeptical public and its government would be better served, Epstein says, by credible evidence of outcomes. His analysis focuses on whether psychotherapy is effective against a variety of unwanted behaviors, such as drug addiction and depression. The nation's social problems are due to the inadequacies of its core social institutions: families, communities, education, and jobs. Social problems emerge because many people are brought up in deficient families, live in dangerous communities, lack education and jobs, and have few or no routes out of poverty. Poor people are exposed to unrelenting risks to their physical and mental health. It is possible to remedy most deficiencies through human services that compensate for these failed social institutions. This position is inevitably unpopular in psychotherapeutic circles and in light of current political preferences since it requires massive new resources and extensive redistribution of existing resources. The extent of society's problems reflects the degree to which deficits in basic social institutions have been tolerated. Basic services have been lacking while psychotherapy diverts our impulse to address poverty into ineffective strategies. In a challenging conclusion, Epstein urges society to solve its problems by confronting the reality implied by the failure of psy-chotherapy's minhnal interventions: to acknowledge that more is necessary to resolve social need. This leads to general theoretical concerns about theory as such. The Illusion of Psychotherapy will be compelling reading for psychologists, psychotherapists, social scientists, and policymakers.
