Book excerpt: Excerpt from The Surgery of the Pancreas, as Based Upon Experiments and Clinical Researches The operative treatment of injuries and diseases of the pan creas belongs to the future. Until now, the efforts of surgeons have been limited to the treatment ofa few cases of cysts of the pancreas. The results obtained in these cases have been so encouraging, that undoubtedly other lesions of this organ will soon constitute new indications for surgical treatment. Book excerpt: Excerpt from The Surgery of the Pancreas, as Based Upon Experiments and Clinical Researches In some of the higher invertebrates certain organs connected with the alimentary canal have received the name of pancreas; but they have done so rather from their position and inferred function than from any certain evidence of their use, or from their anatomical structure. If they exist, they consist of simple cæcal appendages attached to the upper part of the intestine. In the osseous fishes, certain cæca or blind tubes may be seen at the commencement of the intestinal canal, close to the pylorus, which from their position have received the name of pyloric appendages, and have been regarded by most anatomists as the analogue of the pancreas in higher animals. In reptiles, we make a greater approach to the structure of the pancreas of higher animals, both in form and structure. In the frog the pancreas is shaped not unlike that of the human subject, but its broad end is in the opposite direction. It is in close approximation with the duodenum in its whole length. A proper duct cannot be found; probably small ducts from different parts of the gland open into the biliary duct as it passes through the gland. The pancreas of birds is proportionately larger than in any other animal. The gland has always more than one, usually two or three ducts which open by separate orifices and often at some considerable distance from one another. The chief differences between the pancreas in other mammalia and man relate merely to its color, its consistence, its degree of lobulation, its form, its volume, its union into a single mass or its separation into two distinct parts, and, lastly, its position and relations with different portions of the peritoneum. Its form is generally more or less that of a narrow band, divisible into two portions: one, the duodenal, following the curvature of the duodenum, and placed vertically or obliquely; the other, the gastrosplenic, extending transversely, and therefore opposite the other, from the duodenum to the spleen, against which it rests; the latter is always developed, the former is often defective or absent, and must be considered merely as an accessory portion. Available in PDF, EPUB and Kindle. Book excerpt: Excerpt from The Surgery of the Pancreas The pancreas lies not only functionally, but anatomically, at the centre of the abdominal cavity, lying transversely across the spinal column at the level of the first or second lumbar vertebra, though occasionally higher, across the twelfth thoracic. In shape Meckel compared the pancreas to a sort of hammer; Verneuil to a cross placed on its side, the short vertical arm repre senting the head; Winslow compared it to a dog's tongue; Piersol1 compares it with a revolver, the head of the pancreas representing the handle of the revolver. These descriptions convey fully as accurate a concept of the form of the organ as does the division into many parts and surfaces of which the anatomists seem so fond. Since the surgeon has no occasion to investigate the normal organ, and since the normal picture is so complicated by adhesions and infiamma tions when he does see it, the relation of the organ to surrounding structures is of more concern to him than the normal form. Book excerpt: Excerpt from The Pancreas: Its Surgery and Pathology In the chapter on histology, and again later under the heading of diabetes, we have discussed the structure and supposed functions of those characteristic groups of cells known as the islands of Langerhans. In doing so we have endeavoured to impartially summarise the evidence for and against the contending views that are held with regard to them, but, as will be gathered from the text, we personally are of opinion that the balance of available evidence strongly points to their being independent struc tures related to the control of carbohydrate metabolism within the body that the pancreas undoubtedly exerts. Book excerpt: ix Preface Smart surgeons learn from their own mistakes, smarter surgeons learn from mistakes of others, some never learn . . . You are a resident, overworked and constantly tired; sitting down with your mentor for a brief tutorial. What do you want to get out of these few minutes? To organize your thoughts and approaches to the particular problem; to learn how he -the weathered surgeon -"tackles it"; to grasp a few practical "recipes" or "goodies" and take home a message or two; to laugh a bit and unwind. This is also our goal in this book. We hope that you are not repelled or offended by the non-formal character of this book. This is how emergency abdominal surgery is taught best, by trial and error and repetitions, with emphasis on basics. This is not a "complete" textbook, nor is it a cookbook type manual or discussion of case studies; neither is it a collection of detailed lecture notes or exhaustive lists. Instead, it consists of a series of informal, uncensored, chats between experienced surgeons and their trainees. No percentages, series, elaborated figures or complicated algorithms are included; only a surgeon's narrative, explaining how "he does it" -based on his experience and state of the art knowledge of the literature. No references are included as it was our aim to put down nothing which has not been experienced, confirmed and practiced in our own hands.

Book excerpt: Palliative surgery can greatly contribute to improving symptom control and quality of life for terminal cancer patients. Owing to the advanced stage of the disease, however, this type of surgery is also associated with significant morbidity and mortality. It is therefore important for surgeons to have a sound understanding of the medical and scientific background underlying treatment decisions in palliative surgery, a foundation that this book provides. The opening chapters examine the relationship between palliative medicine and palliative surgery and address general issues including pain management and anesthesiological considerations. The role of palliative surgery in a wide range of disease settings is then thoroughly explored, including detailed information on surgical techniques and their indications and outcomes. This book will be an invaluable resource for all who wish to learn more about the emerging role of palliative surgery.

Book excerpt: Experimental surgery is an important link for the development in clinical surgery, research and teaching. Experimental surgery was part of the most important surgical discoveries in the past century. Since 1901 nine Nobel Prizes have been awarded to the pioneers had remarkable achievements in the basic or practical surgery. In recent 20 years, experimental surgery has achieved new advances, like laparoscopic and robotic surgery, tissue engineering, and gene therapy which are widely applied in clinic surgery. The present book covers wide experimental surgery in preclinical research models subdivided in two volumes. Volume I introduces surgical basic notions, techniques, and different surgical models involved in basic experimental surgery and review the biomechanical models, ischemia/reperfusion injury models, repair and regeneration models, and organ and tissue transplantation models, respectively. Volume II introduces several specific experimental models such as laparoscopic and bariatric experimental surgical models. The second volume also introduces graft-versus-host disease, and other experimental models. Review the advances and development of recent techniques such as tissue engineering, organ preservation, wound healing and scarring, gene therapy and robotic surgery. The book documents the enormous volume of knowledge we have acquired in the field of experimental surgery. In this book, we have invited experts from the United States, Canada, France, Germany, China, Japan, Korea, UK, Sweden, Netherland, Hungary and Turkey to contribute 36 chapters in the fields of their expertise. These two volumes are the compilation of basic experimental surgery and updated advances of new development in this field that will be invaluable to surgeons, residents, graduate students, surgical researchers, physicians, immunologists, veterinarians and nurses in surgery.

Available in PDF, EPUB and Kindle. Book excerpt: This book aims to raise awareness of IgG4-related sclerosing cholangitis among practicing physicians and to equip readers with a sound understanding of the principles of diagnosis and treatment. Clinical, serological, and histopathological features are clearly described and imaging appearances on all relevant modalities are illustrated, covering the bile duct and other involved organs. Differential diagnosis from other diseases, including primary sclerosing cholangitis and cholangiocarcinoma, is precisely explained. Information is then presented on all significant current and emerging therapeutic strategies, including steroids, immunosuppressive drugs, and rituximab. Finally, attention is drawn to significant prognostic features. While IgG4-related sclerosing cholangitis is now a widely acknowledged condition, most practitioners are still liable to misdiagnose it owing to a lack of familiarity with its presenting features. This book should help to rectify the situation and will be an asset for all who may encounter the disease in clinical practice.
