Chronic obstructive pulmonary disease (COPD) is a disease state characterized by airflow limitation that is not fully reversible. The airflow limitation is usually progressive and associated with an abnormal inflammatory response of the lungs to noxious particles or gases. Clinically, it is characterized by complaints of cough, sputum production, and/or dyspnoea, and diagnosed by a significantly reduced Tiffeneau index. It was the fourth leading cause of death in the United States in 1996, exceeded only by heart attacks, cancers and stroke. Hence, COPD is a major global health problem and its burden on society is growing rapidly. For these reasons, COPD was chosen as a study population in the present project.

Book excerpt: The purpose of this study was to obtain pilot data comparing aerobic exercise to strength training exercise on measures of physical function in COPD patients. Participants included seven patients who participated in the REACT II trial. REACT II was completed in April of 2006 and consisted of a 12-week aerobic training program. These same patients also partook in the current study of a 12-week high intensity progressive resistance training program that met three times a week. Each participant completed, in both the REACT II and current study, the six minute walk test, stair climb test, short physical performance battery and a self-reported physical function questionnaire. A two factor factorial Analysis of Variance (ANOVA) with two within factors was used to examine differences in outcome measures. The two within factors were time (baseline and three months) and intervention (aerobic and strength). Results of this study found no significant interactions between aerobic and strength training in COPD patients for the six minute walk distance, stair climb time, short physical performance battery or selfreported physical function. There was a trend (p = 0.07) towards significance for the difference between aerobic and strength intervention six minute walk distances (488.2 " 45.4 versus 397.9 " 80.1, respectively). There was also a trend (p = 0.08) towards significance for the difference between aerobic and strength intervention SPPB scores (11.3 " 0.5 versus 10.3 " 0.9, respectively). And lastly, there was a trend (p = 0.07) viii towards significance for the difference between baseline and three month physical function questionnaire scores (1.4 " 0.1 versus 1.3 " 0.1, respectively). In conclusion, neither aerobic or strength training demonstrated significant improvements in measures of physical function in this study. However, both training groups elicited changes in the expected direction for the measures studied. Further research is needed with a larger sample size to determine the optimal training program for COPD patients.

Book excerpt: Chronic obstructive pulmonary disease (COPD) is a progressive, debilitating and irreversible condition characterized by dyspnea or breathlessness upon exertion. Comprehensive pulmonary rehabilitation, which includes exercise training, has been shown to improve exercise tolerance, health-related quality of life and dyspnea. Long-term studies of rehabilitation outcomes show that these benefits tend to diminish over time. This study of patients in a pulmonary rehabilitation program examined (1) the physical and psychological factors related to exercise maintenance and (2) the role of exercise maintenance on long-term functional outcomes. One hundred sixty-four patients with chronic lung disease completed an 8-week rehabilitation program and were randomized to a 12-month maintenance program (n = 83) or a standard care condition (n = 81). Assessments of exercise, health-related quality of life (Quality of Well-Being Scale), dyspnea (UCSD Shortness of Breath Questionnaire and Dyspnea Scale of the Chronic Respiratory Questionnaire), exercise tolerance (6-minute walk test and Perceived Breathlessness), depression (CES-D), self-efficacy for walking, and disease severity were taken at pre- and post-rehabilitation, and at 6, 12, 24 months and 5-8 years after completing rehabilitation. Results show that the maintenance intervention did not prevent attrition from exercise as walking declined similarly across both groups by 24 months after rehabilitation. Patients in the maintenance condition, on average, walked 90 minutes per week during the 12-month intervention. Hierarchical regression analyses show that disease severity at post-rehabilitation was a statistically significant and unique predictor of overall exercise adherence during the 12 months following rehabilitation after controlling for health care utilization, age, and self-efficacy for walking. Patients with higher exercise participation had maintained better levels of self-efficacy, tolerance for dyspnea, and dyspnea during activities of daily living by 12 months after rehabilitation as compared to those who walked less. Findings support the presence of a relationship between health status and exercise maintenance among patients who complete pulmonary rehabilitation. Exercise maintenance in the form of walking may play an important role in the long-term outcomes of pulmonary rehabilitation for some patients. Maintenance interventions including regular medical follow-up need to be tailored to the patient's health status to further optimize long-term benefits of pulmonary rehabilitation.
