Book excerpt: Communication difficulties and social isolation are significant and ongoing consequences of traumatic brain injury (TBI). Literature to date has not specifically addressed if, how and why communication difficulties resulting from TBI impact upon maintaining and forming friendships. This qualitative pilot study describes and analyses how two young adults with TBI construct meaning about their communication and its impact upon their friendships. This information was sought with a view to understanding whether these adults perceived a link between their post-TBI communication and friendship changes; and in addition, which aspects of their communication were important in this scenario. Three major themes were evident upon analysis: Evolution of Friendships Following TBL, Perceptions of Communication, and Opening up to Others. Participants did not describe communication as being overtly linked to the evolution of their friendships. Communication was linked indirectly to their ability to maintain and form friendships via its influence on their ability to engage in activities with friends and peers. Participants wanted to open up to friends and peers about their post-TBI self. Unfortunately their communicative ability did not support this desire. Therefore, a second indirect link existed between communication and its role in supporting friendships. A conceptualisation of themes is presented that represents a method of considering post-TBI communication difficulties and the implications for maintaining and forming friendships. Implications for speech pathologists and future research are discussed.

Book excerpt: Traumatic brain injury (TBI) is the leading cause of disability among young adults in the United States. A major negative outcome from TBI is loss of employment, which is a source of diminished quality of life and economic stress for TBI survivors, their families, caregivers, and society, which often has to bear the costs. There is clear evidence that communication skills are important for successful employment in general, but little progress has been made in specifying the types of communication skills necessary for successful return to work in adults with TBI. In addition, based on literature searches, no published study has characterized the communication demands of different types of jobs. This dissertation research combined qualitative and quantitative methods to address this gap in knowledge. First, this research identified characteristics of competent communication for a specified occupational category, semiprofessional work (Aim 1). Aim 1.1 used qualitative interview procedures to characterize vocational communication variables related to semi-professional jobs. In Aim 1.2 these variables were matched to standardized tests of communication functions. Then, Aim 2 divided a sample of 31 participants with TBI into two groups: stably employed adults with TBI vs. those unable to maintain semi-professional employment post-injury. Both groups were administered the tests identified in Aim 1.2. A multiple logistic regression model was used to test the hypothesis that communication variables differentiate stably employed from unstably employed persons with TBI. Findings from this study contribute to research on outcomes from TBI and address translation of laboratory-based measures to outcomes in everyday life.

Book excerpt: Background: Communication impairments are prevalent after TBI with incidence rates commonly above 75%.1 Provision of education and training to communication partners is considered a key part of the rehabilitation process for people with TBI.2 Training can help to improve communication skills and facilitate communicative competence in people with TBI. Objective: To evaluate the current evidence on communication partner training and its effectiveness in terms of outcomes for people with TBI and/or their communication partners. Method: A systematic literature search was conducted across nine databases (AMED, CINAHL, EMBASE, Medline/EBSCOHOST, PsycINFO, PsycBITE, PsycARTICLES, PubMed, Scopus) from database inception to August 2018. English articles were included if they described an intervention directed at adult communication partners where the primary focus of the program (>50%) was on improving the communication skills for adults following TBI. From included studies, data was extracted for author, year of publication, participants, setting, design, intervention, outcome measures and findings. Studies were critically appraised using the Physiotherapy Evidence Database (PEDro) scale, Risk Of Bias in N-of-1 Trials (ROBiNT) or Critical Appraisal Skills Programme (CASP) tools for qualitative studies. Interventions were described in detail using the Template for Intervention Description and Replication (TIDieR) framework. Results: Eleven articles met eligibility criteria. These articles described six studies including: three RCTs, two non-RCTs and one single-case experimental design. These studies included 340 communication partners and 270 people with TBI. A wide range of outcome measures were reported with some positive changes in the skills of the person with TBI and/or their communication partner being reported. Some maintenance of skills were shown in the few studies (3/6) that conducted follow-up post-treatment. The methodological quality of the studies was varied and intervention description poor. Conclusions: The articles provided quantitative and qualitative evidence for the effectiveness of training communication partners including family members, police officers, paid carers and sales assistants. While the outcomes of the studies are positive, greater methodological rigour is needed with well described interventions to ensure replication into clinical practice. Training communication partners in communication skills should be an important consideration of any intervention to improve the interaction skills of people with TBI.
