Book excerpt: Autism Spectrum Disorder (ASD) is a development delay marked by impairment in social communication and behaviors, resulting in interpersonal relationship deficits (Bowler, Gaigg & Lind, 2011). These social deficits persist throughout the individual's life (Crane & Goddard, 2008), thus it is important to explore various etiologies and interventions to remediate these struggles. One area that has been implicated as potentially contributing to social deficits in ASD is episodic memory (EM), or an individual's ability to re-experience themselves in a past event (Boucher & Bowler, 2008; Hare, Mellor & Azmi, 2006). There are two forms of EM, autobiographical in which the person was actively involved and non-autobiographical in which the person was an observer (Lind, 2010; Gilboa, 2004). Autobiographical EM is reliant on autonoetic awareness, or an understanding of one's personal experience (Wheeler et al., 1997) where non-autobiographical EM is reliant on theory of mind, or an understanding of other's experience (Baron-Cohen, 2000). To better understand EM deficits in children with ASD, this study sought to understand if these weaknesses could be remediated through the use of cognitive behavioral therapy (CBT) and what cognitive processes and additional factors might impact the outcome of treatment. Participants included 73 children (59 male) ages 6-13 with high functioning ASD. Participants were administered two types of EM measures, one that assessed short-term non-autobiographical EM and one that assessed long-term autobiographical EM, before and after participation in either a group or individual 32 week CBT intervention. Results indicated that autobiographical EM functioning was impacted by participant's processing speed and narrative ability. Non-autobiographical EM functioning was influenced by participant age and the severity of their ASD symptoms. Findings also suggest that children who received individual therapy performed better on both measures after receiving treatment. Therefore, based on these outcomes it appears that EM functioning in children with ASD can be improved through the use of CBT. Future research should examine how this improvement impacts social functioning.

Book excerpt: Central challenges for children with autism spectrum disorder (ASD) can include difficulties with social communication, symptoms of emotional dysregulation, and pervasive effects of rigid and repetitive behaviors (Cuccaro et al., 2003; Matson & Nebel-Schwalm, 2007; Samson et al., 2014). Further, the heterogeneous phenotypes of individuals with ASD are often typified by additional forms of psychopathology. Nearly 40% of children with ASD are estimated to present with clinically elevated levels of anxiety (Van Steensel et al., 2011). Psychotherapy is a commonly used treatment for school-aged children with ASD and co-occurring anxiety. This secondary data analysis of a multisite randomized clinical trial (Wood et al., 2019) compared two variants of cognitive behavioral therapy (CBT) to explore the most efficacious treatment for addressing specific caregiver-defined goals. In this study, adapted CBT (BIACA; Wood et al., 2009) and standard-of-practice CBT (Coping Cat; Kendall & Hedtke, 2006) were compared: (1) to better understand the varying types of clinical needs that children with ASD and anxiety present with, and (2) to investigate which CBT approach is best suited to support these varying clinical needs. In this study's subsample, children with ASD and clinical levels of anxiety (N=148; aged 7-13 years, Mage=9.90, SD=1.78) were randomly assigned to a treatment condition (BIACA or Coping Cat). Both treatments provided 16 therapy sessions. BIACA personalized treatment through the utilization of a modular design which apportioned evidence-based treatment elements based on each child's clinical need. Coping Cat provided CBT in a prescriptive format regardless of child's overall clinical presentation. The primary outcome measure was parent severity ratings on caregiver-defined goals throughout treatment (Youth Top Problems; Weisz et al., 2011). BIACA outperformed Coping Cat on the primary outcome measure (p=.005). The findings from this study can inform clinicians about the benefits of personalized treatment plans as well as which caregiver-defined goals are better served by the modular/personalized format of adapted CBT. Further research should explore the feasibility of implementation by clinicians in varying contexts.

Cognitive remediation therapy (CRT) attempts to improve cognitive deficits by teaching information processing strategies through guided mental exercises. Objectives: The aim of this study is to investigate whether CRT, a new psychological treatment, improves working memory ad school performance in children with autism spectrum disorder.Methods:Children meeting the Diagnostic and Statistical Manual for Mental Disorders (DSM-5) criteria for ASD, and following a regular school curriculum were recruited from clinical population at the Child and Adolescent Psychiatry Department in Razi University Hospital u2013 Manouba - Tunisia. The intervention was individual CRT, delivered over a period of six months with one 45-minute session per week. Working memory was assessed using forward and backward digit span. School performance was assessed by school results.Results:Of the 25 patients included, 16 achieved assessments tests after CRT, thus constituting our final sample. Their average age was 10.87 years. The mean number of sessions performed was 22.38. After completion of CRT, patients showed significant improvement (p=0.001) on forward digit span which score was 3.38 before CRT and 4 after completion of the program. Concerning backward digit-span, the mean score increased from 2.21 before CRT to 3.09 after it. This improvement was significant (p=0.006). As for clinical symptoms, lower scores (26.81) were found after CRT in comparison with baseline mean scores (27.87). This improvement was also significant (p=0.001). As for school results, the mean score increased 10.57/20 before CRT to 11.08/20 after it, showing a statistically significant positive correlation (p=0.001).Conclusion: The cognitive remediation Therapy is a promising technique that seems to be effective in the management of children with autism spectrum disorder.

Book excerpt: Episodic memory (EM) is memory for past personal experiences; it is foundational to the development of imagination, introspection, future thinking, and social problem-solving and it shapes one's self-concept, identity, and life story. EM and theory of mind (ToM) are developmentally intertwined and functionally dependent, and impairments in each are well-documented in autism spectrum disorder (ASD). Despite tremendous empirical and theoretical import for the construal of EM as a core deficit in the social challenges in ASD, no formal interventions have been developed to support EM in this population. Meanwhile, an impressive body of literature has documented that one particular caregiver interaction style (i.e., elaborative reminiscing) causally and positively impacts typically developing children's EM and socio-cultural learning. The purpose of this study was to evaluate the effectiveness of a new caregiver-mediated EM intervention that adapts the principles of elaborative reminiscing in ways that align with best practices for supporting social cognition in children with ASD. Participants in this study were 27 caregiver-child dyads; all children had a diagnosis of ASD and were between the ages of 6-15 years. The following hypotheses were evaluated: 1) caregivers will demonstrate improvements in elaborative reminiscing from pre- to post-intervention, and 2) children will demonstrate improvements in EM (i.e., more EMs and EM functions and more accurate and specific EMs), personal narrative discourse, and ToM skills from pre- to post-intervention. We found that caregivers demonstrate increased elaborativeness at post-intervention, including the use of more mental state terms. We also found that children demonstrate improvements in EM specificity and basic theory of mind understanding at post-intervention. These results suggest that caregivers of children with ASD can be trained to be use highly elaborative interaction styles, and that elaborative reminiscing is associated with the development of EM and ToM in children with ASD across a wide range of ages. Clinical applications include providing clinicians with tools to train families to use this new intervention technique with their children with ASD. Additionally, future research should examine the effects of elaborative reminiscing on individuals with other clinical conditions in which EM deficits are present.
