Book excerpt: Type 2 diabetes affects 16 million Americans and disproportionately affects ethnic minorities. Specifically, African American and Hispanics are twice as likely to have Type 2 diabetes compared to their Caucasian counterparts. The impact of this disparity goes beyond the greater prevalence of the disease and includes greater morbidity and mortality. The recent Institute of Medicine (IOM) report documents racial and ethnic disparities in the treatment for many different medical conditions, including diabetes. Understanding and combating health disparities among minority groups is a national priority. Many researchers have focused on socioeconomic status (SES) as the main cause in health disparities. Much of the research over the past two decades has examined the impact of health outcomes among the uninsured and much of this research has found that insured individuals have better health outcomes than those without insurance. In order to begin to unravel the myriad of factors that contribute to these health disparities, researchers must further examine whether there are differences among ethnic minorities and Caucasians with health insurance coverage. The current study examined the association of insurance status on health care utilization patterns and quality of self-care behaviors among ethnic minorities and Caucasians with Type 2 diabetes. The 1998 Medical Expenditure Panel Survey (MEPS), a nationally representative sample of U.S. non-institutionalized civilians, dataset was used in this study. Data were extracted for insured Caucasian, African American, and Hispanic individuals aged 21-64 years with an ICD-9 diagnosis of Type 2 diabetes (i.e., with and without complications). Five hundred six individuals met the inclusion criteria.

Only part of an individual's health status depends on his or her behavior and choice; community-wide problems like poverty, unemployment, poor education, inadequate housing, poor public transportation, interpersonal violence, and decaying neighborhoods also contribute to health inequities, as well as the historic and ongoing interplay of structures, policies, and norms that shape lives. When these factors are not optimal in a community, it does not mean they are intractable: such inequities can be mitigated by social policies that can shape health in powerful ways. Communities in Action: Pathways to Health Equity seeks to delineate the causes of and the solutions to health inequities in the United States. This report focuses on what communities can do to promote health equity, what actions are needed by the many and varied stakeholders that are part of communities or support them, as well as the root causes and structural barriers that need to be overcome.

Book excerpt: Abstract: Caucasians and African-Americans with type 2 diabetes continue to experience preventable diabetes-related life-threatening complications despite medical advances and health care availability. African-Americans are more likely to suffer from diabetes-related complications and experience a higher diabetes-related mortality rate than Caucasians. There is a definite need to better understand the relationship of psychosocial variables to diabetes control in these two racial groups. The purpose of this two-group, comparative, descriptive study was to examine the relationships of social support, self-efficacy, and outcome expectations (beliefs that diabetes-related behaviors will lead to certain outcomes) to self-care behaviors and glycemic control in Caucasian and African-American adults with type 2 diabetes. A convenience sample of 91 Caucasian and African-American subjects receiving health care at three outpatient facilities participated in the study. All subjects completed four self-report measures: Social Support Questionnaire, Self-Efficacy Questionnaire, Outcome Expectancy Questionnaire, and the Diabetes Activities Questionnaire at the time of the outpatient visit. Long-term glycemic control was assessed by glycosylated hemoglobin analyses at the time of the outpatient visit. Two-sample t-tests revealed no significant differences between the two racial groups in regards to age (p = .81), duration of diabetes (p = .69), and glycosylated hemoglobin (p = .85). But has science gone so far in racializing diabetes as to undermine the search for solutions? In a rousing indictment of the idea that notions of biological race should drive scientific inquiry, Sweetness in the Blood provides an ethnographic picture of biotechnology’s framings of Type 2 diabetes risk and race and, importantly, offers a critical examination of the assumptions behind the recruitment of African American and African-descent populations for Type 2 diabetes research. James Doucet-Battle begins with a historical overview of how diabetes has been researched and framed racially over the past century, chronicling one company’s efforts to recruit African Americans to test their new diabetes risk-score algorithm with the aim of increasing the clinical and market value of the firm’s technology. He considers African American reticence about participation in biomedical research and examines race and health disparities in light of advances in genomic sequencing technology. Doucet-Battle concludes by emphasizing that genomic research into sub-Saharan ancestry in fact underlines the importance of analyzing gender before attempting to understand the notion of race. No disease reveals this more than Type 2 diabetes. Sweetness in the Blood challenges the notion that the best approach to understanding, managing, and curing Type 2 diabetes is through the lens of race. It also transforms how we think about sugar, filling a neglected gap between the sugar- and molasses-sweetened past of the enslaved African laborer and the high-fructose corn syrup- and corporate-fed body of the contemporary consumer-laborer.

Differences in health by racial and ethnic status could be increasingly consequential for health policy and programs. Such differences are not simply a matter of education or ability to pay for health care. For instance, Asian Americans and Hispanics appear to be in better health, on a number of indicators, than White Americans, despite, on average, lower socioeconomic status. The reasons are complex, including possible roles for such factors as selective migration, risk behaviors, exposure to various stressors, patient attitudes, and geographic variation in health care. This volume, produced by a multidisciplinary panel, considers such possible explanations for racial and ethnic health differentials within an integrated framework. It provides a concise summary of available research and lays out a research agenda to address the many uncertainties in current knowledge. It recommends, for instance, looking at health differentials across the life course and deciphering the links between factors presumably producing differentials and biopsychosocial mechanisms that lead to impaired health.

The research questions addressed were: 1) What is the self-care agency of adult African Americans diagnosed with type 2 diabetes? 2) What is the self-care practice of adult African Americans diagnosed with type 2 diabetes? 3) Is there a relationship between self-care agency and self-care practice in adult African Americans diagnosed with type 2 diabetes? The Denyes Self-Care Agency Instrument (DSCAI-90) and the Denyes Self-Care Practice Instrument (DSCPI-90) were used to survey a convenience sample of 40 adult African Americans with type 2 diabetes, living in the community. Findings revealed that self-care agency and self-care practice were present, and there was a positive correlation between the two concepts. The results of this study supported the usefulness of Orem's Self-Care Deficit Nursing Theory (SCDNT) with adult African American diabetic populations and identified the need for further research to examine self-care in other African American populations.
