With a greater proportion of the average American's diet being eaten outside of the home, many health experts are advocating for required labeling of the content of foods at the point-of-purchase in order for consumers to make informed and healthier food choices. There is limited evidence regarding the benefits of such regulations. In particular, there is a scarcity of information on the impact of non-calorie nutrition information at the point-of-purchase on consumer food purchasing behavior. ObjectiveTo assess consumer awareness of non-calorie nutrition information in fast food restaurants and food purchasing behavior before and 18-months after enforcement of regulations for posting non-calorie nutrition information at the point-of-purchase in King County, Washington. Study Design/MethodsThe study design incorporated a secondary data analysis that merged 2 cross-sectional data sets. The information was gathered from consumers who purchased a meal at one of eight fast-food restaurants (Burger King, McDonald's, Jack in the Box, ubway, Quizno's, Taco Bell, Taco Time, and Taco Del Mar) located in the King County, Washington area at two distinct points in time. Comparisons were made for the period prior to enforcement of menu-labeling regulation-from October and November of 2008 -and the period 18-months post-regulation enforcement-from April through June of 2010. Information on foods purchased was collected from itemized food receipts, and a short survey was conducted directly after meals were purchased on location in order to assess awareness and usage of menu labeling. The mean sodium content for meals purchased pre and post-regulation was compared by independent Student;s t-test. ResultsThe majority of participants were between 30 and 50 years of age (>40%), male (>60%), and Caucasian (>70%). Compared to data collected pre-regulation, there was a greater number of participants who reported seeing non-caloric nutrition information in the Burger-type (Burger-King, McDonalds, Jack-in-the-Box) and TexMex (Taco-del mar, Taco time, Taco Bell) restaurants post-regulation; the opposite finding was observed with Sandwich-type (Subway, Quizno's) restaurants. There was no significant difference in the amount of participants who saw and used the non-caloric nutrition information from pre to post-regulation in any of the restaurants. The mean sodium content of meals purchased was significantly lower post- compared to pre-regulation in sandwich-type restaurants and TexMex restaurants, but not burger-type restaurants. ConclusionAccording to our study, patrons in King County, Washington who purchased meals at Burger and TexMex-type restaurants seemed to be more aware of non-caloric nutrition information post- compared to pre-regulation enforcement. There was no significant difference in the number of patrons who saw and used the posted nutrition information. As compared between two time frames, a decrease in mean sodium content specifically at the TexMex and sandwich-type restaurants was observed. There is a need for more research in order to understand the complex dynamic of menu labeling, patron awareness, and patron use of such information.

There are limited studies done on the use of sodium nutrition information with regard to fast-food meal purchases. Purpose: To examine the difference in the number of participants who reported seeing and using non-calorie nutrition information at the point of purchase in fast food establishments, and the mean sodium content of their meal choices pre- and post-regulation enforcement of menu-labeling legislation in King County, Washington. Study Design and Methods: A secondary data analysis using cross-sectional data collected at two discrete time points: Wave 1 prior to the enforcement of King County's menu-labeling regulation (October and November 2008), and Wave 2 immediately following enforcement (April, May, and June 2009). Data were collected from itemized food receipts and a short survey developed to assess awareness and usage of menu-labeling given to 3,257 consumers who purchased a meal at one of eight fast-food restaurant chains (Burger King, McDonald's, Jack in the Box, Subway, Quizno's, Taco Bell, Taco Time, and Taco Del Mar). Pearson's Chi Square tests were used for all comparison of proportions for categorical data. Differences in mean sodium content of participant meal purchased and the three fast-food restaurant types (Burger, Sandwich and TexMex) between Waves 1 and 2 were examined using the independent Student's t-test. Results: More participants post-regulation enforcement reported that they saw non-calorie nutrition information at the point of purchase compared to participants pre-enforcement. The mean sodium content of meals pre- versus post-regulation was not significantly different and well above national nutrition recommendations for teenagers and adults. The mean sodium content of TexMex consumers' meals were lower post- compared to pre-regulation enforcement. Conclusion: Based on our findings, patrons to some Burger, Sandwich, and TexMex fast food restaurants in King County, WA, appeared to be more aware of non-calorie nutrition information after federal regulations were enforced to provide this information at the point of purchase. Among patrons to the TexMex Restaurants, this may have impacted the sodium content of their meal purchases. More research is needed to decipher whether it was menu-labeling regulations or other environmental changes that contributed to improvements in sodium purchasing behavior.

All options for addressing the childhood obesity epidemic must therefore be explored. In the United States, legal approaches have successfully reduced other threats to public health, such as the lack of passive restraints in automobiles and the use of tobacco. The question then arises of whether laws, regulations, and litigation can likewise be used to change practices and policies that contribute to obesity. On October 21, 2010, the Institute of Medicine (IOM) held a workshop to bring together stakeholders to discuss the current and future legal strategies aimed at combating childhood obesity. Legal Strategies in Childhood Obesity Prevention summarizes the proceedings of that workshop. The report examines the challenges involved in implementing public health initiatives by using legal strategies to elicit change. It also discusses circumstances in which legal strategies are needed and effective. This workshop was created only to explore the boundaries of potential legal approaches to address childhood obesity, and therefore, does not contain recommendations for the use of such approaches.

In particular, we examine the characteristics of consumers who use nutrition information and of those who express interest in using the information when they eat out in the future.

Policy or business interventions involving a segment of the food system often have consequences beyond the original issue the intervention was meant to address. A Framework for Assessing Effects of the Food System develops an analytical framework for assessing effects associated with the ways in which food is grown, processed, distributed, marketed, retailed, and consumed in the United States. The framework will allow users to recognize effects across the full food system, consider all domains and dimensions of effects, account for systems dynamics and complexities, and choose appropriate methods for analysis. This report provides example applications of the framework based on complex questions that are currently under debate: consumption of a healthy and safe diet, food security, animal welfare, and preserving the environment and its resources. A Framework for Assessing Effects of the Food System describes the U.S. food system and provides a brief history of its evolution into the current system. This report identifies some of the real and potential implications of the current system in terms of its health, environmental, and socioeconomic effects along with a sense for the complexities of the system, potential metrics, and some of the data needs that are required to assess the effects.
