Physical fitness testing in American youth was established on a large scale in the 1950s with an early focus on performance-related fitness that gradually gave way to an emphasis on health-related fitness. Using appropriately selected measures to collected fitness data in youth will advance our understanding of how fitness among youth translates into better health. In Fitness Measures and Health Outcomes in Youth, the IOM assesses the relationship between youth fitness test items and health outcomes, recommends the best fitness test items, provides guidance for interpreting fitness scores, and provides an agenda for needed research. The report concludes that selected cardiorespiratory endurance, musculoskeletal fitness, and body composition measures should be in fitness surveys and in schools. Collecting fitness data nationally and in schools helps with setting and achieving fitness goals and priorities for public health at an individual and national level.

A lack of activity increases the risk of heart disease, colon and breast cancer, diabetes mellitus, hypertension, osteoporosis, anxiety and depression and others diseases. Emerging literature has suggested that in terms of mortality, the global population health burden of physical inactivity approaches that of cigarette smoking. The prevalence and substantial disease risk associated with physical inactivity has been described as a pandemic. The prevalence, health impact, and evidence of changeability all have resulted in calls for action to increase physical activity across the lifespan. In response to the need to find ways to make physical activity a health priority for youth, the Institute of Medicine's Committee on Physical Activity and Physical Education in the School Environment was formed. Its purpose was to review the current status of physical activity and physical education in the school environment, including before, during, and after school, and examine the influences of physical activity and physical education on the short and long term physical, cognitive and brain, and psychosocial health and development of children and adolescents. Educating the Student Body makes recommendations about approaches for strengthening and improving programs and policies for physical activity and physical education in the school environment. This report lays out a set of guiding principles to guide its work on these tasks. These included: recognizing the benefits of instilling life-long physical activity habits in children; the value of using systems thinking in improving physical activity and physical education in the school environment; the recognition of current disparities in opportunities and the need to achieve equity in physical activity and physical education; the importance of considering all types of school environments; the need to take into consideration the diversity of students as recommendations are developed. This report will be of interest to local and national policymakers, school officials, teachers, and the education community, researchers, professional organizations, and parents interested in physical activity, physical education, and health for school-aged children and adolescents.

Today, children are exposed to technology and other gadgets increasing their opportunities to be sedentary thus, children’s lack of physical activity, could increase negative self-esteem reports. Because of this, engaging in an active lifestyle may help improve self-esteem in adolescents. Purpose: The purpose of this study is to assess how body composition and physical activity relates to self-esteem within adolescents. Methods: The study was completed following 15 weeks. A total sample of 44 (28 boys, 16 girls) subjects gave assent and parental consent to be included in the study. Students participated in various weightlifting classes in which they completed three mesocycles of resistance training during the semester. To test pre and post variables, Bioelectrical Impedance Analyzers (BIA) were used to assess body composition (BC). Self-esteem (SE) was measured by the Rosenburg Self-esteem Scale. A Digital Hand Dynamometer was be used to assess the effect of resistance training on student’s handgrip strength (HGS). Results: Due to unequal sample-sizes between genders, the primary statistics of this study were assessed as an independent t-test (collapsed across gender). Results revealed no significant difference for BC or HGS (p = 0.171 – 0.619), however, there was a significant difference between pre and post-test for SE (p = 0.001). Pre versus post SE mean was 26.68 and 30.36 respectively. Additionally independent t-tests were assessed per gender. For females, no significance was observed for any variables (p = 0.059 – 0.729). For males, no differences were seen for FM or HGS (p = 0.161 – 0.687), however, there was a significant difference for SE (p = 0.004). Lastly, a Pearson Correlation Coefficient was used to examine the relationships between FM, HGS and SE. Results from Pearson Correlation Coefficient test revealed no significant differences (p = 0.281 – 0.455) or effect sizes (-.116 - .100). Conclusion: The present findings suggest that structured physical activity, can have a positive outcome on high-school aged self-esteem values. While no significant differences were observed for body composition, or hand grip strength, it is possible that engaging in structured physical activity can increase adolescents’ self-esteem. Statistics suggest that males positive SE could be attributed to structured physical activity, however, more studies are needed to support this theory.

Book excerpt: In recent decades, the prevalence of youth obesity has been on the rise (Ogden et al, 2010) highlighting the need for research evaluating interventions targeting eating behaviors and lifestyle activity. Although it is important to examine the impact of interventions on physical activity, fitness, and body composition given their relationship with physical health, it is also important to assess the impact of interventions on mental well-being. Self-perceptions, including self-esteem, are important indicators of mental well-being (Blaine et al., 2006). In fact, Biddle (2011) found poor self-perceptions to be a predominant mental health issue in overweight adolescents. Although exercise is associated with improved self-esteem, the changes in self-esteem are often not large in magnitude (Spence et al., 2005). The greatest improvements are likely to occur in those populations that would benefit the most such as overweight individuals (Lox, Ginis, & Petruzzello, 2010). In addition to exercise, mentoring programs have also had positive effects on youth self-esteem (King, 2002). Thus, exercise combined with mentoring may by an effective strategy for enhancing self-perceptions. This study evaluated the impact of a mentor-based exercise program on the self-perceptions, body composition, physical activity, and fitness level of overweight adolescents. Adolescents participated in mentor-led, weekly exercise sessions for approximately 14 weeks. The program also involved a lifestyle-coaching component through weekly behavioral challenges designed to enhance self-regulatory skills related to physical activity and healthy eating. To date, 28 participants with a mean age of 14 and a baseline BMI of 32 have participated in the mentoring intervention. An additional 12 adolescents were randomly assigned to a wait-list control condition. In addition to fitness (maximum treadmill test), physical activity (accelerometers) and body composition (DXA) assessments, participants completed a questionnaire that examined exercise specific self-perceptions, physical self-worth, and global self-esteem (Whitehead, 1995; Harter 1988) pre/post intervention and after a no treatment maintenance period. A series of 2 (condition) X 2 (time) repeated measures ANOVA's combined with effect size inspection revealed that participants in the mentoring condition showed improved aerobic fitness (d = .44) compared to the control condition, which exhibited a negative change (d = -.06). Participants in the mentoring condition showed increased moderate-vigorous intensity activity (d = .41), compared to the control group (d = .14). While BMI-Z score improvements did not differ across conditions, percent body fat change for the experimental group showed a small, positive change (d = .22), compared to the control group (d = .08). In terms of self-perceptions, mentor participants reported improved self-esteem, physical self-worth, and perceived physical condition with the effect sizes being in the moderate to large range (i.e., d = .52, .79, .82), while the control group showed minimal change (i.e., d = .11, .26, .30). However, when evaluated after a no contact follow-up period, all variables in the experimental condition had regressed toward baseline. These results suggest a mentor-based exercise intervention has a positive impact while adolescents are in the program. However the program does not seem to be effective at helping participants maintain healthy changes when the program ends.

Book excerpt: Despite the many health benefits of being physically active, nearly a quarter of U.S. adults and adolescents report no participation in leisure-time physical activity. It is recommended that children and adolescents participate in physical activity for at least 60 minutes daily at moderate or vigorous intensity. In addition to potentially enhancing academic performance, participation in physical activity may also influence perceptions of physical appearance and global self-esteem, which tend to decrease with age in adolescent females. The purposes of this study were to: (a) examine the prevalence of overweight and obesity and the percentage of adolescent females from a rural community that meets physical activity recommendations; (b) examine the relationship of body composition, physical activity, and sedentary behavior on academic performance; and (c) examine the relationship between physical activity and physical self-concept among adolescent females. Thirty adolescent females (mean age = 15.6 " 1.3 years) wore an Actigraph GT1M accelerometer for seven consecutive days, set to measure in 15-second epochs. Age specific cutpoints were used to determine minutes of sedentary behavior and light, moderate, vigorous, and moderate-to-vigorous physical activity (MVPA). Participants completed the Physical Self-Description Questionnaire (PSDQ) to assess how they perceive themselves physically. Height, weight, and percent body fat were measured. Grade point averages (GPA) were obtained from school records. Regression analysis was performed to predict GPA from measures of body composition, sedentary behavior, and MVPA. Measures of physical activity and sedentary behavior were correlated with PSDQ subscale scores with Pearson correlations. Thirty percent of participants (n = 9) were obese (BMI [greater than or equal to] 95th percentile) and another 36% (n = 11) were overweight (BMI between the 85th and 94th percentiles). None of the participants accumulated [greater than or equal to] 60 minutes of MVPA per day. Daily MVPA averaged 9.7 (" 7.1) minutes.
