Book excerpt: Iterative learning control (ILC) has its origins in the control of processes that perform a task repetitively with a view to improving accuracy from trial to trial by using information from previous executions of the task. This brief shows how a classic application of this technique – trajectory following in robots – can be extended to neurological rehabilitation after stroke. Regaining upper limb movement is an important step in a return to independence after stroke, but the prognosis for such recovery has remained poor. Rehabilitation robotics provides the opportunity for repetitive task-oriented movement practice reflecting the importance of such intense practice demonstrated by conventional therapeutic research and motor learning theory. Until now this technique has not allowed feedback from one practice repetition to influence the next, also implicated as an important factor in therapy. The authors demonstrate how ILC can be used to adjust external functional electrical stimulation of patients’ muscles while they are repeatedly performing a task in response to the known effects of stimulation in previous repetitions. As the motor nerves and muscles of the arm reaquire the ability to convert an intention to move into a motion of accurate trajectory, force and rapidity, initially intense external stimulation can now be scaled back progressively until the fullest possible independence of movement is achieved.

The interaction between the spinal cord and sensory feedback also play a significant role in facilitating plasticity and functional recovery after neural trauma. Cross-education describes training one side of the limb to enhance the strength of the homologous muscle on the contralateral side. Previous study with chronic stroke participants found significant strength gains in the more affected leg following unilateral dorsiflexion training on the less affected side, which suggested cross-education can be used to boost strength gain when training the more affected side is hard to initiate. However, there is lack of evidence showing cross-education in the arm muscles after stroke and the neural pathways mediating strength cross-education in stroke participants require further study. The modulatory role of sensory feedback in movement control has been studied by using cutaneous stimulation as a proxy of the sensory input from skin. Mechanistic studies on neurological intact participants show that cutaneous reflex pathways are widespread in the cervical and lumbar spinal cord and have a global effect on the muscles in the non-stimulated limbs. In rehabilitation training, sensory enhancement from prolonged electrical stimulation has been used to facilitate training outcomes for those had stroke and other neurological disorders. Therefore, cutaneous pathways may be important in regulating cross-education training-induced strength gain. The purpose of this dissertation was to explore the effects of upper limb cross-education strength training in chronic stroke participants and the role of sensory inputs in regulating intra- and interlimb neural excitability in neurologically intact participants. In the first project (Chapter 2), we explored the efficacy of cross-education strength training in wrist extensor muscles of chronic stroke participants. Strength improvements were found bilaterally with altered excitabilities in the cutaneous pathways on the untrained side. These results show the potential role of cutaneous pathways in mediating strength transfer after unilateral strength training which led us to further explore the factors may affect the cutaneous modulation. In neurologically intact participants, we investigated the effects forearm position (Chapter 4), stimulation trigger mode and parameters (Chapter 5) on the cutaneous reflexes in the stimulated limb. Following the findings from Chapter 3, 4, and 5, the interlimb effects of self-induced sensory enhancement on the cutaneous reflexes were examined in Chapter 6. Taken together, data from this thesis confirms the clinical application of cross-education in strength training after stroke. It addresses that exaggerated bilateral strength gains and neural plasticity can be induced following unilateral strength training on the less affected side. In addition, sensory enhancement may be applied to amplify cross-education effects in strength training.

Book excerpt: Although evidence supports the use of electrical stimulation and repetitive task training for improving upper limb function after stroke, patients with minimal wrist and hand movement are often excluded from this treatment option. An important limitation of traditional electrical stimulation in this low level population is that the hand does not achieve a sufficiently open position to perform functional activities. The aim of this study was to examine the effect of combining a simple wrist-hand splint to stabilise the wrist and allow better hand opening, with electrical stimulation (ES) of the wrist and finger extensor muscles, in addition to functional practice in the sub acute stroke population. A case study design was employed that included four subjects who were average of 36.7 days post stroke and presented with palpable muscle activity of the wrist extensors. Subjects completed a 2 week period of therapy consisting of two conventional physiotherapy sessions a day, followed by a two week period of Electromyogram (EMG) triggered electrical ES in conjunction with splinting and functional practice in lieu of one physiotherapy session a day. Measurement of arm function (Chedoke Arm and Hand Activity Inventory), spasticity (Modified Tardieu Scale), grip strength (Jaymar dynamometry) and active wrist extension was performed repeatedly throughout the conventional treatment and trial intervention phases. Four subjects were recruited. One subject was withdrawn during the study as he could no longer trigger the stimulator at the commencement of the intervention phase. Subject responses were very variable. Case one showed no change in arm function or active wrist extension during the conventional or intervention phases. Grip strength improved more during the intervention compared to conventional phase, although the difference between phases did not reach statistical significance. Case two showed clinically significant gains in arm function with both conventional therapy and intervention. There was no significant difference between the two phases. Case three had difficulty completing all intervention sessions using patient triggered ES due to high levels of fatigue. There was minimal change during either phase in grip strength, active wrist extension or overall arm function. All subjects showed a greater change in the Modified Tardieu scale during the intervention phase compared to the conventional phase, suggesting that flexor spasticity in the wrist and fingers was reduced during the treatment phase. This provides some supporting evidence for the use of splinting and ES to reduce spasticity, although the difference between phases only reached statistical significance in one case. These findings provide initial support for the inclusion of combined electrical stimulation, splinting and functional practice as part of therapy for patients with minimal wrist and hand movement in the sub-acute period post stroke. Larger controlled trials are needed to explore the efficacy of this treatment approach further.
