These disorders are defined narrowly as the clinical syndromes of mania and melancholia. The latter is our particular focus, for the simple reason that it is more common and thus more is known about it. Our approach owes much to Adolf Meyer, who first used the term psychobiology. It was he who emphasized in a practical way the importance of the clinician consider ing the joint contribution of psychosocial and biological factors in the genesis of mental disorders. However, until the 1960s, our relative ignorance of basic mechanisms that link brain and behavior prevented the development of a genuine psychobiological perspective. Thus Meyer's work was concerned largely with teaching the importance of the personal biography and a consideration of social history in the development of mental disorder. We feel that sufficiently rigorous data have now emerged in psychia try to permit tentative but real psychobiological integration. Affective illness is proba bly the most promising area for an attempt at such a synthesis. It is our belief that the theory and clinical practice of psychiatry now can be woven into a coherent theme, integrating insights and evidence generated by the psychodynamic, biological, and behavioral methods; hence in part we review the emerging psychobiology of mood disorders with the hope that it can serve as a generic paradigm for other psychiatric syndromes.

Book excerpt: Depression is a widespread condition affecting approximately 7.5 million parents in the U.S. each year and may be putting at least 15 million children at risk for adverse health outcomes. Based on evidentiary studies, major depression in either parent can interfere with parenting quality and increase the risk of children developing mental, behavioral and social problems. Depression in Parents, Parenting, and Children highlights disparities in the prevalence, identification, treatment, and prevention of parental depression among different sociodemographic populations. It also outlines strategies for effective intervention and identifies the need for a more interdisciplinary approach that takes biological, psychological, behavioral, interpersonal, and social contexts into consideration. A major challenge to the effective management of parental depression is developing a treatment and prevention strategy that can be introduced within a two-generation framework, conducive for parents and their children. Thus far, both the federal and state response to the problem has been fragmented, poorly funded, and lacking proper oversight. This study examines options for widespread implementation of best practices as well as strategies that can be effective in diverse service settings for diverse populations of children and their families. The delivery of adequate screening and successful detection and treatment of a depressive illness and prevention of its effects on parenting and the health of children is a formidable challenge to modern health care systems. This study offers seven solid recommendations designed to increase awareness about and remove barriers to care for both the depressed adult and prevention of effects in the child. The report will be of particular interest to federal health officers, mental and behavioral health providers in diverse parts of health care delivery systems, health policy staff, state legislators, and the general public.

Depression not only has a profound impact on quality of life but it is associated with an increased risk of mortality from suicide and vascular disease. In fact for every medical illness studied, e.g. heart disease, diabetes, cancer, individuals who are depressed have a worse prognosis. Research has illuminated the physiological and behavioral effects of depression that accounts for these poor outcomes. The deleterious relationship between depression and other illnesses has changed the concept of late-life depression from a "psychiatric disorder" that is diagnosed and treated by a psychiatrist to a common and serious disorder that is the responsibility of all physicians who care for patients over the age of 60. This is the first volume devoted to the epidemiology, phenomenology, psychobiology, treatment and consequences of late-life depression. Although much has been written about depressive disorders, the focus has been primarily on the illness as experienced in younger adults. The effects of aging on the brain, the physiological and behavioral consequences of recurrent depression, and the impact of other diseases common in the elderly, make late-life depression a distinct entity. There is a compelling need for a separate research program, specialized treatments, and a book dedicated to this disorder. This book will be invaluable to psychiatrists, gerontologists, clinical psychologists, social workers, students, trainees, and others who care for individuals over the age of sixty.

It was the intent of the organizers to draw together leading international researchers to address, in a comprehensive fashion, the persisting problem of depression in schizophrenic individuals. As many of the authors point out, depression was clearly identified as a central problem in schizophrenia by the pioneers of psychiatry. Their wisdom and clinical acumen was lost for a time to be only recently re discovered. Their insights must now be integrated with modern taxonomic systems, evolving etiological models and methods of assessment. With increased recognition of the problem of depression in this population we must also examine appropriate methods of ameliorating the suffering of those afflicted. The conference was divided into sections (theoretical aspects, phenomenology, suicide and treatment) and the chairmen of these sections have given brief introductions. Necessary overlap of topics and methods provided a convergent and validating examination of empirical results which may serve to direct continuing studies. As well we hope that thoughtful review may directly influence clinical practice and thereby improve the quality of life for schizophrenic patients.
