Book excerpt: The therapeutic principle of pharmacological vasodilation in congestive heart failure is based upon established physiological and pathophysiological mechanisms and has been proved to be effective and useful. In acute heart failure this method of treatment has assumed importance as the first measure to be taken. In chronic heart failure a sizeable increase in cardiac output can be achieved with a number of vasodilating drugs. This effect can be maintained over several weeks or months. The adjustments of the heart and the circulation to the arteriolar and/or venous unloading effect of vasodilatation in the situation of acute and chronic heart failure are well understood. However, long-term adaptations of the organism to the vasodilat ing intervention have not yet fully been described. In many instances of chronic ad ministration of vasodilators, attenuation of the response has been observed. The mech anisms responsible have not been fully elucidated, nor has there been a satisfactory demonstration of improved exercise performance or of life expectancy under this mode of treatment in the patient with chronic heart failure. In an attempt better to understand the pathophysiological mechanisms of chronic congestive heart failure as they apply to long-term treatment with vasodilators, the working group on vasodilator therapy of the Deutsche Gesellschaft fUr Herz- und Kreis laufforschung has assembled a group of physiologists, pharmacologists, and clinicians. The Proceedings of their Symposium held in Hinterzarten in June 1981 are presented here.

Book excerpt: For many years, there has been a great deal of work done on chronic congestive heart failure while acute heart failure has been considered a difficult to handle and hopeless syndrome. However, in recent years acute heart failure has become a growing area of study and this is the first book to cover extensively the diagnosis and management of this complex condition. The book reflects the considerable amounts of new data reported and many new concepts which have been proposed in the last 3-4 years looking at the epidemiology, diagnostic and treatment of acute heart failure.

Available in PDF, EPUB and Kindle. Book excerpt: Arterial hypertension, coronary heart disease and heart fail ure are the commonest cardiovascular conditions to present in clinical practice. Over the past few years it has become in creasingly clear that they are closely and causally interrelated and that their relationship can have a significant bearing on prognosis. Epidemiological studies have shown that arterial hypertension is one of the most important risk factors for de veloping heart failure. Only one in four patients with hyper tension is adequately managed, and in 50% of cases, the hypertension has not been recognised or treated. Patients with pre-existing hypertension who go on to suffer an acute myocardial infarction have usually not previously had typi cal angina symptoms, the infarct territory is larger, life threatening arrhythmias are commoner and hence in-hospi tal mortality and long-term prognosis are markedly worse. The presence of raised blood pressure in the post-infarct phase doubles the risk of manifest heart failure. The close relationship between hypertension, coronary heart disease and heart failure makes the choice of therapeu tic strategy particularly important. Agents and classes of agents that have prognostic value in all three conditions should be considered first, as synergy might result in addi tional benefits. In such patients, this sort of therapeutic deci sion-making might have further advantages. The use of these agents may prevent complications which are not yet clinically obvious (such as heart failure).

Available in PDF, EPUB and Kindle. Book excerpt: The bicentenary of William Withering's now famous medical report entitled "An Account of the Foxglove and Some of its Medical Uses" has given us the occasion to analyze the state of the art. Cardiac glycosides in 1985 are considered tobe the basis for medical treatment of myocardial failure, together with diuretics and vasodilators in the more severe cases. Nevertheless, the controversy as to their exact place in the treatment of heart failure with sinus rhythm has never ceased. Although cardiac glycosides are of unquestionable value in tachycardia caused by atrial fibrillation or atrial flutter, the chronic use of these drugs in sinus rhythm is not generally accepted. The development of tolerance has been reported. It is of interest that Withering hirnself wrote: "-let it (foxglove) be continued until it either acts on the kidneys, the stomach, the pulse, or the bowels; Iet it be stopped upon the first appearance of any of these effects, and I will maintain that the patient will not suffer from its exhibition, nor the practitioner be disappointed in any reasonable expectation. " In West Germany alone, more than three million patients (about 5% of the total population) are constantly taking cardiac glycosides. These drugs are the most prescribed medicaments in many countries. Thus, we considered it worthwhile and necessary to review in a critical way today's knowledge about the foxglove; to report new findings and to evaluate old Statements.

It comprises a series of 'cutting edge' reviews of the most advanced treatment concepts available in the modern ICU. Whilst assuming a basic knowledge of underlying conditions, it nonetheless outlines key physiological principles where necessary, and critically reviews current literature and best practice. The heart of the book is aimed at providing key practical information on treatment techniques to the busy clinician in an easily accessible style. In addition to conventional drug therapy, ventilator, fluid and physical therapies are also discussed in detail. All of the authors are directly involved in ICU research and practice and are familiar with all of the latest developments in this fast-moving field of medicine.
