Available in PDF, EPUB and Kindle. Book excerpt: Designed to bring all of orbital anatomy into perspective, this expert reference is the first to: 1) Provide a comprehensive review of the microsurgical anatomy of the orbit and sellar region; 2) Demonstrate the relationship of the orbit and surrounding structures; and 3) Illustrate orbital structures from multiple operative approaches Hundreds of vivid dissections show the orbit from above, below, laterally, medially, and anteriorly, with illustrations fully labeled for valuable review and study. The organization of THE ORBIT AND SELLAR REGION leads to clarity and comprehension. Divided into three sections, the book begins with a full description of osseous, neural, arterial, venous, and muscular anatomy. It then goes on to stepwise dissections of the orbit from different directions, in which each layer is peeled away to expose the next deeper layer and the placement of the orbit and concludes with multiple common operative approaches to the sellar region.

Available in PDF, EPUB and Kindle. Book excerpt: The sellar region and paranasal sinuses constitute the anatomical sections of the skull base in which pathological entities warrant interdisciplinary management. Processes originating in the paranasal sinuses can reach and involve the skull base in and around the sella, sometimes not respecting the natural dural boundary. On the other hand, lesions involving the sellar block, such as pituitary adenomas and meningiomas, can also extend downwards into the paranasal sinuses. The orbit and cavernous sinus may be subject to involvement and infiltration by both paranasal and sellar pathology. The advancement and new achievements of modern diagnostic procedures, such as high-resolution CT, three-dimensional reconstruc tion, MRI, and MRI angiography, as well as the detailed selective angiographic protocols and endovascular techniques, have increased the possibilities for surgical management of this type of pathology with extra- and intracranial involvement. Long-standing and intense inter disciplinary work has led to sophisticated operative approaches which for benign tumors allow total excision with preservation of structures and function, and for some malignant lesions permit an en bloc resec tion via a combined intracranial-extracranial approach. This volume reflects the work and scientific exchange which took place during the IV International Congress of the Skull Base Study Group, held in Hanover. Leading authorities in the basic sciences including anatomy joined with diagnosticians, clinicians, and surgeons from different fields to evaluate the state of the art of this topic in skull base surgery.

Available in PDF, EPUB and Kindle. Book excerpt: This OpenAccess - textbook sheds new light on pathology in and around the orbit, which is typically an area where many medical disciplines overlap. Each physician brings a specific expertise, but the goal should be that the end result of all this input is much more than the sum of the parts. Collaboration, insight and overall knowledge of all parties involved are essential to achieve an optimal patient outcome. Oral and Maxillofacial surgeons have traditionally focused on the bony parts of the orbit, but usually have limited knowledge of the intricacies of binocular single vision, and for the Ophthalmologist it may be the other way around. In the past, scientific articles were often written from a single point of view, resulting in tunnel vision for the physician. The multidisciplinary approach to the orbit leads to an improvement in treatment that should not be underestimated and from which the patient benefits immensely. But especially for the physician and the trainees, a multidisciplinary consultation is a real goldmine in which a lot can be gained. Not every medical problem needs to be tackled in a multidisciplinary framework, but problems around the eye socket often do. At Amphia Hospital in Breda, the Netherlands, more than fifteen years ago, a collaboration between Oral and Maxillofacial Surgeons, Endocrinologists, Radiologists and Ophthalmologists was initiated, which has proven to be very fruitful. The main focus was on Graves' Orbitopathy and orbital fractures, but problems associated with these conditions were also frequently encountered. For those who want to follow this example, or for those who are interested in both orbital surgery, anatomy or ophthalmological orbital issues, this book could prove to be complementary.

Available in PDF, EPUB and Kindle. Book excerpt: This book is designed to have a comprehensive review of the spectrum of diseases involving orbital apex and periorbital skull base and the up-to-date advancement in different treatment modalities. Management of diseases at the orbital apex and periorbital skull base has always been a challenge. Multiple specialties are involved, including skull base neurosurgeon, oculoplastic ophthalmologist, otorhinolaryngologist, head and neck surgeon, oncologist, neurologist and radiologist. However, frequently it results into a “no-man’s land”, as no single specialty is entirely familiar with this complex and overlapping anatomical territory. Cranial nerves, carotid artery, and cavernous sinus are just one of the few examples of important anatomical structures that pass through. However, this has often been managed by one specialty especially during surgical planning and operation, resulting in biases in choices of approach and surgical strategies. We believe that this interesting yet complex region deserves special attention with a well-orchestrated multi-disciplinary effort. Traditionally, surgical treatments for diseases in this region involve different types of craniotomy and orbitotomy. In this book, it covers the advancement in imaging modalities, medical therapies, operative instruments, radiation therapy namely stereotactic radiosurgery or radiotherapy, management of diseases in orbital apex and periorbital skull base evolve and improve with time. Minimally invasive surgery in terms of mastering neuro-endoscopy contributes to the intervention advancement.

Each case description commences with a concise clinical scenario. High-quality radiologic, laboratory, and histopathologic images depicting the differentiating features of the lesion subtype in question are then presented, and key operative and clinical management pearls are briefly reviewed. The interdisciplinary nature of this easy-to-use color atlas and textbook reflects the fact that the management of patients with sellar and parasellar lesions is itself often interdisciplinary. The format is unique in that no similar interdisciplinary book is available on lesions of this region of the brain. Atlas of Sellar and Parasellar Lesions: Clinical, Imaging, and Pathologic Correlations is of great value for practitioners and trainees in a range of medical specialties, including radiology, neurology, endocriniology, pathology, oncology, radiation oncology, and neurosurgery.

Available in PDF, EPUB and Kindle. Book excerpt: The management of vascular and tumorous lesions of the parasellar region still remains one of the most demanding tasks in neurosurgery. It is only a short time ago that the major concepts of the anatomy of the so-called cavernous sinus were described in detail. Surgical interventions in this region are very complex, they are time-consuming and require an extensive back ground of experience in surgery of the cranial base. Pioneer anatomical studies of the parasellar region done by Taptas, and the daring direct operative approach introduced by Parkinson pro moted the development of modern neuroradiological intervention proce dures, which were initiated by Serbinenko and further refined by Debrun, Vifiuela and others. The technique of the detachable balloon catheter stimulated surgeons to proceed with the direct operative approach to lesions of the parasellar region. Today, it is hard to imagine a successful man agement of vascular pathologies of this region without a complementary use of the two techniques.

Available in PDF, EPUB and Kindle. Book excerpt: Neurosurgery is a rapidly developing and technically demanding branch of surgery that requires a detailed knowledge of the basic neuro-sciences and a thorough clinical approach. The Oxford Textbook of Neurological Surgery is an up-to-date, objective and readable text that covers the full scope of neurosurgical practice. It is part of the Oxford Textbooks in Surgery series, edited by Professor Sir Peter Morris. The book is split into 20 overarching sections (Principles of Neurosurgery, Neuro-oncology of Intrinsic Tumours; Extra-axial Tumours and Skull Lesions; Cerebro-Pontine Angle Tumours; Sellar and Supra-Sellar Tumours; Posterior Fossa Tumours; Pineal tumours; Uncommon Tumours and Tumour Syndromes; Neurotrauma and Intensive Care; Vascular Neurosurgery; Principles of Spinal Surgery; Spinal Pathology; Spinal Trauma; Peripheral Nerve Surgery; Functional Neurosurgery; Epilepsy; Paediatric Neurosurgery; Neurosurgery for Cerebrospinal Fluid Disorders and Neurosurgical Infection). Each section takes a dual approach with, 'Generic Surgical Management' chapters that focus on specific clinical problems facing the neurosurgeon (e.g. sellar/supra-sellar tumour, Intradural Spina Tumours etc.) and 'Pathology-Specific' chapters (e.g. Glioma, Meningeal Tumours, Scoliosis and Spinal Deformity, Aneurysm etc.). Where appropriate, this division provides the reader with easily accessible information for both clinical problems which present in a regional fashion and specific pathologies. The generic chapters cover aspects such as operative approaches, neuroanatomy and nuances. Specifically each chapter in the book incorporates several strands. Firstly the fundamental neuroscience (anatomy, pathology, genetics etc.) that underlies the clinical practice. Secondly, a review of the requisite clinical investigations (e.g. angiography, electrodiagnostics, radiology). Thirdly, a thorough evidence based review of clinical practice. Following this a consideration of the key debates and controversies in the field with 'pro-' and 'con-' sections (e.g. minimally invasive spine surgery, microsurgical treatment of aneurysms) is provided. A summary of the key papers and clinical scales relevant to neurosurgery form the concluding part. The book is a 'one-stop' text for trainees and consultants in neurosurgery, residents, those preparing for sub-specialty exams and other professionals allied to surgery who need to gain an understanding of the field. It acts as both a point of reference to provide a focussed refresher for the experienced neurosurgeon as well as a trusted training resource.

The first part of the book addresses epidemiology, anatomy, radiological diagnosis and innovations, fracture classification, fracture mechanisms, and symptoms. The second part then focuses on treatment, explaining operative principles and providing step-by-step descriptions of a variety of hard and soft tissue reconstructive procedures. Here, individual chapters are devoted to neurosurgical management, surgical repair of fractures, methods of dural and skull base treatment, osteosynthesis, intraoperative navigation techniques and systems, reconstruction with bone grafts and alloplastic materials, and complications and late sequelae. The book concludes by discussing surgical strategy in complex craniofacial trauma care and presenting a treatment algorithm that takes into account new developments. Craniofacial Trauma will be an indispensable reference for residents in maxillofacial training and for maxillofacial/neurosurgeons in the specialized field of craniofacial traumatology.
