Book excerpt: After the publication of the AO book Technique of Internal Fixation of Fractures (Miiller, Allgower and Willenegger, Springer-Verlag, 1965), the authors decided after considerable discussion amongst themselves and other members of the Swiss AO that the next edition would appear in three volumes. In 1969, the first volume was published (the English edition, Manual of Internal Fixation, appeared in 1970). This was a manual of surgical technique which discussed implants and instruments and in which the problems of internal fixation were presented schematically without radio logical illustrations. The second volume was to be a treatise on the biomechanical basis of internal fixation as elucidated by the work done in the laboratory for experi mental surgery in Davos. The third volume was planned as the culminating effort based upon the first two volumes, treating the problems of specific fractures and richly illustrated with clinical and radiological examples. It was also to discuss results of treatment, comparing the results obtained with the AO method with other methods. The second and third volumes were never published. The second edition of the AO Manual appeared in 1977. It dealt in greater detail with the problems discussed in the first edition, although it still lacked clinical exam ples and any discussion of indications for surgery. Like the first edition, it was trans lated into many languages and was well received. Finally, after 22 years, the much discussed and much needed third volume has appeared.

Book excerpt: The why, where, when and how of modern fracture treatment, written by two world-renowned experts. The first edition sold over 5,000 copies in the US alone and soon became a standard reference. This completely revised and enlarged second edition takes into account all the important advances that have taken place since. It is richly illustrated with clinical and radiological examples, and describes how to assess, diagnose and classify fractures, together with the relevant treatment in each case. Readers are offered advice on the daily practice of dealing with fractures, including the surgical approach, selection of the best implant, avoiding common pitfalls and the importance of post-operative care. Unparalleled in its coverage of the pelvis and acetabulum in addition to the upper and lower extremities.

Book excerpt: The purpose of this book is to report on my 17 years of experience in the development and implementation of a closed functional method of treatment of certain fractures of long bones of the appendicular skeleton. My personal experiences, clinical results, and the basic concepts underlying the treatment philosophy are reported. Illustrations demonstrating the techniques of application of functional casts and braces are presented in detail as well as a step-by-step description of the management regime and a discussion of the behavior of fractures under this system. Indications and contraindications for the use of this method of treatment are clearly outlined. I consider it important to compile this information for I believe that the system has withstood the test of time and is sufficiently sound to be incorporated in the armamentarium of the orthopaedic surgeon. Also, it may have a favorable socioeconomic impact since it offers a viable alternative in the treatment of frac tures.

Book excerpt: It is to the great and lasting credit of LORENZ BOHLER and his school that they have in the last decade developed and demonstrated so thoroughly the techniques for the conservative management of fractures. Nevertheless there have always been many, including some from BOHLER'S school, who have found considerable place for surgical management, and with the significant progress in general surgery seen in postwar years, a new stimulus has been given to this part of traumatic surgery, especially since bone injuries have become more complex and frequent. The concept of internal fixation is not new. The serious criticisms that have been levelled at it retain today their basic significance. Progress in the fields of asepsis, corrosion-free metal implants, operative experience and postoperative care has diminished the dangers but has not relieved the surgeon of responsibility. The Association for the Study of the Problems of Internal Fixation (AO) has devoted itself over a number of years to the basic principles and best methods of open treatment of fractures by means of extended clinical and scientific studies in order to determine in each individual case the most promising line of treatment. At the same time a well designed and tested instrument set has been developed with precise instructions for the appropriate techniques. As a result, the new observations about primary bone healing which have emerged from the practice of rigid internal fixation are as interesting as the uses to which they can be put in allowing early mobilization.

My thanks are due to Mr. Shenton and others for help in connection with the radiographs, and to the printers for the care they have bestowed upon the typography. Available in PDF, EPUB and Kindle. Book excerpt: The term "orbital fracture" refers to any fracture that involves the bony cavity containing the eyeball. Such traumatic injuries of the ocular region are found in about 80% of cases of central facial fractures, and their spectrum ranges from a simple fracture of the cheek bone to a complex comminuted fracture of the orbit. The possible treatments begin with closed repositioning and end with extensive reconstructive surgery of the orbital region. The relatively recent introduction of new craniofacial or maxillofacial surgery techniques has resulted in considerable advances in both the primary management of severe orbital injuries as well as the treatment of posttraumatic deformities. This unique book is clearly not only essential reading in the specific field of orbital and oculoplastic surgery, but will also be of great practical value to all those working in cranial and maxillofacial trauma and plastic and reconstructive surgery, as well as to otolaryngologists, ophthalmologists, and radiologists.

Available in PDF, EPUB and Kindle. Book excerpt: The second English-language edition of the Small Fragment Set Manual was enthusiastically received and quickly went into a second printing. In preparing a third edition, we found it necessary to revise the text extensively and partly restructure it. The reasons for this are numerous. Experience of recent years has brought technical refinements in the operative treatment of many types of small fracture. Many of these changes stem from the small-fragment-set training programs conducted in Switzerland since 1980, and also from courses and symposia that have been held in other European countries and the United States. These events were occasions for a fruitful ex change of experience with surgeons who were critical of our methods. As a result of this exchange, we perceived a need both to revise our indications and to give greater attention to alternative techniques. We also felt it necessary to respond to criticisms of the first two editions concerning the catalog-like instrument lists and illustrations, and the attention given to fundamental techniques. Many surgeons who work or would like to work with small implants, especially those practicing abroad, are inexperienced in operations on the larger bones. It is imperative that these colleagues be given a basic introduction to the "biomechanical thinking" of the Swiss Association for the Study of Internal Fixation (ASIF).

Book excerpt: This book charts the history of the worldwide introduction of an operative treatment method for broken bones, osteosynthesis, by a Swiss-based association, called AO. The success of the close cooperation between the AO's surgeons, scientists and manufacturers in establishing a complicated and risky technique as a standard treatment sheds light on the mechanisms of medical innovation at the crossroads of surgery, science and industry and the nature of modern medicine in general.
