Book excerpt: The optic nerve is a white matter tract that conveys visual information from the eye to the brain. Inside the eye, the optic nerve head is exposed to ocular fluids, and behind the eye, the nerve is surrounded by cerebrospinal fluid (CSF). Ocular fluid and CSF are similar in composition and function, and are in close proximity at the optic nerve interface. Additionally, both fluids drain into cervical lymph nodes via the lymphatic system. Despite these common features, the relationship between these fluids and the optic nerve is poorly understood. Recent studies point to glymphatic paravascular entry of CSF into brain tissue to deliver nutrients and to clear metabolic waste. The hypothesis of this thesis is that ocular fluid and CSF enter the optic nerve via paravascular pathways and that this is relevant to glaucoma, the leading cause of irreversible blindness worldwide. My investigations reveal that ocular fluid flows into the optic nerve head by diffusion rather than via a paravascular pathway. In contrast, CSF flows into the optic nerve via paravascular spaces bordered by optic nerve capillaries and astrocytic endfeet containing aquaporin water channels. Furthermore, studies revealed size dependent entry of CSF solutes with preferential entry of smaller molecules. Additionally, CSF flowed into the optic nerve only up to the optic nerve head. Finally, in a spontaneous glaucoma model, studies revealed markedly impaired CSF flow into the optic nerve in association with nerve damage. These results are the first to show the intimate relationship of ocular fluid and CSF to the optic nerve. These novel findings will help us to understand the optic nerve in both health and in disease such as glaucoma.

Book excerpt: Prolonged microgravity exposure during long-duration spaceflight (LDSF) produces unusual physiologic and pathologic neuro-ophthalmic findings in astronauts. These microgravity-associated findings collectively define the Spaceflight Associated Neuro-ocular Syndrome (SANS). In this book, the editors compare and contrast prior published work on SANS by the National Aeronautics and Space Administration’s (NASA) Space Medicine Operations Division with retrospective and prospective studies from other research groups. The book describes the possible mechanisms and potential etiologies for SANS, and provides an update and review on the clinical manifestations of SANS including: unilateral and bilateral optic disc edema, globe flattening, choroidal and retinal folds, hyperopic refractive error shifts, and focal areas of ischemic retina (i.e., cotton wool spots). The ocular imaging findings (e.g., retinal nerve fiber layer, optic disc, and choroidal changes on optical coherence tomography) of SANS is also described, including the intraorbital and intracranial findings on orbital ultrasound and magnetic resonance imaging. The knowledge gaps for in-flight and terrestrial human research including potential countermeasures for future stud is also explored, including reports on the in-flight and terrestrial human and animal research being investigated by NASA and its partners to study SANS both prospectively and longitudinally and in preparation for future long duration manned missions to space including the moon, the asteroid belt, or Mars. We think this is a unique topic and hope that NASA and its research partners continue to study SANS in preparation for future longer duration manned space missions. Written in an easy-to-read manner, the book adopts a translational approach and explores the science and the clinical manifestations of Space flight associated neuro-ocular syndrome. Book excerpt: Understanding the structure and function of the blood-brain barrier (BBB) and recogniz ing its clinical relevance require a concert of scientific disciplines applied from a view point of integrative physiology rather than from only molecular or analytical approaches. It is this broad scope that is emphasized in this book. In my opinion, four original contributions define the field as it exists today. The first, a monograph by Broman,1 entitled The Permeability of the Cerebrospinal Vessels in Normal and Pathological Conditions, was the model for many subsequent clinical and 3 experimental studies on BBB pathology. Second, experiments by Davson, summarized in his book entitled Physiology of the Ocular and Cerebrospinal Fluids, indicated that passive entry of nonelectrolytes into brain from blood is governed largely by their lipid 4 solubility. This research supported the original suggestion by Gesell and Hertzman that cerebral membranes have the semipermeability properties of cell membranes. The modem era of the barrier was introduced with the 1965 paper by Crone,2 entitled "Facilitated transfer of glucose from blood to brain tissue. " This paper identified stereospecific, facilitated transport of glucose as part of a system of regulatory barrier properties at a time when only a barrier to passive diffusion had been contemplated. Finally, the 1967 paper by Reese and Kamovsky, 11 entitled "Fine structural localization of a blood-brain barrier to exogenous peroxidase," sited the barrier at the continuous layer of cerebrovascular endothelial cells, which are connected by tight junctions.
