Biodefense in the Age of Synthetic Biology explores and envisions potential misuses of synthetic biology. This report develops a framework to guide an assessment of the security concerns related to advances in synthetic biology, assesses the levels of concern warranted for such advances, and identifies options that could help mitigate those concerns.

Throughout this history, the military biological disease threats were relatively straightforward, there was little urgency linked to successful product fielding, there was no mechanism by which to gain Food and Drug Administration (FDA) product licensure, and there was little competition for mission or funds. In the post-September 11, 2001 (9/11) environment, however, the scope of potential threats has increased immeasurably, relative funding for the DoD has decreased, urgency to field solutions has skyrocketed, the FDA has provided a way forward to product licensure, and active non-DoD players in this arena have grown exponentially, aligning with newly designated, congressionally mandated funding sources. The old paradigms that governed the DoD research program structure and mission are no longer viable in this changing environment. This monograph examines the current organization of the DoD biodefense research program in light of the changing national biodefense landscape and industry best practices, and argues that all aspects of the DoD biodefense program should be consolidated with all other federal biodefense resources, including those within the National Institutes of Health, to create a single, focused, and productive program. This new agency, subordinate to the Department of Health and Human Services, will be positioned and equipped to provide medical solutions to the warfighter on the battlefield, as well as to U.S. citizens.

Smith III addresses this puzzling and largely untold story about science, technology, and national security.Smith argues that organizational frames and stereotypes have caused both military neglect and the rise of civilian biodefense. In these cases of natural or engineered disasters, such as a terrorist attack, influenza pandemic, or earthquake, state or local authorities can request that the federal government provide assets from the Strategic National Stockpile to augment the state and local jurisdictions' resources. The Centers for Disease Control and Prevention's (CDC's) Strategic National Stockpile (SNS) is the nation's repository of antibiotics, chemical antidotes, antitoxins, vaccines, antiviral drugs, and other medical materiel designed to supplement and resupply state and local public health agencies in the event of an emergency. The materiel is intended to support national health security and is managed by the Office of Public Health Preparedness and Response's (OPHPR's) Division of Strategic National Stockpile (DSNS). The stated mission of the SNS is to prepare and support partners and provide the right resources at the right time to secure the nation's health. The National Academies of Sciences, Engineering, and Medicine organized a two-day public workshop to explore opportunities to improve the efficiency, effectiveness, and sustainable methods used by the CDC's SNS to distribute medical countermeasures and other supplies during disasters and other public health emergencies, especially those which result in disruption of physical infrastructure such as the electrical grid, central roadways, bridges, and tunnels within the impacted community. Participants explored relevant distribution lessons learned from other federal agency stockpiles and the private sector as well as opportunities to develop public-private collaborations in the purchase, warehousing, management, and distribution of medical countermeasures. This report summarizes the presentations and discussions from the workshop.

Book excerpt: Environmental biodetection (BD) and human biosurveillance (BSV) are part of the foundation of national biodefense. The Department of Homeland Security's (DHS's) Science and Technology Directorate (S&T) chemical and biological defense (CBD) research and development (R&D) portfolio improves technologies and operations in both of these areas. In the context of evolving technologies, greater adversary capabilities, and unpredictable federal budgets, this report examines S&T's CBD R&D portfolio to identify priorities for future investments. The policy context includes multiple national strategies addressing biodefense, biosurveillance, health security, and national security; multiple federal agencies have missions and programming in this space. The R&D outputs of most CBD projects and most projects in illustrative inventories of non-S&T CBD R&D are not ready for deployment. S&T's CBD R&D has focused more on environmental BD than on human BSV. While S&T has supported some projects to develop tests for environmental or human samples, other federal agencies have supported more studies in this area. There is less R&D-supported by S&T or other sources-that targets system configuration or concept of operations (CONOPS). The recommendations are to (1) develop a DHS strategic plan for environmental BD and human BSV R (2) prioritize environmental BD over human BSV for future DHS R&D investments, given the department's unique role in this area; (3) prioritize R&D addressing CONOPS; (4) actively monitor R&D supported by other agencies, and adapt relevant outputs to meet DHS needs; (5) shorten the timeline for real-world application by prioritizing midstage and later stage R&D over early stage research.
