As to just how often such infections may be ascending or lymphogenous rather than hematogenous in origin there is a diversity of opinion. My own opinion is that such infections, other than hematogenous, are extremely rare so far as the kidney filter 15 concerned, though it is possible they may bey more frequent 111 that part of the kidney devoted to collecting urine. M. Despris, in a lecture which he delivered at La Charite (gazette Medica/e, Med. Times) made some interesting observations. A young woman, striking a table with the palm of her hand, thrust a needle into the base of her middle finger and this, striking against the first phalanx, broke and became fixed there. When seen two days afterwards, the fragment of needle had completely disappeared amidst the in amed tissues. On pressing at the base of the mid dle finger, however, a foreign body, pressure on which occasioned pain, could be felt An incision was made at this point, and a fragment of needle, measuring 1% centimetres, removed by the forceps. Here the incision was justified by the fact that the foreign body was firmly fixed. The subject of a second case was a young woman, into whose breast a needle was driven obliquely by a blow, and entirely disappeared under the skin. Guided by the patient the presence of the needle could be ascertained but in this case an incision for its removal would be im proper, for the integuments of the breast are so mo bile that an incision made in the skin would not correspond to the foreign body. The presence of the needle having been exactly determined, we should seize it in its length and make pressure on its two ends. At one of these we may perceive a slight cracking sound, and here the patient also feels a sharper pain than elsewhere, and this is the point of the needle. If we now press firmly upon the other end this point'will be forced through the integument and can then be seized with a forceps. An incision should never be made except when the body is fixed in the tissues, as in the first case. One caution must be borne in mind, and that is, we should never attempt an extraction on the mere statement of the patient that a needle is present in the tissues, and when we are unable, to verify its presence; for sometimes persons declare that they have needles in their tissues when they have not; or when we are consulted the needle may have already migrated to another part of the body, this migration sometimes taking place very rapidly. From experience in army base hospitals, two facts of importance have been rather universally recognized. The first fact refers to the general preva lence of the streptococcus (normally one of the hemolytic varieties) in pneumonia following measles. The second fact refers to the prevalence of the streptococcus, likewise one of the hemolytic varieties, as an organism associated in many localities with the pneumococcus in the production of epidemic pneumonia not pre ceded by measles or influenza, but as a distinct type in which the pneumococcus has appeared to play but a minor role. In certain localities, during the recent epidemic of influenzal pneumonia, the pneumococcus as the prevalent type of infection both in the acute disease and in the subsequent empyemas. In other localities the pneumococcus and streptococcus have been associated, while in others streptococcus pneumonias have been more frequently encountered than had been heretofore generally recognized. In the series of empyema here reported only those are included which required treatment by aspiration or operation. There were available for this report 35 empyema patients who recovered by repeated aspirations alone, while came to Operation. Those patients were not included who died within a few days of admission with an established diagnosis of empyema or those who died during the course of aspirations designed for their relief, but who, because of activity of the pneumonic process, acute bilateral empyema, purulent pericarditis, meningitis or other serious complications, were not considered reasonable operative risks. Has not this last war supported the assertion I have made that the flag of three stripes has been laid tenderly away, and in its place we have a flag of many stripes, one flag that waves over all of the United States, and justly so. Georgia is a great state, with its rivers and mountains and plains and all that is needed to make its future glorious. Georgia has been one of the states that has always had an Eclectic Medical Association and it has always done good work in the National. I can not go further without taking too much time, but I desire to say to you, Mr. Mayor, and to the President of the Georgia Society, that we people here today thank you for the address you have given us.
