Book excerpt: In 2010, the Philippines appeared to be on track to attain by 2015 its target for Millennium Development Goals 4 (Reduce child mortality), but less so for Goal 7 (Ensure environmental sustainability). In pursuit of the latter, the government expands its provision of water and services to more households. Applying propensity score matching technique on the data from the four rounds of a nationwide survey, such interventions are found to reduce the incidence of child diarrhea, a persistent top cause of child mortality, though not always. The impact of improved sources of drinking water is 1.3% to 2.6% in 1993 and 2.9% to 4.6% in 2003, but none is found in 1998 and 2008. The impact of improved sanitation is 1.2% to 2.1% in 1993 and 3.1% to 4.7% in 2008; but none is found in 1998 and 2003. In addition to health interventions, the regular monitoring of the quality of water and sanitation at the household level is suggested to achieve Goal 4. -- Water and sanitation ; child health ; MDGs ; Philippines

Book excerpt: Water, sanitation and hygiene (WASH) investments are widely seen as essential for improving health in early childhood. However, the experimental literature on WASH interventions identifies inconsistent impacts on child health outcomes, with relatively robust impacts on diarrhea and other symptoms of infection, but weak and varying impacts on child nutrition. In contrast, observational research exploiting cross-sectional variation in water and sanitation access is much more sanguine, finding strong associations with diarrhea prevalence, mortality and stunting. In practice, both literatures suffer from significant methodological limitations. Experimental WASH evaluations are often subject to poor compliance, rural bias, and short duration of exposure, while cross-sectional observational evidence may be highly vulnerable to omitted variables bias. To overcome some of the limitations of both literatures, we construct a panel of 442 subnational regions in 59 countries with multiple Demographic Health Surveys. This large subnational panel is used to implement difference-in-difference regressions that allow us to examine whether longer term changes in water and sanitation at the subnational level predict improvements in child morbidity, mortality and nutrition. We find results that are partially consistent with both literatures. Improved water access is statistically insignificantly associated with most outcomes, although water piped into the dwelling predicts reductions in child stunting. Improvements in sanitation predict large reductions in diarrhea prevalence and child mortality, but are not associated with changes in stunting or wasting. We estimate that sanitation improvements can account for just under 10% of the decline in child mortality from 1990-2015.

Each year, 1 in 10 child deathsroughly 800,000 in totalis the direct result of diarrhea. Can private-sector participation (PSP) in the urban piped water sector improve child health? The author uses child-level data from 39 African countries during 19862010 to show that introducing PSP decreases diarrhea among urban dwelling children under five years of age by 5.6 percentage points, or 35 percent of its mean prevalence. PSP also leads to greater reliance on piped water. To attribute causality, the author exploits time variation in the private water market share controlled by African countries former colonizers. A placebo analysis reveals that PSP does not affect symptoms of respiratory illness in the same children, nor does it affect a rural control group unaffected by PSP.

Book excerpt: Since the 1960s, it has been known that poor water and sanitation causes diarrhea, which consequently compromises child growth and leads to undernutrition. Ample evidence shows that poor water and sanitation causes diarrhea, but there is a growing body of knowledge discussing the magnitude of the impact of diarrhea on undernutrition. A recent hypothesis by Humphrey (2009), for example, states that the predominant impact of contaminated water and poor sanitation on undernutrition is via tropical/environmental enteropathy (triggered by exposure to fecal matter) rather than mediated by diarrhea. This new hypothesis has generated much debate, especially in the South Asia region, on the contribution of water and sanitation to the South Asian Nutrition Enigma. The region is characterized by unusually high rates of child undernutrition relative to its income level, as well as a slow reduction in undernutrition. Practitioners have struggled to decipher the reasons behind this 'anomaly.' This report provides a systematic review of the evidence to date, both published and grey literature, on the relationship between water and sanitation and nutrition. We also survey the potential impact of improved water, sanitation, and hygiene (WASH) on undernutrition. This is the first report that undertakes a thorough review and discussion of WASH and nutrition in Bangladesh. The report is meant to serve two purposes. First, it synthesizes the results/evidence evolving on the pathway of WASH and undernutrition for use by practitioners working in the nutrition and water and sanitation sectors to stimulate technical discussions and effective collaboration among stakeholders. Second, this report serves as an advocacy tool, primarily for policy makers, to assist them in formulating a multisectoral approach to tackling the undernutrition problem.

Our results show that uncontaminated household storage water and safe child stool disposal decrease incidence of child diarrhea by 16% and 23% respectively. In contrast, neighborhood concentration of pit latrine increases incidence of child diarrhea by 12%. The latter result casts serious doubt on the assumed health and social benefits of moving from open to fixed-location defecation. Creating open defecation free communities in rural areas is not enough to achieve the desired health benefits of sanitation. To protect rural households from the risk of contracting communicable diseases, existing pit latrines should be upgraded to make them safer to use - fly-proofed and hygienic. There is a need for appropriate policy actions to improve household drinking water quality and to change people's behavior towards safe sanitation practices. Increasing access to clean water supply and providing means for safe excreta disposal will bring significant health and social gains. Moreover, promotion of hygiene education campaigns about household water treatment, safe water storage and handling, washing hands with soaps at critical times, and adequately removing child feces from the domestic environment can also help ensure that people preserve good health in their household and their community.
