To get this book out at the earliest possible moment we have also cut the normal consultation between editors and authors to a minimum. One decision made in the interests of uniformity was to re place the various terms used by different writers by one, C. A. T. , consistent with the title of the Seminar of which the book is arecord. The organisers of ESCAT decided upon a largely didactic meeting with as little overlap as was practicable between contributions and a de termined attempt to view computerised axial tomography in the light of its clinical usefulness. As consequence, the purpose of this book is to provide a comprehensive introduction to the whole field for clin icians as weIl as radiologists. Book excerpt: The introduction of computed tomography in the diagnosis of pathological intracranial conditions has had considerable significance in cases of cranio cerebral injury. The decisive diagnostic advantage lies in the possibility of demonstrating both gross pathological change directly as well as secondary changes in normal brain structures. Computed tomography has proved its considerable worth, especially in evaluation of patients with craniocerebral injury and its sequelae. The capabilities of CT were quickly recognized and use of the technique spread rapidly. It is likely that CT will be available within a few years in all hospitals and clinics treating patients with craniocerebral injury. We believe it appropriate to present a detailed report on our experience with CT in 1800 cases of craniocerebral injury treated in the neurosurgical departments in Miinchen-GroBhadern and Berlin-Charlottenburg over a period of five years. Both acute posttraumatic complications and late sequelae are discussed extensively. A large number of illustrations is provided in order to facilitate the reader's introduction to CT diagnosis. The great interest in our conjoint study originally published in the German language, induced us to translate this book and to update the clinical material. It is a harmless and almost painless technique that does not require the introduction of contrast medium into the subarachnoid spaces. Its interpretation is based on the modifications of the epidural veins which do not themselves have any proven pathology. The longitudinal epidural veins are situated in the anterolateral angles of the spinal canal between the nerve root and the intervertebral disc. They conse quently represent a major anatomic landmark in the investigation of disc pathol ogy because they are compressed by a disc herniation before the corresponding nerve root is reached. The improvement of the techniques of lumbar phlebography by catheterization of the lateral sacral veins and of cervical phlebography by catheterization of the vertebral veins now permits opacification of the epidural venous system along the total length of the lumbar and cervical spine. Spinal phlebography can consequently be used routinely, primarily in disc pathology but now also in other fields of vertebromedullary pathology such as myelopathy, tumors, or stenosis of the spinal canal, where it provides new information as compared to the other techniques of investigation. The techniq ue and results of spinal phlebography in these various conditions are presented in this monograph which is an attempt, with the collaboration of several authors, to summarize current knowledge in this field. Only lumbar and cervical phlebography will be discussed in this monograph.

The male: female re lation was 2 : 1. Fifty percent were males in the 5th decade. About 20% had a congenital narrow spinal canal with an ap-diameter below the minimal standard values (3). The maximum of spondylogenic narrow ing was found between C4 and C6 in 2/3 of the cases. Results: good 44%, satisfactory 31%, unchanged 13%, deteriorated 12%, mortality O. The duration of the neurological symptoms before operation had no in fluence on the surgical results. Final neurological restitution was reached within 1 week to 3 months after the operation. The operative results in patients with congenital narrow spinal canal showed the same percentual distribution as the results of the entire group. The introduction of direct CT sections in horizontal and more recently in frontal or modified frontal planes, the use of reconstruction to indirectly obtain sagittal, parasagittal, and frontal CT images, and the visualization of the ventricular system, sulci, or cisterns with injection of metrizamide have led us to prepare this monograph. The full benefit of CT scanning can only be obtained from an accurate three-dimensional concept of anatomic structures of the brain including sulci, cisterns, ventricles, and deep nuclei. This may be achieved by studying in detail serial sections of the skull and brain in multidirectional planes. CT scanning, the single most important noninvasive diagnostic innovation in recent years, has widely changed the practice of neuroradiology. Indeed, neuroradiology remains a most fascinating field in the study of anatomy of the brain in vivo. The first part of this book is devoted to sagittal and parasagittal sections, the second part to frontal and modified frontal sections, and the final part to horizontal and modified horizontal sections of the skull and brain.
