Book excerpt: Fear of recurrence (FOR) is a common survivorship concern, even years after initial breast cancer diagnosis. Although past research has identified predictors of FOR, these predictors have largely been immutable demographic or disease/treatment-related factors examined almost exclusively in cross-sectional studies. However, cognitive formulations of FOR hold that cancer-related stimuli (both external and internal) exhibit day-to-day variability and induce coping responses that in turn explain within-person variability in FOR. Consequently, the aim of this study was to investigate predictors of daily variability in FOR, focusing on patients' daily experiences of both internal (e.g., the experience of physical symptoms) and external (e.g., receiving communications related to cancer treatment) cancer-related stimuli and their coping responses. Seventy-two women with recent diagnoses of breast cancer completed an initial cross-sectional questionnaire soon after surgery, as well as an online diary for 21 consecutive days at the completion of adjuvant treatment. Multilevel modeling suggested that the daily experience of cancer-related events influenced their experience of FOR, primarily through intrusive thoughts. These findings help to expand our knowledge of predictors of FOR in the everyday lives of cancer survivors.

Research on targetable mutations, determination of genetic “signatures” that can use multiple individual genes/pathways, development of targeted therapy, and insight into the value of new technology remains at the cutting edge of research in this field. We are soliciting papers that present new technologies to assess predictive biomarkers in cancer, original research (pre-clinical or clinical) that demonstrates promise for particular targeted therapies in cancer, and articles that explore the clinical and financial impacts of this paradigmatic shift in cancer diagnostics and treatment.

Book excerpt: Fear of cancer recurrence (FCR) is a top ongoing concern of breast cancer (BC) survivors and thus the focus of recent intervention development. According to prominent theory, certain events trigger FCR, which, in turn, leads to specific behavioral responses, including checking the body for signs or symptoms of cancer (Lee-Jones, Humphris, Dixon, & Hatcher, 1997). Links between triggering events, FCR, and checking behavior have not yet been studied in the context of daily life. The goal of this study was to examine whether FCR has a within-person link with daily checking behavior and whether FCR mediates the link between triggering events and checking behavior. Seventy-two early-stage BC survivors completed daily diaries over a 21-day period approximately five months after BC surgery. FCR, checking behavior, and triggering events were assessed each evening. Multilevel modeling results indicated that FCR predicted greater odds of same-day, but not next-day, checking behavior. Checking behavior had a positive but non-significant effect on next-day FCR. We found that daily FCR significantly mediated the same-day effect of triggering events on checking behavior. These average within-person effects varied substantially between patients and were not explained by negative affect. The results support the within-person sequence of triggering events, FCR, and checking behavior posited by guiding theory, and can inform FCR intervention development.

Demographic data such as age and clinical history data such as age at diagnosis were factors that were examined. A total of 36 participants were recruited from a college basketball event honoring breast cancer survivors and three breast cancer support groups in mid-Michigan. Participants completed the Concerns About Recurrence Scale (CARS) and a demographic and clinical history survey. The study was guided by the Uncertainty in Illness Theory. Pearson's correlations, ANOVA, t-tests, and regressions were used to explore the relationships between fear of cancer recurrence and the explored factors found in this study. The study limitations included a small sample size, ethnically homogeneous sample, and cross-sectional design. Fear of breast cancer recurrence is an ongoing concern for breast cancer survivors and there is a need for continued research. In addition to many survivors’ not receiving psychosocial care, prevention and health promotion activities often do not meet basic care recommendations. This study adds to sparse literature by describing the prevalence of FCR, testing its associations with validated mental health measures, and examining relationships between FCR and health behaviors in a U.S. population-based sample of post-treatment cancer survivors. Methods: This cross-sectional observational study used data on cancer survivors from the 2011 Medical Expenditure Panel Survey (MEPS) Experiences with Cancer Survivorship Supplement. Multinomial logistic regression was used to test the effects of individual characteristics and validated measures of mental health on FCR. Multivariable logistic regression was used to test the effects of FCR on health behaviors. MEPS survey weights were applied in all analyses to account for the survey design. Results: Overall, 34% of cancer survivors reported no FCR, 54% low FCR, and 11% high FCR.
