Book excerpt: This thesis is an investigation of methicillin-resistant Staphylococcus aureus (MRSA) and Clostridium difficile in community hospitals in southern Ontario, Canada. The first part of this thesis presents the findings of two longitudinal studies that describe the epidemiology of MRSA and C. difficile contamination in the environment of four hospitals. Overall, the prevalence of MRSA and C. difficile contaminated surfaces ranged from 2.5%-11.8% and 2.4%-6.4%, respectively. The majority of MRSA isolates were identified as spa type 2/t002; however, spa type 539/t034, a livestock-associated (LA) MRSA strain, was identified on several surfaces in two hospitals. For C. difficile, various ribotypes were identified in the hospital environment, including internationally recognized ribotypes 027 and 078. Chair backs, hand rails, isolation carts, and sofas were high risk surfaces for MRSA contamination. Cork surfaces and surfaces located in rooms exposed to a C. difficile patient were at a higher risk for C. difficile contamination. The second part of this thesis describes the utility of the temporal scan statistic for detecting MRSA and C. difficile clusters in a community hospital, using historical microbiological data. Clusters were investigated using molecular techniques and hospital records. Temporal factors (e.g., month, season, year) associated with these two pathogens were investigated using regression models. Several MRSA and C. difficile clusters were concordant with outbreaks identified by the hospital. Based on typing data, potential MRSA and C. difficile outbreaks and transmission events were identified that were not recognized by hospital personnel. The identification of specific time periods with significantly increased MRSA or C. difficile rates may have been associated with particular administrative and health events. The identification of MRSA and C. difficile on different surfaces in the hospital environment demonstrates that protocols are required to ensure that these surfaces are adequately cleaned and disinfected. Furthermore, the identification of LA-MRSA strains in the environment requires additional surveillance. The incorporation of the temporal scan statistic and molecular typing to standard surveillance methodologies are valuable tools for hospital personnel to evaluate surveillance strategies, understand transmission events, and focus investigations to address increases in the rate of MRSA and C. difficile cases in the patient population.

Book excerpt: The emergence of virulent, antibiotic-resistant Clostridium difficile ribotypes over the past thirty years has greatly concerned both the medical and research communities. As of 2010, C. difficile has surpassed methicillin-resistant Staphylococcus aureus (MRSA) as a cause of nosocomial infections and is now found in both healthcare settings and outside communities in the United States. Attendant-borne transmission in hospitals has been associated with the evolution of increased virulence in other nosocomial pathogens, but evolutionary theory has yet to be applied to the epidemiology and virulence of C. difficile. Differences between nosocomial C. difficile infections (CDI) and community-associated CDI (CA-CDI) mortality and ribotype distributions were evaluated in order to determine if attendant-borne transmission in hospitals and long-term care facilities favors highly virulent C. difficile ribotypes. Analysis of the current literature on C. difficile revealed that nosocomial CDIs were significantly more virulent and associated with greater mortality than CA-CDIs. Additionally, the data indicate that ribotypes 027 and 001/smz are particularly well-adapted for transmission in healthcare settings given their prominent role in nosocomial CDI but near absence in healthy C. difficile carriers in the outside community. These findings suggest that interventions that prevent attendant borne transmission may aid in the incidence reduction and virulence modification of C. difficile infections.

Book excerpt: Methicillin-resistant Staphylococcus aureus (MRSA) is a major nosocomial pathogen causing significant morbidity and mortality. The epidemiology of MRSA has continued to evolve since its first appearance more than three decades ago. Epidemic strains of these MRSA are usually resistant to several other antibiotics. During the past 15 years, the appearance and worldwide spread of many such clones have caused major therapeutic problems in many hospitals as well as the diversion of considerable resources to attempts at controlling their spread.It has been seen that the increasing prevalence of MRSA infections in the hospitals, other care centers and in the community has become a worldwide phenomenon. The widespread of the multi-drug resistant strains and the antibiotic clones of the bacteria is worrying as it complicates the diagnosis and chemotherapies. There is a need for the adequate policy framework on infection control that will reflect the current epidemiologic characters of MRSA as well as the strict implementation of such control program to checkmate the spread of MRSA infections. MRSA and now vancomycin resistance has also resulted in a steady decline in efficacy of these antibiotics. MRSA is no longer only an infection that is acquired in hospitals (HA-MRSA), although this remains a primary source of transmission. The incidence of HA and CA-MRSA infections as well as their prevalence varies considerably among countries. Some of the MRSA clones lineages are more frequently isolated than others owing to their survival and transmissibility. The HA-MRSA is endemic in many hospitals worldwide. The MRSA has markedly influenced the empirical therapy for Staphylococcal infections. Limited therapeutic options are available for the management of these infections. Most β-lactam antibiotics are ineffective against both HA and CA-MRSA. The HA-MRSA is usually MDR. Resistance to β-lactam drugs varies geographically and may change over time. The infections were treated with oral antibiotics including doxycycline, minocycline, clindamycin, trimethoprim-sulfamethoxazole, rifampicin and fusidic acid. Severe HA-MRSA infections demand intravenous vancomycin therapy. Transmission can be prevented by following infection control strategies and decolonization therapy. The key to MRSA control is the early treatment of MRSA infections and the following of good infection control practices. As only limited drugs are available for the treatment of MRSA, irrational use of antibiotics should be avoided and a rational antibiotic policy must be adopted.

It is one of the leading causes of healthcare associated, or nosocomial infections, particularly in intensive care units. Hospital acquired MRSA (HA-MRSA) has been a battle for inpatients since the 1960's. However, in the late 1990's, a new strain of MRSA emerged. It appeared outside of the hospital setting and has been termed, community associated MRSA (CA-MRSA). Presently, CA-MRSA has been found to be spreading into the healthcare system presenting a new obstacle for patients and hospitals to overcome. It has been suggested that employees play a role in transmission of MRSA to hospitalized patients. Since healthcare workers are at the interface between hospitals and the community, they may serve as a potential reservoir for spreading MRSA. However, there are a limited number of studies that investigate employee MRSA colonization and subsequent transmission to patients. This study seeks to provide molecular evidence supporting the likelihood that employees play a role in MRSA transmission to patients. Furthermore, with the implementation of a version of "search and destroy", an infection control strategy, we show how reductions of hospital-acquired infections are achieved using this method.

Most of the social sciences were represented, with contributions touching on sociology, management science, economics, psychology, environmental science, ecology, and linguistics. The workshop was organised in association with SimSoc, an informal group of social scientists who have arranged an irregular series of influential workshops on using simulation in the social sciences beginning in 1992. While the papers were quite heterogeneous in substantive domain and in their disciplinary origins, there were several themes which recurred during the workshop. One of these was considered in more depth in a round table discussion led by Jim Doran at the end of the workshop on 'Representing cognition for social simulation', which addressed the issue of whether and how cognition should be modelled. Quite divergent views were expressed, with some participants denying that individual cognition needed to be modelled at all, and others arguing that cognition must be at the centre of social simulation.
