This meeting followed the three earlier symposia on Menière’s disease, which have been organized every six years since 1981 in Düsseldorf, Boston and, most recently, in Rome. 1999 saw the bicentenary of Menière’s birth, and to commemorate this, François Legent organized a historical symposium on the different aspects of Prosper Menière’s life. Together with Michel Portmann, the two honorary presidents, Robert Kimura and Jean-Marc Sterkers, gave excellent lectures on the two main topics of Menière’s disease: on experimental endolymphatic hydrops, and the role of both vestibular neurectomy and sac surgery. Numerous papers were presented on research and clinical experiences, and were particularly devoted to the better understanding of both the development of experimental endolymphatic hydrops and the evolution of Menière’s disease. Attention was also drawn to the local treatment of Menière’s disease by the application of drugs to the round window, and furthermore, the indications for and results of surgery for incapacitating vertigo were well defined. Finally, we are pleased to say that this was a very successful meeting and we would like to express our thanks to all the companies, and in particular Solvay Pharmaceuticals, that supported this event. We are certain that the first international symposium of the new millennium on Menière’s disease, which is to be organized by David Lim in the USA, will be even more successful and fruitful than this one. Book excerpt: Several methods have been used to demonstrate the vasculature of different organs in man and other species. Many attempts to evaluate the precise microangioarchitecture of organ systems remained unproductive, others were controversial. The development of electron microscope in thirties opend new perspectives in researching microvascular systems. Transmission electron microscopy provided a two-dimensional view on microcirculatory system at higher magnifications, however, its standardization was delayed unnecessarily. The use of methyl methacrylate and related compounds for obtaining replicas of vascular beds, and their study in scanning electron microscope opened a new window in micromorphological research. For the first time, a three-dimensional image analysis of the vascular system was possible. The microvascular corrosion casting method has meanwhile attracted the interest of many contemporary scientists. Its application to medical and biological problems justify it to be used as a routine method for microvascular investigations. The first investigators who used this method, focused either on methodological details or they dealt with the normal microanatomy of organs. The advantages of this method in demonstrating pathological microvascular patterns are also evident.

Measurements have classically been made from electrodes placed in the outer ear canal, on the tympanic membrane, the round window niche, or inside the cochlea. Recent innovations have led to ECochG being used for exciting new purposes that drive clinical practice and contribute to the basic understanding of inner ear physiology. Cochlear implant recording electrodes can monitor the preservation of residual, low-frequency acoustic hearing, both in the operating room and post-operatively. ECochG measurements can quantify differential effects of inner ear surgery or other manipulations on vestibular and auditory physiology simultaneously. Various attributes of cognitive neuroscience can be addressed with ECochG measurements from the auditory periphery. These advances in ECochG provide a way to understand a variety of inner ear diseases and are likely to be of value to many groups in their own clinical and basic research.

The digital anatomy correlation used in this work is the natural continuation of efforts to apply the University of Colorado Visible Human data set to gastroenterology. The Visible Human data set was created by Dr. Vic Spitzer and colleagues at the University of Colorado and is currently housed at the universitys Center for Human Simulation. The data set consists of high resolution transaxial digital images captured as cadavers were abraded away at 1 mm or less depths. These images are compiled into blocks of data and each structure is identified. This information can be used to pull out and manipulate 3-D structures as well as allowing one to review planar anatomy in any orientation. Using the Visible Human dataset, one should be able to find a normal anatomy correlate to any image found during a EUS examination. However, as important as normal anatomy is, it is the abnormal features which are the crux of an EUS examination. Endosonographers are asked to define lumps, bumps, cysts to find correlates for symptoms and abnormal laboratory findings. Accuracy requires a tremendous amount of skill and experience. To help in this task, we have assembled chapters from a world-wide group of expert endosonographers. These authors have shared their insight and images to help the readers of this work better see and understand some of the complexities uncovered during a EUS evaluation.
