Book excerpt: "Purpose: The purpose of this systematic review was to evaluate the effectiveness of exercise alone vs. exercise combined with the use of bisphosphonates/hormone replacement therapy as an intervention to reduce bone loss in postmenopausal women. Methods: A comprehensive exploration of various databases was conducted from Nov.1, 2009 to June 22, 2010, yielding a total of 2,929 results. Key search terms included postmenopausal women, exercise, bisphosphonates, and hormone replacement therapy, and multiple combinations of these terms. Fourteen articles met the criteria for inclusion in this review. Results: Bone mineral density, muscle strength, and lean tissue mass can be increased and fat mass can be decreased through exercise and exercise combined with a bisphosphonates/HRT. Conclusion: Exercise is effective in increasing BMD, lean tissue mass, and muscle strength. Bisphosphonates/HRT combined with exercise does not have an additive effect for preventing bone loss in postmenopausal women. Book excerpt: A meta-analysis was done to determine what effect exercise has on the bone mineral density (BMD) of the lumbar spine, proximal femur, and distal radius in postmenopausal women, utilizing data from 18 studies. Treatment effects (TE) were calculated for each of the skeletal sites, aerobic and strength training, as well as methods of BMD measurement (dual energy x-ray absorptiometry and dual photon absorptiometry). The resulting TEs found exercise, in general, to significantly increase (p c 0.05) the BMD of the lumbar spine (0.73%), and the proximal femur (0.35%); however, the distal radius showed a decrease ( -0.91%) in BMD. When looking for the effect of aerobic versus strength training, significant (p c 0.05) increases in BMD were found in the spine and femur. Since positive changes were not seen in all sites, the benefits of exercise may be specific to the skeletal sites which the working muscles are attached. When the % change for exercise and control groups were analyzed separately, the exercise group increased 2.31% at the lumbar spine and 0.61% at the proximal femur. However, the exercise group decreased -2.21% at the radius. The control group decreased at the lumbar spine -1.09%, and at the femur -1.1 I%, however, increased at the radius 0.36%. The BMD % changes were significantly larger for the spine and femur, therefore, vigorous exercise along with resistance training may help slow the decrease in BMD as one ages.

Book excerpt: The results of randomized trials evaluating the use of early or adjuvant systemic treatment for patients with resectable breast cancer provide an eloquent rebuttal to those who would argue that we have made no progress in the treatment of cancer. Many of the tumors that we have been most successful in curing with chemotherapy and other newer forms of treatment are relatively uncommon. In contrast, breast cancer continues to be the single most common malignancy among women in the western world, is increasingly a cause of death throughout Asia and Third-World countries, and remains one of the most substantial causes of cancer mortality world wide. The use of mammography as a means of early detection has been shown to reduce breast cancer mortality by 25-35% among those popu lations in which it is utilized. The use of adjuvant systemic treatment in appropriate patients provides a similar (and additional) reduction in breast cancer mortality. Few subjects have been so systematically studied in the history of medicine, and it seems fair to conclude that the value to adjuvant systemic therapy in prolonging the lives of women with breast cancer is more firmly supported by empirical evidence than even the more conventional or primary treatments using various combinations ofsurgery and radiotherapy.
