Book excerpt: The overall objective of this research is to develop dietary methods to increase bone mass in young men and women in order to reduce the incidence of stress fractures during physical training and the incidence of osteoporotic fracture later in life. The study evaluates the efficacy and safety of two different types of dietary interventions to promote gain in bone mass at several skeletal sites in young Naval Academy Midshipmen. The dietary interventions optimize different nutritional factors, not just calcium intake, and enable us to examine the effect of maximizing all the nutrients essential for both bone matrix formation and bone mineralization under the conditions of usual dietary intake at the Naval Academy. The youngest Midshipmen were recruited because they have the greatest potential for bone accretion, consequently recruitment was coordinated with the initiation of the class of 2003. The beginning phase of recruitment and baseline measurement of bone mineral density parameters and estimates of dietary intake that are needed to randomize the subjects to treatment groups started in July 1999. To date, we have recruited and scanned approximately 100 Midshipmen. The two year dietary intervention phase will start in early 2000. Midshipmen will be randomized to groups consuming daily either a calcium supplement or a placebo and either a fortified protein and energy bar or its placebo. Contracts for the development and production of the two different dietary supplements and their respective placebos are under negotiation with the U.S. Army Combat Feeding Center, Natick Soldier Center, SBCCOM, Natick, MA.

Book excerpt: Osteoporosis is a condition involving decreased bone mineral density (BMD) and increased fragility of the skeletal system. Osteoporosis affects ^75 million individuals in the United States, Europe, and Japan. In the United States alone, hip fractures affect 500,000 individuals per year, and annual healthcare costs for osteoporotic fractures are approximately $l4 billion. A high peak BMD can prevent or delay the onset of osteoporosis and its complications. Exercise and diet may affect peak BMD by as much as 20 to 40% each and have been identified as the two most important controllable factors determining BMD. The current study investigated the effect of a calcium, vitamin D, and vitamin K supplement combination during initial military cadet exercise training on: BMD, stress fracture occurrence, hormones associated with BMD, and biochemical markers of bone turnover. Significant changes in BMD, either between the supplemented group or the unsupplemented group or across time for both groups were not found. The majority of participants (n = 22) had unexpectedly high levels of physical activity prior to enrollment, and the initial military exercise training program included only moderate levels of activity. Therefore, the exercise stimulus to bone was likely insufficient to promote gains in BMD, regardless of the nutrient supplement status. Although weight-bearing exercise among previously sedentary adult females can increase bone mineral density (BMD), cross-sectional studies suggest that high-intensity exercise among amenorrheic athletes may be detrimental to bone health. The long-term effects of athletic participation on BMD should be examined. Objectives. This study was designed to: (1) Determine the prevalence of varsity athletic participation among female university graduates who participated in a mail survey; and, (2) Compare BMDs of high school varsity female athletes with women who were sedentary during the same time period, while controlling for covariates. Methods. A survey was mailed to 7291 alumnae of 2 Southern California universities in the classes of 1987-1991. The survey was returned by 1900 (26.1 %), and 1373 (72.3%) consented to further contact. Unfortunately, it is common to see nutrition deficiencies in this population that may put them at risk for a variety of health issues including lower BMD. Purpose: To determine the influence of the type of sport on regional and total BMD and its association with dietary energy, vitamin D and calcium intake. Methods: Seventy-three female students from McGill University were evaluated (average age of 20.8 years old, height 167.4 cm, weight 62.3 kg, and BMI 22.4). Forty-four athletes of 4 different Varsity Teams: basketball (n=13), volleyball (n=11), figure skating (n = 13), and synchronized swimming (n=7); and 29 age-matched sedentary students took part in the investigation. Dietary intake, Vitamin D and Calcium intake were assessed using a 3-day Food Log and a FFQ (short version) then analysed with the Food ProcessorTM Software. Total Body (TB), lumbar spine (LS) (L1-L4) and femoral neck (FN) BMD were assessed by DXA scanning. A one-way ANOVA explored between-group differences. Book excerpt: Lifestyle factors, namely nutrition and physical activity, are known to be invol ved in bone mass acquisition during childhood to optimize peak bone mineral accr etion. Serum leptin, a marker of total body fat, and bone mass are also concorda nt in several respects. Few studies have investigated the relationship between s erum leptin and bone mineral in youth in the context of other mediators of bone mineral accretion. The purpose of the present investigation is to determine the impact of exercise, dietary calcium intake and leptin on bone mass and body composition in 363 heal thy Lebanese school children and adolescents (184 boys and 179 girls), aged 10-1 7 years. There was a sexual dimorphism in serum leptin concentrations with girls having h igher leptin levels than boys across all pubertal stages. In a linear regression model, after adjusting for height, puberty stages, lean mass, fat mass, exercis e, dietary calcium intake, vitamin D and IGF1, leptin was an independent predict or of BMD at the femoral neck, trochanter, total hip and total body in girls onl y. Physical activity was a consistent and independent predictor of BMC and BMD a t the 3 hip sites and total body in boys explaining 7% of the variability in BMD and 3.5% of the variability in BMC. Dietary calcium intake accounted for 5% of radius BMD variability in boys only. There seem to be a relationship between lifestyle factors, the hormonal milieu and bone mineral mass in puberty. These findings suggest the need for the develo pment of public health strategies in the alteration of diet and exercise lifesty les to optimize musculoskeletal health in youth. Results also show a distinct se xual dimorphism in the impact of leptin and lifestyle factors on musculoskeletal outcomes; the basis for these gender differences is unclear and deserves furthe r investigation.
