Book excerpt: This study examined the methods of preparing the enamel surface for orthodontic bracket adhesion in overcoming pellicle and possible fluid contamination from prebonding cleaning techniques and the effect of these treatments on the enamel surface and resultant bond strength. Shear bond strength testing, Raman linear mapping, and scanning electron microscopy was used to evaluate shear bond strength, depth of resin interface, and surface and cross-sectional characteristics respectively. Based on an analysis of variance (a=.05), there was a significant effect of pretreatment enamel conditioning prior to self-etching primer application on shear bond strength. This difference was found to exist solely between the pumice prophylaxis group and the 34% phosphoric acid etch group. No difference was found to exist across all groups with regards to depth of resin penetration. Scanning electron microscopy showed no significant differences in enamel surface or resin/enamel interface characteristics. Results suggest that enamel cleaning techniques prior to SEP application are unnecessary and even deleterious to bond strength and resin penetration.

Book excerpt: INTRODUCTION: The objective of this study was to determine the effect of phosphoric acid etching prior to application of self-etching primer on orthodontic bonding in terms of shear bond strength and the adhesive remnant on the enamel surface at debonding. METHODS: Fifty-one extracted human premolars were pumiced, rinsed, dried, and then subjected to three different treatments: Group 1: no phosphoric acid etching + Transbond Plus SEP; Group 2: 15-second etching with 37% phosphoric acid + rinsed + dried + Transbond Plus SEP; Group 3: 30-second etching with 37% phosphoric acid + rinsed + dried + Transbond Plus SEP. Precoated metal brackets (3M Unitek, APC Plus, Victory brackets) were then bonded following the manufacturer's recommendation. An Instron Universal Testing Machine was used to test shear bond strength (SBS) at a crosshead speed of 0.5 mm/min. Adhesive remnant index (ARI) was assigned to all specimens under a 16 times magnification using a light microscope. RESULTS: One-way repeated measured analysis of variance test (ANOVA) revealed no significant differences in shear bond strength among all three groups. Kruskal Wallis test revealed that there was a significant difference in ARI score between Group 1 when compared to Groups 2 and 3. Two-way analysis of variance was used to examine any statistical difference in frequency of enamel fracture craters among the three groups. CONCLUSION: There was no statistical difference in shear bond strength between the three groups. Most specimens demonstrated adequate shear bond strength (>/= 6 MPa) for routine clinical application. Etching enamel with phosphoric acid prior to the use of Transbond Plus SEP resulted in more adhesive remaining on the enamel surface. The removal of this excess surface cement may lead to increased enamel loss during post debonding clean up.

It was found that self-etching primers not only reduce the chair time in an orthodontist's clinic but also reduce the amount of enamel lost during etching. The bond strength achieved was less than that of the conventional bonding system but was acceptable for orthodontic purposes. Thus self-etching primers are an important and useful invention for simplifying the most critical step in orthodontic clinic- bonding.

Debonding force was measured with an Instron universal testing machine. A two-way ANOVA comparing the four groups indicated that there was a significant difference in debonding force (P = 0.001). The SEP without prior pumicing group (17.69 " 7.18 MPa) was statistically different from the SEP group with prior pumicing (25.82 " 6.84 MPa). There was no statistical difference found among the other groups. Differences in the Adhesive Remnant Index (ARI) were analyzed by chi-square. ARI scores differed significantly (P =0.0048).

Fluoride releasing sealants has also been used commonly to prevent decalcification during orthodontic treatment. The purpose of this study was to compare the shear bond strength (SBS) of APC precoated brackets bonded with two different self-etching primers: Transbond Plus self-etch primer (TPSEP) and Reliance self-etch primer. This study also evaluated the effect of combining TPSEP or the Reliance SEP with LED Pro Seal, a fluoride releasing sealant for the enamel surface. Each of the self-etching primers (SEPs) and the combination of SEP combined LED Pro Seal were evaluated for shear bond strength after 24 hours of storage and thermocycling with 1 week of storage, in order to simulate some of the temperature changes in the oral cavity.^Materials and Methods: One hundred and twelve teeth were randomly divided into eight groups as follows: Group 1: Transbond Plus self-etch primer (TPSEP), Group 2: Reliance self-etch primer (Reliance), Group 3: TPSEP and LED Pro Seal, Group 4: Reliance and LED Pro Seal, Group 5: TPSEP with thermocycling, Group 6: Reliance with thermocycling, Group 7: TPSEP and LED Pro Seal with thermocycling, Group 8: Reliance and LED Pro Seal with thermocycling. An occlusogingival load was applied to the brackets producing a shear force at the bracket-tooth interface utilizing a Universal Testing Machine. Results: Significant differences in shear bond strengths among the groups containing the TPSEP or Reliance SEP, were detected by two-way ANOVA (p = 0.018). The results of the Newman-Keul's test that compared the test groups at 24 hours show that the shear bond strength of Reliance SEP (3.98 MPa " 1.30) was not statistically different from TPSEP (4.25 MPa " 1.16).^When each of the SEP is combined with the Pro Seal, a statistical difference in shear bond strength is detected between TPSEP (6.01 MPa " 3.05) and Reliance SEP (3.76 MPa " 1.52). Reliance SEP used alone vs. Reliance SEP combined with Pro Seal did not show any statistical difference in shear bond strength. However, when TPSEP was compared to TPSEP combined with Pro Seal, it showed a statistical difference in shear bond strength. Nonetheless, a two-way ANOVA did not detect any statistically significant interaction between the SEPs and the Pro Seal (p = 0.058). None of the groups showed statistically significant differences in shear bond strength when compared at the one week group. Conclusion: The newly introduced Reliance SEP provides similar shear bond strengths to Transbond Plus SEP. Reliance SEP with LED Pro Seal and Transbond Plus SEP with LED Pro Seal both provide adequate shear bond strengths.^Both SEPs used alone or with LED Pro Seal maintained its bond strength after thermocycling. LED Pro Seal is demonstrated to have good bond strength to enamel and can withstand the oral environment.

Book excerpt: The purpose of the current study was to develop a bonding method that can achieve clinically acceptable bond strength values while leaving a minimum amount of adhesive on the tooth surface during the debonding process. One hundred teeth were randomly assigned into groups. Five different enamel surface preparation protocols were tested (N = 20, each): Conventional acid etch, standard SEP, SEP applied with a light brush stroke (altered SEP 1), SEP applied directly to the composite of pre-coated bracket (altered SEP 2), and Primer Only groups. Brackets were debonded using an Instron universal testing machine (Instron, Canton, MA) in shear mode and the mean shear bond strength values were calculated. In addition, enamel surfaces were examined under light microscope to determine the location of failure using ARI. The Primer Only group had significantly lower mean shear strength (0.14 MPa, P0.05) than the conventional acid etch (13.81 MPa), standard SEP (12.10 MPa), altered SEP 1 (10.80 MPa), and altered SEP 2 (11.48 MPa) groups. The conventional acid etch, standard SEP, altered SEP 1 and altered SEP 2 groups were not significantly different from each other (p0.05). With respect to ARI values, there was a significant difference among these four groups. 85% of samples in the altered SEP 2 group had 10% or less composite resin left on their surfaces. This group had also a mean shear bond strength value of 11.43 MPa, significantly above the minimal strength needed for orthodontic attachment bonding, and the lowest ARI values overall. Therefore, application of SEP directly to the composite resin of the pre-coated brackets may be an ideal bonding method by providing adequate bond strength and leaving a minimum amount of composite resin on the tooth surface during debonding. It should be kept in mind that future in-vivo studies would be needed to confirm the findings obtained from the current in-vitro study.

Metal orthodontic brackets were bonded to the primed surface with resin adhesive (Transbond XTTM) and light cured. To simulate intraoral conditions, all primer and bonding protocols took place in an environmental chamber set at 33±2°C and 75±5% humidity followed by a 24-hour dark cure at 37 C. All rights not granted by the above license are retained by the author. Abstract: Enamel acid etching has been studied for almost 60 years, i.e., since successful enamel adhesion was first achieved. It was proposed to be due to the micro-mechanical interlocking based on the use of phosphoric acid to etch the so-called ideal etching pattern etching, at certain conditions. According to the systematically screened 2500 non-duplicated publications, with the inclusion of 175 laboratory studies and 18 clinical trials, phosphoric acid etching was found and is still considered to produce promising bonding on intact enamel surfaces. However, other etching agents, such as poly(acrylic acid), self-etching primers (SEP), non-rinse conditioners and other acids could also contribute to satisfactory bond strengths without creating too high surface roughness, etching depth and/or morphological alteration. Thus, an ideal etching pattern might not be necessary in enamel adhesion. An unduly use of ideal etching pattern in literature to assess the enamel adhesion seems more prone to the flaw observations and vague conclusions. Therefore, a fundamental understanding of enamel adhesion is vital. In this thesis, an ultra-structural investigation based on chemical composition, crystallography, micro-roughness, morphology, micro-hardness and surface tension, on intact human mature enamel with various pretreatment methods were studied, with an ethical approval (HKWC-HKU Cluster REC/IRB document No.: UW13-262). The current study found that phosphoric acid etching by liquids and gels could remove the superficial aprismatic enamel layer. The acid etching could also generate a rougher, softer and porous enamel surface, whilst commercial SEPs that contain 10-methacryloyloxydecyl dihydrogen phosphate (10-MDP) and 2-(methacryloyloxy)ethyl phosphates (HEMA-phosphates) left excess amounts of phosphates onto enamel surfaces and showed a minor demineralization effect. Furthermore, mono-HEMA-phosphate solutions could demonstrate both mild etching and a chemically reactive effect on the intact enamel, particularly when at 40 v/v%. All the pretreatment methods used in this study could effectively increase enamel surface free energy, i.e. physical adsorption deemed to be the critical factor rather than micro-mechanical adhesion in enamel adhesion. Apparently, the measurement of surface free energy was probably more appropriate than morphological examination to evaluate the enamel surface pretreatment method. In addition, due to the insignificant etching effect, the definition of "self-etching" might need to be re-considered. To conclude, the current ultra-structural investigation on various intact human enamel pretreatment methods may offer new insights about the basics and fundamental theory for enamel adhesion.
