Book excerpt: The purpose of the study was to determine if there were any significant differences in perceived mood states between patients in hospice care who received music therapy services and patients in hospice care who did not receive music therapy services. Twenty-eight participants were surveyed using the Profile of Mood States (short form). Fourteen participants received music therapy services for a minimum of five hours, and fourteen participants did not receive music therapy services. The participants were matched in pairs by gender, location, diagnosis, and age, and were patients in one of three hospice locations in the United States which agreed to participate in the study. Staff at each hospice location was responsible for choosing participants, securing informed consent and administering the surveys. Results of paired t-tests revealed that there were no significant differences between the groups. Two factors approached significance, however. The participants who received music therapy had lower scores on the anger scale and higher scores on the fatigue scale than the non-music therapy participants.

Book excerpt: The purpose of this study was to investigate the effects of music therapy interventions on grief and spirituality of family members of patients in a hospice setting. The subjects (N = 60) were the family members or significant others of hospice patients who had been in the hospice program for at least 2 weeks. The subjects were divided into a control group (N = 30), the family members of patients previously with no music therapy visit, and an experimental group (N = 30), the family members of patients receiving music therapy interventions. Three imminent patients were also included in both groups. A self-report questionnaire was used for a post-test only in this study and had 5 categories: grief, coping strategies, spirituality, satisfaction with hospice care, and satisfaction for family members' or significant other's perceptions of the loved one's quality of life (QOL). A multivariate analysis of variance (MANOVA) was applied to analyze data in each subtest. There was a significant difference in mean scores of QOL between the control and experimental groups. Although the mean scores of grief and spirituality in the experimental group were higher than the scores of those with no music therapy, a significant difference was not found in the 4 subtests. However, there was a tendency that those who received music therapy showed higher scores than those who did not in each subtest. In addition, the mean scores of family members of imminent patients were much higher than those with no music therapy in all 5 categories. The results in this study support that music has a potential to provide a positive influence for patients and their family members in a hospice setting. Further quantitative research for family members of imminent patients were suggested to provide better music therapy services in a hospice and palliative care.

However, each control subject completed the Hospice Quality of Life Index- Revised (see Appendix D) questionnaire and a self-report using a Visual Analog Scale (VAS) (see Appendix B) that measured anxiety levels. There was no change in the usual procedure of hospital care for those subjects. The self report questionnaire from the Visual Analog Scale, (VAS) showed significantly lower anxiety scores for the experimental subjects then for the control subjects. The Family Satisfaction Survey filled out only by the experimental subjects, also showed a 97% satisfaction of music therapy and its uses in the medical setting. There were no significant differences between groups for quality of life measure.

The complexity of existential pain and suffering in hospice patients is a challenge for a hospice care team to treat and manage and emotionally testing on the patient's support system. The purpose of this scholarly project is to evaluate a music therapy program at a local community hospice center as an aid to relieve patients' existential pain and suffering they incur from their terminal illness and to increase hospice care providers' knowledge about incorporating music therapy as an adjunct healing modality. Bearing witness to patient experience occurred in collaboration with a board-certified music therapist during patient therapy sessions. Jean Watson's Theory of Human Caring Science provides a theoretical framework for developing an education session on music therapy to employ with hospice care providers. Participants of the education sessions discussed the physical effects music has on the body, using music as a healing modality for patients, and caregivers incorporating music as a healing modality. Findings of the developmental evaluation of the education sessions offered indicated the audience of nursing students gained the greatest benefit from the education offered. Future integrations of this scholarly project will allow an advanced nurse practitioner to implement music therapy as a healing modality and incorporate education sessions to meet the needs of a diverse patient population throughout life's transitions.

Book excerpt: The purpose of this study was to examine the quality of life and anxiety/depression level of cancer patients and their families within a hospital setting. Any patient admitted to Tallahassee Memorial HealthCare that met the criteria for this study as determined by the medical personnel, was considered as a potential subject. The music therapy sessions consisted of a song written about the patient and family, live preferred music and counseling to help the patient and family. The experimental and control groups were randomly assigned. The experimental group (N=40), recieved two sessions of music therapy. The first music therapy session was used to gather information that the patient and family wanted to use in the song and also to assess the level of coping. The second music therapy session consisted of giving the patient a copy of the song, and a live performance of the song. The patient and one family member then provided self-report data on the Anxiety/Depression questionnaire, and the Hospice Quality of Life Index questionnaire. One week later, the patient was contacted and asked to fill out the same questionnaires as before and a Songwriting Quesionnaire. The control group (N=40), received one music therapy session consisting of preferred live music and counseling for the patient and family. At the end of each session, the patient and family member completed the Anxiety/Depression questionnaire and the Hospice Quality of Life questionnaire. One week later, the patient was contacted again to complete the same questionnaires as before. The patient Hospice Quality of Life questionnaire showed significantly higher scores for the experimental one-week post session group than the control one-week post session group. The Songwriting Questionnaire filled out only by the experimental subjects, showed a high mean score of 8.5 on a 10 point scale for coping skills.
