Book excerpt: This study uses data from the Malawi Demographic and Health Surveys for men and women in 2000 and 2004. Using a logit model it examines the relationship between the characteristics of individual Malawians and their condom use overtime. Over twenty-five years since the discovery of HIV/AIDS, prevalence rates for the infectious disease are at a historical high of 14.1 percent in adults ages 15 to 49 in Malawi. The Government of Malawi relies heavily for health care support on financial aid and in-kind donations from international organizations and donor governments to help combat the spread of HIV. In 2001, U.S. President George W. Bush reenacted the Mexico City Policy and then in 2002 stopped all U.S. funds going to the United Nations Family Planning Association. Both of these international family planning policies had detrimental effects on the developing world, reducing capacity and supplies in some of the poorest countries in the world. This paper evaluates the impact of that policy in Malawi overtime. It also analyzes individual characteristics that drive the demand for condoms. The results of this paper show that higher levels of education, access to condoms, condom use history, and being tested for AIDS are all positively associated with condom use. This knowledge will help the Government of Malawi to improve the targeting and design of preventative HIV/AIDS public health programs. The findings further support previous research on the negative consequences of the Bush administration's resigning of the Global Gag Rule and the 2002 defunding of the United Nations Family Planning Association.

Book excerpt: If a woman in a developing country learns all that she can about HIV/AIDS but lacks the ability to convince her partner to use that knowledge to prevent disease transmission via condom use, is her likelihood of receiving the virus really reduced? Policymakers and practitioners dispute the relative effects on HIV risk behavior of HIV/AIDS knowledge and interpersonal power. To identify entry points for HIV/AIDS interventions aimed at reducing high-risk behaviors among Haitian women, this study uses Demographic and Health Survey data collected in Haiti in 2000 to consider four questions: (1) Are women more likely to practice safe sex than are men? (2) Are women more likely to use their HIV/AIDS knowledge to improve their sexual behavior outcomes than are men? (3) Is a woman's interpersonal power a stronger determinant of her behavior than her HIV/AIDS knowledge? (4) Is a woman's interpersonal power channeled primarily through her ability to actualize her contraceptive preference? Results do not support the hypothesis that women are more likely than men to use condoms, nor do they indicate that women use their HIV knowledge more effectively than men. Logistic regressions examining the role of a female's interpersonal power and autonomy show little association to condom use. HIV knowledge is correlated to condom use, suggesting that interventions targeted at increasing knowledge among both men and women may be a more efficient allocation of prevention resources than those targeted at increasing condom use by improving women's interpersonal power.

In policy practice, gender is often misconstrued to mean women, when in fact it refers to men and women℗þs roles and responsibilities and the relationships between men and women. Nutrition policies, in particular, are understandably skewed in favour of women because children℗þs nutrition depends on women℗þs diet during pregnancy. However, in low-income countries, gender dynamics often constrain women℗þs access to resources and limit their decision-making in the household, preventing women from accessing nutritious food. While policymakers committed to integrate gender in all policies by signing the Beijing Declaration and Platform for Action, gender norms prevalent in their societies influence policy decisions. These gender norms often affect policy decisions. Using Malawi as a case study, this thesis explores how interpretations of gender influenced the nutrition policy design in Malawi. Malawi℗þs National Nutrition Policy and Strategic Plan 2007 aÌ22́Ơ0́− 2012 and Malawi℗þs draft National Nutrition Policy 2016 - 2020 was used as a case study. A desk review was used to assess the extent to which Malawi℗þs NNPSP (2007 aÌ22́Ơ0́− 2012) was gender-responsive using an integrated framework for gender analysis. Focus group discussions and in-depth interviews identified factors facilitating and constraining men℗þs involvement in maternal and child nutrition. A policy dialogue which brought together a range of stakeholders from the nutrition and gender domains in Malawi validated the research findings. While Malawi has made much progress in reducing undernutrition in under five children from 47% in 2010 to 37% in 2016, this reduction is not sufficient for achieving the Sustainable Development Goals. Gender-responsive nutrition policies could assist in accelerating progress to meeting SDG two in particular, which focuses on ending all forms of malnutrition by 2025. The study found that Malawi℗þs National Nutrition Policy 2007 aÌ22́Ơ0́− 2012, although reported to address gender, is not gender-responsive. Weaknesses in understanding how to integrate gender in nutrition policy were identified, coupled with possible biases and ideologies that influenced decision-makers. The policy reinforces the role of women in nutrition, overlooking the gender dynamics that constrain women℗þs access to nutritious food. The supportive role of men is overlooked. Community members℗þ interpretations of gender differed significantly from policymakers℗þ. Men reported taking up childcare and housework that was previously perceived to be women℗þs responsibility, shifting traditional gender roles. In Malawi, Non-Governmental Organisation programmes that work with men to increase men℗þs participation in housework have influenced gender roles and responsibilities in the household. In particular, the role of traditional leaders in promoting men℗þs participation in maternal and child health has been a successful method of involving men. However, weaknesses in the way in which these programmes are designed hinder progress toward gender equality. While the forthcoming National Nutrition Policy is far less gender-blind compared to its predecessor, the policy remains gender-tepid. Policymakers℗þ preconceptions around gender continue to influence policy design. Although policymakers attempt to address gender inequality through approaches such as men℗þs involvement in maternal and child nutrition, their gender biases prevent them from recognising weaknesses in policy design that may deepen the gender gap rather than lead to equality. Dialogue between policymakers and other stakeholders is needed to ensure that policymakers become aware of their gender biases and do not perpetuate gender norms.
