It has recently been established as a UK speciality in it’s own right, and is at the centre of the modern acute hospital, responsible for managing the sickest and most complex patients. There is a growing cadre of critical care specialists in the UK, underpinned by a large number of doctors in training within the specialty. Management of patients with severe traumatic injury is provided by intensive care specialists, often in conjunction with a range of other professionals such as surgeons and interventional radiologists. The management of these patients, who have competing complex injuries can be challenging. Traumatic injury is recognised as a significant cause of preventable mortality and such patients are now clustered within Major Trauma Centres across the UK. The Defence Medical Services of the UK have spent the last 10 years managing patients with very severe traumatic injuries, first in Iraq and most recently in Afghanistan. The lessons learnt from this experience has filtered through to the NHS, resulting in many changes to established practice. Whilst several books have been published based on this experience, none have focused on the intensive care management of such patients, which represents a vital link in the chain of survival from injury to recovery.

Book excerpt: Care for patients who have suffered major trauma, for example following a road accident or a fall, has not significantly improved in the last 20 years despite numerous reports identifying poor practice, and services are not being delivered efficiently or effectively. Survival rates vary significantly, with a range from five unexpected survivors to eight unexpected deaths per 100 trauma patients, reflecting the variable quality of care. 450 to 600 lives could be saved each year in England if major trauma care was managed more effectively. For best outcomes care should be led by consultants experienced in major trauma; but major trauma is most likely to occur at night and at weekends, when consultants are not normally in the emergency department. Major trauma care is not coordinated and there are no formal arrangements for taking patients directly for specialist treatment or transferring them between hospitals. A significant number of patients that need a scan CT do not receive one. Not enough patients who need a critical care bed are given one. Access to rehabilitation services varies and patients are not always receiving the care that they need. The estimated annual lost economic output from deaths and serious injuries from major trauma is between £3.3 billion and £3.7 billion. Only 60 per cent of hospitals delivering major trauma care contribute to the Trauma Audit and Research Network (TARN). The performance of the 40 per cent of hospitals that do not submit data to TARN cannot be measured.

Traumatic brain injury and hemorrhage are the most common causes of early death, whereas complications such as infections, (multi-)organ failure and “persistent inflammation, immunosuppression, and catabolism syndrome” (PICS) represent relevant factors for late adverse outcomes. Pre- and intra-hospital diagnostic and therapeutic standard operating procedures have been shown to beneficially influence posttraumatic outcome. However, development of patient-specific diagnostic and therapeutic strategies remains challenging due to uncertainties regarding the assessment of the individual risk profile. Furthermore, the relevance of prevention and rehabilitation measures to avoid unfavorable long-term consequences of trauma is not fully elucidated.
