Book excerpt: The satisfaction of new graduate nurses with hospital orientation may impact job satisfaction, retention, and the ability of healthcare facilities to develop proficient nurses. Preceptors are an integral part of nursing orientation and the use of one or multiple preceptors during new graduate nurse orientation is discussed in previous research studies with varying results. Some study participants value using a minimal number of preceptors, while other new graduate nurses enjoy the variety of having multiple preceptors. The purpose of this thesis was to examine the effect of orienting with one or multiple preceptors on the degree of satisfaction with the orientation process. A convenience sample of 30 new graduate nurses employed at a 247 bed, nonprofit hospital was asked to complete one section of the Casey-Fink Graduate Nurse Experience Survey at the completion of 12 weeks of new graduate nurse orientation. The section of the survey used measured support, organization, stress, communication, and satisfaction at the end of 12 weeks. There was a moderate inverse relationship between the number of preceptors utilized during orientation and the support and satisfaction subcategories. Today's professional nurse is overworked and over burdened with exceptionally difficult patient caseloads. Self esteem and self worth become low when job dissatisfaction is high. The goal of nursing administrators and nurse educators should be focused on retaining staff nurses and providing for high worker satisfaction. The purpose of this research study was to determine if participating in a student nurse preceptorship program increased the level of job satisfaction- experienced by the staff nurse. The Employee Assessment Tool is a thirty-five item quality of work analysis tool used to determine the importance of work factors and their relationship to job satisfaction. The tool was distributed to thirty nurses who were presently preceptors for student nurses, and thirty nurses who were not preceptors for students, and one hundred percent of the nurses responded and returned the questionnaire to this researcher. The hypothesis stated that nurses who work as student nurse preceptors have an enhanced sense of job satisfaction over nurses wh6 do not work as student nurses preceptors. This hypothesis was not found to be supported from the research data collected. A T-test was used to statistically analyze the data. Limitations of the study such as controlling for the number of times a nurse had worked as a preceptor, and that only one college program and hospital were used, may have lead to the unsupported results obtained. Recommendations for future research include using a larger sample population with greater control over how many times the nurse was a preceptor. More intensive research is needed in order to improve staff nurse retention and to increase the quality of the job satisfaction felt by the staff nurse.

Preceptor involvement in the development of these evaluation tools is imperative for successful implementation of them. The purpose of this descriptive study was to describe the preceptors' perceptions of two tools, the RN Weekly Performance Summary and the Benner Guide to the RN Weekly Performance Summary, to determine utility and helpfulness when evaluating nurse orientees. A convenience sample of 38 RN preceptors (47% response rate) from three system hospitals were surveyed via the Preceptor Evaluation Questionnaire. Over 76% of participants perceived the two tools as easy to use and helpful when evaluating their nurse orientees. It was hoped through this study, by eliciting preceptors' perceptions, more effective evaluation tools could be developed that will be more readily accepted by preceptors and show orientee progress through orientation.

Book excerpt: The first year of professional employment as a Registered Nurse is an exciting yet somewhat stressful time for the new graduate nurse. Most health care institutions provide opportunities to make the transition from student nurse to professional nurse through a new graduate nurse orientation. These orientations vary widely in the amount of time allowed, the resources provided the new graduate nurses, and the experiences afforded the new graduate nurses. Much of the research of these orientation programs looks at the impact of the orientation on retention of new graduate nurses. The body of research in this dissertation attempts to identify how and education-based new graduate nurse orientation contributes to retention by identifying essential elements of that orientation. Through a single case, explanatory, holistic case study multiple sources of evidence including archival data, one-on-one interviews with new graduate nurses who completed the orientation program, and a focus group interview of new graduate nurses who completed the orientation program were examined to answer the question of how the orientation impacts retention and which elements are essential for retention, satisfaction, increased competency levels, and ease of transition for the new graduate nurse. Using a theoretical framework including Chickering's psychosocial model of development and Romiszowski's analysis of knowledge and skill development, it is concluded that preceptorship is the essential element of new graduate nurse orientation that promotes new graduate nurse retention, satisfaction, increased competency levels, and ease of transition. Preceptorships facilitate the socialization necessary in orientation for new graduate nurses' success by affecting all elements of young adult development as well as providing the necessary continuation of acquisition of nursing knowledge and skills for the novice nurse. Implications of these findings as well as suggestions for future research are provided.

The development and implementation of a preceptorship model reflects a partnership between nursing education and nursing service--between the urban nursing school and the rural hospital. A five-stage preceptorship model is presented that includes development of guidelines, selection of participants, orientation of participants, implementation, and evaluation. The model draws on a literature review plus feedback from nurse preceptors and graduating nursing students. The model emphasizes the importance of clearly stated purposes and objectives; guidelines outlining the roles and responsibilities of the student, the preceptor, and the faculty liaison; and careful selection and preparation of all participants. In spring 1992, 24 students completed their senior practicums in rural hospitals using the preceptorship model; all were satisfied with the experience, met their clinical objectives, and completed their learning contracts. Evaluations of the model by the faculty liaison person, preceptors, and students resulted in 18 recommendations to improve implementation of the model. The strongest preceptor recommendation was that students and preceptors work identical hours with identical patient assignments. Contains 53 references. Unrealistic job expectations, inadequate initial socialization, and increased work demands result in lowered job satisfaction and higher turnover among new nurses. Extant literature on graduate transition has identified "best practice" principles that can ease the transition, and foremost among them is a formal precepted orientation. This study used a quasi-experimental survey design to assess the effects of a formal precepted orientation on the self-perceived transition experiences of new graduates hired into a 140-bed rural county hospital. Perceptions of graduate registered nurses hired into hospital units that possess a formal orientation are described along with those of their counterparts hired into hospital units without a formal orientation. The convenience sample of new graduates (n = 10) found that both the control (n = 5) and experimental (n = 5) groups reported a positive transition when they had a supportive work environment and preceptors. This implies a supportive work environment may be as conducive to a positive transition as a formal precepted orientation. For the nurses in this study, a supportive work environment might have been established prior to hire, as many of the new graduates had spent time in the hospital during nursing school clinical rotations. Graduates' level of personal stress in transition was unaffected by a formal precepted orientation.
