Book excerpt: This study assessed the alignment and rotation in mobile bearing total knee arthroplasty (TKA) with the tibia cut first technique using an imageless referencing computer navigation protocol evaluating 41 patients. Prerelease mechanical alignment (MA) averaged 7° varus +/−5° (Range: 8° valgus to 20° varus). Post implant MA was 0.5° varus +/−1.2° (Range: 2° valgus to 3° varus). Post operative radiographic MA was 0.3° varus +/−1.3° (Range: 2° valgus to 2° varus). The baseline measurement of tibial rotation from 0° to 90° flexion was 6°+/−7.2° of tibial internal rotation (Range: 8° external rotation to 19.5° internal rotation). The post implant tibial rotation from 0° to 90° flexion was 3.6°+/−8° of tibial internal rotation (Range: 17° external rotation to 29° internal rotation). Of the baseline group, 25 % demonstrated tibial external rotation with flexion. After TKA, 28 % had tibial external rotation with flexion. When comparing the nominal tibial position in relation to the femur at 0° before and after TKA, the tibial rotation point at 0° moved more externally in 21 % and more internally in the rest with mean change for the overall group of 3.9° of internal rotation (Range: 17° internal to 5° external). This study identified significant changes in knee rotation that may be caused by correction of alignment and deformity. Mobile bearing implants by nature of unconstrained rotation are likely to accommodate these variations. This feature could be defined as a significant advantage over fixed-bearing prostheses.

Buechel and Pappas's design was based on the principles of; restoring anatomical joint function to as near normal as possible, minimising contact stresses to avoid wear and darn age to the bearing surfaces. and finally the idea that constraint should refiect the need for mobility, to avoid shear stresses and loosening of the implant. In 1977, the LCS® knee was implanted by Dr. Frederick Buechel. This was the first mobile bearing, tri-compartmental knee implant. This was also the first to successfully address the key issues ofloosening, wear and patello-femoral problems associated with earlier designs. The unique design solution was the creation of a common articulating geometry for the tibia and patella on the distal femoral surface. This resulted in a tibial and patellar articulation that was mobile in nature, but with an identical radius of curvature and conformity. The mobile bearing concept was considered sufficiently novel and unproven that the US FDA (Food & Drug Administration) required that it be validated in an Investigational Device Evaluation (IDE). An FDA IDE study involving 25 US surgeons was initiated in 1981. Validation of the clinical success of the device in this study resulted in FDA approval of the LCS, Knee (for cemented, tri-compartmental use) in 1985.

Excellent survivorship, function restoration and pain relief have been reported for many designs of TKA that differ in methods of fixation, including fixed-bearing (FB) and mobile-bearing (MB). In FB knee, the polyethylene is mounted firmly on the metal implant tibial component whereas the design of MB knee is intended to lower the risk of tibial component loosening by reducing bearing stress on implant surface with enhanced conformity between tibial and femoral components. MB design is also believed to allow greater range of motion (ROM), and the knees restored by MB implant might be closer to natural knee in terms of knee kinematics. However, the advantages of MB over FB implant have not been clearly demonstrated. Regarding the clinical and radiological differences in outcomes and evaluations between fixed- and mobile-bearing total knee replacements, controversy still exists on which design overall performs better than the other one does. From the prospective of biomechanics, this thesis was to delineate the knee joint motion and loading after TKA surgery with MB and FB implants in daily activities. Twenty-three patients who received TKA surgeries using MB implant and twelve received FB implant were recruited from the same medical center. Ten healthy subjects without history of knee injuries were recruited as the control group. Human motion data and ground reaction force data were collected during level walking, stair ascent and descent. Three-dimensional (3D) knee joint motions were determined from redundant markers using an optimization approach. The 3D knee joint moments and net reaction forces were calculated from motion data and ground reaction data by using an inverse dynamics model of the lower extremity. The results from two groups of individuals receiving different total knee implants were compared. TKA restored but not fully restored the ROM of the knee in sagittal plane, and there was no statistical difference of this functional restoration between two types of implants. Knees with MB implant better constrained the tibia anterior translation when tibia exhibited extension. They also exhibited less femoral internal rotation and knee valgus than knees in FB implants did; therefore, it might have less risk of patellofemoral joint damage or pain under excessive compressive stress in weight-bearing circumstance. The results also demonstrated that MB implants were more effective to reduce knee joint rotation and adduction torque during stance phase. The study provides evidences of that the features of knees in MB implant may be functionally better than FB design to reduce knee joint loading; however, we might not be able to fully attribute the changes in dynamic loading to the design of implant. Other important factors such us performance of surgery, improvements of materials, surface finish techniques of implants, alignment of the components, anatomical and physiological alterations after injuries or surgeries should be considered with respect to longevity and functionality of the knee implants. The best design of implant might be the one with which the surgeon is most comfortable.

Book excerpt: The objective of this paper is to describe the incidence of different types of re-operations following total knee replacement (TKR) with mobile bearing designs and to understand the extent to which they are susceptible to spin-out, a specific complication that may arise with these designs. The design is a systematic review of the incidence of re-operations, classified by the type of re-operation and stratified by study date, reported by clinical publications following TKR with fixed and mobile bearings from a single manufacturer. A search for relevant papers was conducted in online databases including EMBASE and Medline and a manual search of bibliographies. Studies following 8739 mobile bearing knees implanted in 1985-1997 and studies following 3413 mobile bearing knees implanted in 1997-2006 were identified. In the pre- and post-1997 studies, the number of insert revisions was 190 (2.3%) and 16 (0.4%), respectively; the number of revisions of the tibial tray/femoral components was 295 (3.6%) and 43 (1.2%), respectively; the number of revisions for spin-out,dislocation, and instability was 117 (1.4%) and 10 (0.26%), respectively. In the fixed bearing studies there were no spin-outs, but the number of revisions for instability were 6 (0.16%) and 6 (0.21%) in pre- and post-1997 studies. For knees implanted between 1985 and 1997 the incidence of all wear related insert or component revision was 2% in the fixed bearing knee studies and 2% in the mobile bearing knee studies. For knees implanted in 1997-2006, the incidence of all wear related insert or component revision was 0.1% in the fixed bearing knee studies and 0.3% in the mobile bearing knee studies. In conclusion, polyethylene spin-out remains a unique complication of mobile bearing knees symptomatic of instability. Recent trends (after 1997) suggest that improved awareness of surgical technique and/or changes in design (posterior stabilization) have significantly decreased the incidence of this complication, with no evidence of a higher overall risk of revision for instability with contemporary mobile bearing versus fixed bearing knees.
