Substances released from activated immune cells (cytokines) stimulate peripheral nerves, thereby signaling the brain and spinal cord that infection/inflammation has occurred. Additionally, peripheral infection/inflammation leads to de novo synthesis and release of cytokines within the brain and spinal cord. Thus, cytokines effect neural activation both peripherally and centrally. Through this communication pathway, cytokines such as interleukin-1, interleukin-6 and tumor necrosis factor markedly alter brain function, physiology and behavior. One important but underrecognized aspect of this communication is the dramatic impact that immune activation has on pain modulation. The purpose of this book is to examine, for the first time, immune-to-brain communication from the viewpoint of its effect on pain processing. It is aimed both at the basic scientist and health care providers, in order to clarify the major role that substances released by immune cells play in pain modulation. This book contains chapters contributed by all of the major laboratories focused on understanding how cytokines modulate pain. Book excerpt: The vertebrate immune system defends the organism against invading pathogens while at the same time being self-tolerant to the body’s own constituents thus preserving its integrity. Multiple mechanisms work in concert to ensure self-tolerance. Apart from purging the T cell repertoire from auto-reactive T cells via negative selection in the thymus dominant tolerance exerted by regulatory T cells plays a major role in tolerance imposition and maintenance. Among the various regulatory/suppressive cells hitherto described, CD4+CD25+ regulatory T cells (Treg) and interleukin-10 producing T regulatory 1 (Tr1) cells have been studied in most detail and are the subject of most articles in this issue. Treg, also called "natural" regulatory T cells, will be traced from their intra-thymic origin to the site of their action in peripheral lymphoid organs and tissues. The repertoire of Treg is clearly biased towards recognition of self-antigens, thereby potentially preventing autoimmune diseases such as gastritis and oophoritis. Regulatory T cells, however also control infections, allergies and tolerance to transplanted tissues and this requires their induction in the periphery under conditions which are not yet fully understood. The concept of dominant tolerance, by far not novel, will offer new insights and hopefully tools for the successful treatment of autoimmune diseases, improved cancer immunotherapy and transplant survival. The fulfillment of these high expectations will, however, require their unambiguous identification and a better understanding of their mode of action.

Upon activation by bacterial LPS, macrophages produce a variety of pro-inflammatory cytokines such as TNF-α, IL-1 and IL-6. Excessive and uncontrolled production of these cytokines could lead to serious systemic complications with high mortality. Transforming growth factor-β (TGF-β) is a multifunctional cytokine which is generally believed to suppress tissue macrophages activity and involved in the resolution of inflammatory responses. However, the role of TGF-β signaling in the initiation of cytokine expression is not known. This study aims to investigate the effects of LPS on TGF-β receptor I (TGF-βRI, ALK5) and its downstream effector functions. Upon LPS activation ALK5 was expressed with concomitant expression of the intracellular TGF-β signaling molecule "phophorylated-SMAD2." In particular, LPS induced TNF-α protein expression by macrophages was blocked by ALK5 specific inhibitor (SB431542) in a dose dependent manner, however this effect was not observed in LPS induced IL-6 production, which suggested the possible role of TGF-β/ALK5 signaling is specifically in control of the inflammatory cytokine TNF-α production. However, TNF-α mRNA level was unchanged after SB431542 treatment which revealed the iiregulation is at post-transcriptional level. TNF-α production was partially blocked by the p38 MAPK inhibitor SB203580 and a 95% additive inhibitory effect was observed with the addition of both inhibitors. My data indicates that LPS-induced TNF-α production in macrophages is not solely dependent on TLR4/p38 MAPK signaling pathway.
