Book excerpt: Introduction : Le frottis cervico-utérin (FCU) est actuellement l'outil de dépistage de référence du cancer du col de l'utérus (CCU). Sa généralisation a permis une nette régression de l'incidence et du taux de mortalité du CCU, mais il subsiste des inégalités de dépistage en France. L'objectif de cette étude était d'évaluer l'influence du sexe du médecin généraliste (MG) et de sa pratique sur le taux de participation au dépistage du CCU. Méthodes : Étude épidémiologique descriptive réalisée sur 2 périodes du 1er mars 2015 au 23 janvier 2017 auprès de 343 MG rattachés à la Caisse Primaire d'Assurance Maladie (CPAM) des Flandres. Un sondage téléphonique chez les médecins inclus a pwwwwwwwwwwwwwwwwwermis de statuer sur la pratique par le médecin du FCU. La base de données de la CPAM a fourni les caractéristiques des MG (sexe, nombre de patientes âgées de 25-65ans et celles ayant un remboursement de FCU, cotation du FCU). Elle a été enrichie par l'indice de défavorisation européen (EDI) et l'offre de soins gynécologiques de l'aire d'installation du MG. Résultats : Le taux de réponse des MG interrogés était de 98,6%. Le taux de participation moyen au dépistage du CCU était de 50,1 %. Ce dernier était plus élevé si le MG était une femme (p=0,0011), qui réalisait (p=0,0058) et cotait les frottis (p=0,0009). Toutefois, cette association n'était plus vérifiée en analyse multivariée pour le sexe du médecin (p=0,0530) et la pratique du FCU (p=0,0822), mais restait significative sur la cotation du frottis (p=0,0007). Conclusion : Le sexe du MG n'influençait pas le taux de dépistage du CCU par FCU de sa patientèle alors que la cotation de l'acte était associée à un meilleur taux de dépistage. Une plus forte implication des MG dans la stratégie de dépistage du CCU par le FCU ou d'autres méthodes (test HPV etc.) est primordiale pour réduire les inégalités de santé.

The advent of more sophisticated diagnostic methods and their relatively easy clinical application allow for the incorporation of that knowledge into the evaluation of the infertile couple. These facts, together with an obvious change in social psychology, have facilitated the development of different specialities dealing with the problems of infertility. It is now possible in medical centers all around the world to undertake a better disposition of the infertile husband to look for advice, to be studied, and treated. Confronted with this situation we are still unable to offer specific therapy in most cases; however, empirically based therapy abounds. Advances in therapy have not kept pace with our increased physio logical knowledge and improved diagnostic techniques. Patho physiological mechanisms and etiological factors in male infertility are largely unknown. This has significantly hampered both clinical evaluation and advances in treatment allowing for frequent non scientific therapeutic incursions into the armamentarium of the an drologist. Several factors have given birth to therapeutic "fashions", which are bound to survive as long as this state of lack of knowledge con tinues. For example, we may ask whether the treatment of varicocele constitutes a fashion? Though we accept the treatment of varicocele as the best available treatment of male infertility, we do not know its mechanism of action and so we cannot predict a therapeutic result.

The idea was to found in Europe an institution simi lar to the American Gordon Research Conferences of Bone and Tooth. After the U. Book excerpt: This volume represents the first attempt to present in one place the clinical syndromes and the pathophysiologic basis for the "resistance states" to each of the classes of steroid hormones. Glucocorticoids, mineralocorticoids, androgens, estrogens, progesterone and vitamin D have widely diverse roles ranging from the control of homeostasis to reproduction and bone formation. They are similar in that they share a chemical structure and that their action is in the cell nucleus where they induce transcription of specific genes leading to synthesis of function-specific proteins. Clinical syndromes of steroid hormone resistance to androgens (complete and partial testicular feminization), aldosterone (pseudo hypoaldosteronism) and vitamin D (vitamin D-dependent rickets type II) have been known for many years. Progesterone and glucocorticoid resistance syndromes have been described only recently. Resistance to estrogens has not been reported in man or in animals. It is hoped that a detailed reexamination of what is known about each of these conditions at the clinical and molecular levels will enhance our understanding of the function of these hormones and their mechanisms of action. The steadily increasing atten dance has shown that, as could be expected from the rapid progress of modern medicine, there is a widely felt need for this form of postgra duate study. In 1957, therefore, the Leiden Medical Faculty appointed a permanent committee for the organization of postgraduate medical education. Of the courses given since then, certain material proved to have sufficient immediate scientific value to justify publication, and it now gives the Committee great pleasure to announce that in collaboration with the Leiden University Press it will publish the Boerhaave Series for Postgraduate Medical Education. The first volume of this new series is the product of the course on Human Blood Coagulation given in Novem ber 1968. It is our hope that this book will prove valuable not only to those who participated in the course but also to many others working in this and associated fields.
