Un tiers se développe chez des femmes de plus de 70 ans. La radiothérapie est une alternative, lorsque la chirurgie est contre-indiquée. La fréquence des séances peut néanmoinsêtre un facteur limitant. La radiothérapie hypofractionnée semble représenter un choix préservant la qualité de vie de ces patientes agées. Notre travail s'est donné pour objectifs d'évaluer la radiothérapie hypofractionnée exclusive comme traitement curatif en terme d'efficacité sur la progression tumorale et la survie et d'étudier ses effets secondaires. La première partie a relaté les particularités cliniques et biologiques du cancer du sein chez la femme agée et les traitements qui pouvaient lui être proposés. La deuxième partie a concerné l'étude de 115 patientes dont l'âge moyen était de 82.7 ans, porteuses de cancers du sein primitifs de stade1 à 3, traitées au Centre Antoine Lacassagne par radiothérapie hypofractionnée exclusive associée à une hormonothérapie entre 1987 et 1999. Nos résultats montrent que la radiothérapie hypofractionnée exclusive associée à l'hormonothérapie peut être une proposition thérapeutique raisonnable pour des patientes très âgées ou inopérables.
