Parmi les risques identifiés, le tabagisme est considéré comme l’un des plus importants dans la progression de la maladie parodontale. L’usage du tabac est un problème majeur de santé publique. Un certain nombre de lois dans les pays industrialisés ont été votées pour tenter de diminuer la consommation de tabac car elle est liée à un grand nombre de pathologies incluant les cancers et les maladies cardiovasculaires. Néanmoins, une proportion significative de la population est toujours dépendante du tabac. En odontologie, l’association maladies parodontales/tabac a été rapportée pour la première. fois en 1947 (Pindborg) et confirmée par des études épidémiologiques depuis les armées 80 jusqu’à nos jours. il a été montré que le risque qu’un fumeur développe une parodontite était plus que doublé par rapport à une population de non fumeurs. (Preber et Bergström, 1994). Cependant un nombre très restreint d’études ont cherché à étudier la corrélation entre intensité du tabagisme et sévérité des parodontites. Pour cette thèse, après une première partie consacrée à une revue de la littérature nous avons réalisé une étude ayant pour but d’étudier l’association entre la dose de tabac consommé et la sévérité des parodontites (effet dose).

Book excerpt: La parodontite est une maladie parodontale, dont l'étiologie majeure est une flore bactérienne pathogène et ses facteurs de virulence. Face à cette agression, les tissus parodontaux de l'hôte vont se défendre par une réaction inflammatoire qui met en jeu de nombreuse cytokines pro-inflammatoires, dont les interleukines. Cependant, sous l'influence des certains facteurs de risque (génétiques, environnementaux), l'hôte peut se retrouver « débordé » ou devenir « permissif », ce qui favorise en particulier une surproduction d'interleukines, conduisant à une réaction inflammatoire exacerbée et à une destruction tissulaire plus sévère au niveau du parodonte. Cette réaction de défense qui est la réaction inflammatoire fait intervenir des cytokines pro-inflammatoires, dont les interleukines. Ces interleukines vont avoir une action à tous les niveaux de la pathogenèse des parodontites. Le tabac est un facteur de risque majeur dans le développement, l'évolution et la sévérité des parodontites , par la fumée qu'il génère et les nombreux éléments irritants et nocifs qui le composent, va notamment perturber les réactions de défense au niveau du parodonte. Parmi les autres facteurs intervenant dans l'apparition et la progression des parodontites on trouve un certain polymorphisme génétique, qui lorsqu'il est présent, va orienter la réponse de l'hôte au détriment des tissus parodontaux. Depuis les années 90, les recherches scientifiques se sont orientées vers les interactions entre les maladies parodontales et la réponse de l'hôte : aujourd'hui, nous savons qu'une réponse inflammatoire exacerbée devient néfaste pour les tissus qu'elle est censée protéger. Il semble que les interleukines jouent un rôle central dans ces interactions. Le but de cette thèse est de refléter, à l'aide des données scientifiques actuelles, le lien existant entre les parodontites, les interleukines et le tabac.
