Elles jouent un rôle essentiel dans la constitution et le maintien de la forme d'arcade, et la fonction de l'incision. Elles font également partie de l'esthétique et de l'équilibre du visage et du sourire. L'agénésie d'une incisive centrale est très rare. Cette situation se présente dans le syndrome de l'incisive centrale unique médiane maxillaire (prévalence de 1/50 000 naissances vivantes). Cette incisive se retrouve en denture temporaire et permanente. Cette anomalie peut être isolée ou faire partie d'un syndrome touchant les structures de la ligne médiane (holoprosencéphalie). Ce syndrome peut se retrouver également associé à des anomalies des voies aériennes (sténose congénitale des orifices piriformes). Des caractéristiques faciales touchant les structures de la ligne médiane sont visibles chez ces patients. D'autres anomalies peuvent également être repérables comme une petite taille, un hypopituitarisme, un retard mental. Des études récentes montrent que ce syndrome serait lié à une mutation génétique du gène SHH. La transmission est autosomique. Les retentissements sur le développement des arcades et sur l'esthétique nécessitent une prise en charge multidisciplinaire médicale, dentaire et orthodontique, qui sera traitée dans cette thèse.
