Book excerpt: Dear colleagues, Promotion of the development of ENT is one of the most important tasks of IFOS (International Federation of Otorhinolaryngology). Apart from organizing the World Congresses it supports different international and regional meetings, organizes symposiums on well determined subjects, etc. Since the last World Congress in 1981, in addition to the Danube Symposium in 1982 and the Congress of ENT Societies in Asia Oceania in 1983, an international symposium was organized in Antwerp by the Belgian ENT Society on evaluation problems of middle ear surgery. This is a very controversial question on which there is no concensus. During a four-day meeting many anatomical and pathological aspects, operative methods, possibilities of the evaluation of the results with computers, were widely discussed. You will find all the lectures presented in this book. I am convinced that these papers will prove to be a good basis for further discussion and hope that finally we shall achieve an international agreement on the evaluation of our results in middle ear surgery. The digital anatomy correlation used in this work is the natural continuation of efforts to apply the University of Colorado Visible Human data set to gastroenterology. The Visible Human data set was created by Dr. Vic Spitzer and colleagues at the University of Colorado and is currently housed at the universitys Center for Human Simulation. The data set consists of high resolution transaxial digital images captured as cadavers were abraded away at 1 mm or less depths. These images are compiled into blocks of data and each structure is identified. This information can be used to pull out and manipulate 3-D structures as well as allowing one to review planar anatomy in any orientation. Using the Visible Human dataset, one should be able to find a normal anatomy correlate to any image found during a EUS examination. However, as important as normal anatomy is, it is the abnormal features which are the crux of an EUS examination. Endosonographers are asked to define lumps, bumps, cysts to find correlates for symptoms and abnormal laboratory findings. Accuracy requires a tremendous amount of skill and experience. To help in this task, we have assembled chapters from a world-wide group of expert endosonographers. These authors have shared their insight and images to help the readers of this work better see and understand some of the complexities uncovered during a EUS evaluation. Efficient ENT practice, like that in other surgical specialties, relies on close collaboration between surgeon and pathologist through the medium of biopsies. Accordingly much of the present volume emphasizes diagnostic surgical pathology. The role of the pathologist is, however, a broader one-in otolaryngology as in other specialties-contributing as well to an understanding of the disease processes. For this pur pose autopsy study is important and nowhere more so than in the disease of the inner ear, an area which has been developed mainly by otologists, but neglected by pathologists. It is hoped that, by displaying the interest and potential for development of such fields, this book may stimulate some pathologists to work in them. I kept in mind while writing the possibility that it might also be useful to ENT clinicians as a review of the pathology of their specialty.
