La douleur dentaire est très difficile à supporter, et elle l’est certainement plus encore, lorsque d’autres douleurs liées au handicap s’y ajoutent. Certaines de ces douleurs liées au handicap sont parfois difficiles à atténuer. Contrairement à celles-ci, la douleur dentaire peut être soulagée et il est de notre devoir de ne pas laisser ces patients souffrir inutilement. De nombreuses échelles d’évaluation de la douleur se développent actuellement, afin de mieux comprendre les patients et les prendre en charge. Nous avons établi à partir des outils existants, un questionnaire court et simple, d’estimation de la douleur dentaire. Il a pour objectif d’aider à la compréhension de la douleur ressentie par le patient présentant des difficultés de communication, afin de pouvoir le soigner en structure ambulatoire. Choisir l’anesthésie générale en milieu hospitalier, revient parfois à choisir la facilité pour le praticien, mais ne correspond pas nécessairement à la meilleure approche pour le patient. Une formation spécifique des étudiants et chirurgiens-dentistes à la sédation et à la prise en charge des personnes handicapées devient indispensable, afin de pouvoir traiter ces patients en dehors de l’hôpital

Book excerpt: Cette étude prospective observationnelle monocentrique avait pour but d'évaluer les suites opératoires après extraction d'une dent temporaire sous anesthésie locale. Le premier objectif était de décrire la prévalence de la douleur post-extraction (DPE), du saignement post-extraction (SPE), des morsures post-extraction (MPE) et de l'utilisation des analgésiques chez les enfants. L'objectif secondaire était de déterminer un profil de patients ou un type d'extraction prédictif de la douleur et de l'administration d'analgésiques, ou prédictif des saignements ou de morsure. 125 enfants âgés de 3 à 13 ans ont participé à l'étude. Immédiatement après l'extraction et jusqu'au lendemain, nous avons demandé aux parents d'évaluer les suites post-extraction et d'administrer du paracétamol dès que l'enfant exprimait un score de douleur ≥ 2 évalué sur l'échelle analogique des visages (EAV). 37,3% des enfants ont déclaré une douleur post-extraction (EAV ≥ 2), mais 23,25% de ces enfants n'ont reçu aucun médicament analgésique pour aider à soulager la douleur. Une incidence plus élevée de DPE et d'utilisation d'analgésique a été observée à la fois dans le groupe d'enfants défavorisés par rapport au groupe socio-économique favorable et dans le groupe d'enfants ayant ressenti douleur per-opératoire comparée à l'absence de douleur lors de l'extraction (p
