Book excerpt: Introduction : les infections respiratoires aiguës (IRA) hautes constituent un motif très fréquent de consultation et génèrent la majorité des prescriptions d'antibiotiques chez l'enfant. L'objectif principal de notre étude était d'analyser la conformité des prescriptions aux recommandations. Matériels et méthodes : étude épidémiologique observationnelle descriptive rétrospective réalisée aux urgences d'Arcachon du 1er mai 2015 au 22 mai 2016. Les enfants de 3 mois révolus à 15 ans admis en consultation externe pour IRA haute étaient inclus. Exclus : moins de 3 mois, lourde comorbidité, IRA basse ou infection concomitante et IRA haute grave. Résultats : 334 enfants ont été inclus. Les otites ont été le diagnostic le plus fréquent (44.4%), suivies des rhinopharyngites (31.9%) et des angines (22.8%). 55.1% des patients sont sortis des urgences avec une prescription d'antibiotiques. 37.1% des prescriptions étaient non conformes. Les causes de non-conformité étaient l'erreur de durée (31.5%), l'absence d'indication d'antibiothérapie (24.7%), l'erreur de molécule (19.2%), l'erreur de prescription d'un antibiotique local (13%) et l'erreur de posologie (11.6%). Parmi les prescriptions d'antibiotiques per os à large spectre, 80.4% étaient injustifiées. Le TDR était réalisé dans 90% des angines chez les plus de 3 ans. Conclusion : le mésusage des antibiotiques dans les IRA hautes reste un problème majeur de santé publique du fait de l'émergence de résistances bactériennes. Nos résultats sont contrastés. Plus d'un tiers des prescriptions restent non conformes et les prescriptions d'antibiotiques à large spectre sont souvent injustifiées. Des efforts doivent être poursuivis pour améliorer la prise en charge de ces affections.
