Surface chemistry, which is influenced by surface energy, wettability, and composition, is another factor that determines osteoblast phenotype and regulates osteoblast maturation. Increased surface energy is desirable for bone implants due to enhanced interaction between the implant surface and the biological environment. The extent of bone formation in vivo is also increased with increasing water wettability of implants. The physiological role of implant surface chemistry is important in determining the success of implant osseointegration because of molecular rearrangements, surface reactions, contamination, and release of toxic or biologically active ions that are determined by the starting chemistry. However, the role of surface chemistry on osteoblast response is not fully studied. Therefore, the overall goal of this dissertation is to understand how the surface chemistry, including wettability, chemical composition, and charge density, of titanium biomaterials impacts osteoblast maturation (in vitro). This study focuses on the general hypothesis that modifications of surface chemistry of titanium surfaces with sterilization or polyelectrolyte coating on titanium surfaces regulate osteoblast response.

However, it cannot bridge large defects without the additional help of a supporting material. Among the different synthetic bone grafts, calcium phosphates (CaPs) are excellent candidates for bone regeneration due to their close resemblance to the mineral phase of bone, as well as the bioactive and osteoinductive potential. The present thesis explores the effect of the physicochemical features of CaPs on the interaction with the cells involved in the different stages of the bone healing process: inflammation, angiogenesis and osteogenesis, paying special attention to biomimetic calcium deficient hydroxyapatite (CDHA). This is analysed through in vitro cell cultures with immune, endothelial and bone forming cells, respectively. An overview of bone biology and bone healing process is presented in Chapter 1. Moreover, the Chapter 1 describes synthetic bone grafts based on CaPs and the impact of their physicochemical features on cellular behaviour in vitro. Chapter 2 and Chapter 3 study the response of immune cells to calcium phosphates and its outcome on osteogenic differentiation of bone forming cells. Specifically, Chapter 2 explores the link between nanotopography, specific surface area and porosity of biomimetic CDHA and its immunomodulatory, osteoimmunomodulatory and antinflammatory features, demonstrating that neddle- like topography of CDHA stimulated the osteogenic activity of bone forming cells, whilst a reduction of porosity to CDHA substrates decreased the inflammatory state. Chapter 3 is devoted to study the interaction of two chemically and texturally different calcium phosphates i.e. biomimetic CDHA and sintered ß-tricalcium phosphate (ß-TCP) with macrophages under inflammatory environment and its further impact on osteogenic differentiation of mesenchymal (MSCs) and osteoblastic cells. Angiogenesis ensures the availability of oxygen and nutrients and controls the recruitment and osteogenic differentiation of bone forming cells, being one of the crucial processes during bone healing. Chapter 4 explores the interaction of two texturally different CDHA, i.e. needle-like and plate-like structured surfaces, as well as ß-TCP with endothelial progenitor cells (EPCs). Moreover, the cellular crosstalk between EPCs and MSCs is investigated by performing co-culture studies. Finally, the independent effect of surface chemistry and ionic exchange of sintered and biomimetic calcium phosphates on osteogenesis are explored in Chapter 5 and Chapter 6. Chapter 5 describes the response of MSCs and osteoblastic cells to biommimetic CDHA and sintered CaPs i.e. ß-TCP, a-tricalcium phosphate (a-TCP) and hydroxyapatite (HA). The results showed that the direct contact with substrate is required to induce the osteogenic differentiation of both MSCs and osteoblasts. Moreover, the culture with the CDHA resulted in greater upregulation of ALP secretion for both cell types suggesting osteoinductive capacity of this substrate. Nonetheless, MSCs proliferation was impaired both in direct and indirect cultures on CDHA, suggesting that the increased ionic fluctuations, triggered by high SSA and calcium deficiency of the substrate, were responsible for this scenario. Hence Chapter 5 revealed the complexity to assess the cell-material interactions of highly reactive substrates in static in vitro cultures. This is further explored in Chapter 6, where a strategy is proposed to analyse the parameters affecting the response of MSCs to CDHA. The approach consisted of adjusting the volume ratio between the cell culture medium and CaP substrate, which allowed mitigating the drastic ionic changes. The results demonstrated that the alterations of calcium and phosphate concentrations impaired cell adhesion by reducing the number of focal adhesions, this leading to cell shrinkage and apoptosis. On the contrary, when the ionic fluctuations were attenuated, MCSs spread, proliferated and differentiated over time.

Book excerpt: "Poly(etheretherketone) (PEEK) is a synthetic polymer with great potential for use as orthopedic implants with its radiolucency and mechanical properties close to those of native bone. Despite these advantages, PEEK implants tend to fail because immune cells, such as macrophages, prevent the attachment of bone by inducing chronic inflammation and fibrous encapsulation. Surface modification can improve osseointegration, but the mechanisms underlying the improvements are not well known. Since surface chemistry has been linked to reductions in fibrous encapsulation of model surfaces, this thesis aimed to investigate the effect of surface chemistry on the steps in the process of osseointegration, such as protein adsorption and macrophage responses, to provide insight about the role of surface chemistry in osseointegration. This thesis begins with a literature review that describes various surface modification strategies used to improve the integration of PEEK implants. The review proposed the development of surfaces with controlled properties to better understand how surface modifications influence the osseointegration of PEEK, and it highlighted a need for including protein adsorption and macrophage responses in future studies to better understand the full process of osseointegration around implants with modified surfaces. In the first study, a model surface, poly(styrene) (PS), with controlled surface chemistry was developed to investigate the effect of surface chemistry on protein adsorption and macrophage responses through diazonium chemistry reactions. Three common biological functional groups (COOH, NH2, and PO3H2) were selected and attached to PS surfaces in similar surface densities to compare the chemistries directly. Surfaces containing COOH groups increased the expression of anti-inflammatory macrophage markers while surfaces with NH2 groups exacerbated the response to an inflammatory stimulus. Characterization of the adsorbed protein layer demonstrated that surfaces with COOH groups adsorbed more proteins associated with integrin signaling from serum while surfaces with NH2 groups adsorbed more proteins associated with inflammatory reactions. These results demonstrated that surface chemistry could modulate macrophage responses through alterations in the layer of proteins adsorbed on the surfaces. In the second study, NH2 and COOH groups, which provoked pro- or anti-inflammatory macrophage responses, were selected and attached to PEEK surfaces to investigate whether these different macrophage responses could enhance the osseointegration of PEEK. In vitro, NH2 and COOH groups on PEEK promoted relatively pro- and anti-inflammatory macrophage responses as they did on PS, and protein adsorption on these surfaces also followed similar patterns. After 14 days in vivo, the NH2 groups promoted bone thinning while COOH groups had no effect. Surprisingly, the NH2 groups significantly increased adhesion to bone compared to the unmodified PEEK implants while COOH groups increased adhesion but not significantly. Since the NH2 groups were able to promote mineral deposition in acellular environments better than COOH groups, the NH2 groups may have enhanced mineral-binding in vivo. These results suggested different mechanisms involved in the osseointegration of NH2 and COOH-functionalized implants"--
