Abstract: Abstract of the thesis entitled An epidemiological study on the living environment, passive smoking and respiratory health of a cohort of children aged 3-6 years in Hong Kong submitted by CHUNG Siu Fung for the Degree of the Master of Philosophy at the University of Hong Kong. in March 1996 Objectives: To describe the health experience (with the focus on respiratory illnesses) of a cohort of children and to analyse the associations between the children's health and their living environment and exposure to passive smoking. Design: Cross-sectional study. Subjects: All the 3230 children born between April 1987 and June 1989 in Hong Kong who had participated in "A survey on breast-feeding and fertility in Hong Kong" in 1988-89 were eligible as subjects. From December 1992 to October 1993, when the children were aged 3-6 years, information of 2303 (71.3%) children were obtained from their parents who were interviewed by telephone. Results: The prevalence of diseases and health care utilization in the present study was quite comparable with those reported in other local studies. When compared with other places, the prevalence of health care utilization and most respiratory diseases and symptoms was not high. However, the cumulative prevalence of allergic rhinitis in Hong Kong was three times more than that in the UK, and was alarming with a prevalence of 17.3% in itself. Bad outdoor air quality in the living districts (i.e. respirable suspended particulates exceeded the Air Quality Objective) was significantly associated with the symptom "often had blocked or runny nose" in the previous twelve months (adjusted O.R.=1.31, 95% C.I. 1.06-1.60). Only among those children who lived in living quarters with an area of three hundred square feet or less, was incense burning in the living quarters significantlyassociated with the symptom "often had blocked or runny nose" (adjusted O.R.=1.61, 1.01-2.57) and with more than eight episodes of common cold (adjusted O.R.=1.62, 95% C.I. 1.05-2.50) in the previous twelve months. Grandparents smoking was significantly associated with the children's "often cough" (adjusted O.R.=1.81, 95% C.I. 1.08-3.06), doctor consultation for often cough/phlegm (adjusted O.R.=1.86, 95% C.I. 1.08-3.19) and common cold which usually lasted for more than six days in one episode (adjusted O.R.=1.64, 95% C.I. 1.05-2.57) in the previous twelve months. Other family members smoking was significantly associated with the children's ever having doctor diagnosed otitis media (adjusted O.R.=4.33, 95% C.I. 1.25-5.0), doctor consultation for respiratory problems in the previous thirty days (adjusted O.R.=2.04, 95% C.I. 1.05-3.93) and the children's health being perceived as 'not good' (adjusted O.R.=2.21, 95% C.1.1.11-4.44). Adverse effects of passive smoking were more likely to be found if the children lived in small living quarters. Among the children who lived in living quarters with an area of three hundred square feet or less, maternal, grandparents and other family members smoking were significant risk factors for some current respiratory symptoms. If they lived with two or more smokers or, in a family whose members smoked more than twenty cigarettes totally per day, when compared with those lived with no smokers, significant associations were found with most items of current respiratory symptoms. All rights not granted by the above license are retained by the author. Abstract: Introduction: Influenza illness is considered to be associated with high mortality, morbidity and economic loss. Numerous studies and mathematical simulations are conducted to understand the disease pattern, transmission dynamics, and psychosocial response of the population; however, little is known about the health seeking behavior of patients after the onset of respiratory infection symptoms. Respiratory infections exert substantial burden on the health care system of Hong Kong, therefore in addition to detecting the disease early, it is necessary to understand health care utilization behavior of the population. This information will not only benefit to formulate public health policies, it will also improve the preparedness plan to deal with future pandemics. Objective: To examine the influence of factors such as gender, age, education and occupation on the health seeking behavior of Hong Kong citizens and to understand the magnitude of health care utilization by the patients with respiratory infections. Method: This study is based on the data from a large randomized controlled trial of 703 households comprising of 2,586 individuals. Bi-weekly telephone contact was established with these households to assess the health seeking behavioral response of the individuals with respiratory infection episodes separately for laboratory confirmed influenza, and two other clinically defined infection episodes that are influenza like illness (ILI) and acute respiratory infection (ARI) episodes. Descriptive analyses were performed to assess the health care utilization within various health care delivery institutions. Multivariable logistic regression models were applied to determine the association of demographic factors with the health seeking behavior of individuals with respiratory infection episodes. Fischer exact test was used to analyze the association of promptness of health care contact with ARI and ILI episodes. Multivariable logistic regression model was used to analyze the association of demographic factors of ILI patients with early health care contact. Results: The results showed that seeking health care varied by the severity of illness and characteristics of the patients/ parents. As compared with the patients of ARI (non-ILI) episodes, higher proportion of ILI patients sought medical care and among all the health care delivery systems, private health care delivery system was attended by majority of the patients. Individuals with ILI episodes more likely to seek health care were male patients by 66% (Crude OR 1.66; 95% CI 1.01-2.77), and children below 10 years by 219% (Crude OR 3.19; 95% CI 1.30-7.79). Within the group of laboratory confirmed influenza patients, adjusted odds ratio showed male patients 243% more likely to seek health care (Adjusted OR 3.43; 95% CI 1.01-11.63). Unemployed ILI adult patients were less likely to seek medical care (Crude OR 0.39; 95% CI 0.15-0.98), and employed ARI (non-ILI) patients were 52% more likely to attend medical care (Crude OR 1.52; 95% CI 1.10-2.08). Health care contact within 24 hours of onset of symptoms was significantly associated with ILI patients (OR 1.85; 95% CI 1.33-2.59; p-value 0.0003) showing that patients with febrile ARI were more likely to seek early health care rather than patients with afebrile ARI. Abstract: Background: Air pollution is a serious concern all over the world, and there have been numerous studies showing its adverse effects on health outcomes including disease-specific hospitalization and mortality. Most of these studies were conducted in Western settings and focused on NO2, SO2, PM10 and black smoke; limited results on PM2.5 in Asia have been published. Objectives: This study attempted to identify association between PM2.5 concentration and daily mortality due to all-natural causes, cardiovascular diseases, and respiratory diseases in Hong Kong, a tropical city in Asia from 2008 to 2011. Methods: The study used a case-crossover study design with time-stratified referent selection strategy. The referents were selected on the same day of the week of the same month and year as the event day. In this way, biases due to autocorrelation, time trend, and seasonal pattern were controlled for by study design. Covariates including temperature, humidity, and gaseous pollutants (NO2, SO2, O3 and CO) were adjusted for by statistical modelling. The statistical method applied was Conditional Logistic Regression. Sensitive analyses using matched by month strategy were also conducted to check the robustness of the main analyses. The health outcome variables included were mortality due to all natural causes, cardiovascular disease, and respiratory disease. Each model was examined for effects of PM2.5 at each lag 0 through lag 5 day and, for the current and lag 1 day moving average (lag 0-1). Effects were measured in Excess Risks (ER) associated with 10 μg/m DEGREES3 increase in PM2.5. Results: Significant associations with PM2.5 were observed for mortality from all natural causes and cardiovascular disease at lag 0, lag 1, and lag 0-1; and from respiratory disease only at lag 1. At lag 0-1, PM2.5 was associated with mortality from all natural causes, cardiovascular disease, and respiratory disease with the ER (95% confidence interval) of 0.74% (95% CI: 0.29, 1.19), 1.4% (95% CI: 0.52, 2.27), and 0.67% (95% CI: -0.30, 1.64), respectively. After adjusting for each of the co-pollutants in two-pollutant model, the effect magnitude dropped except that one adjusting for CO. The ERs of mortality in co-pollutant models from all natural causes ranged from 0.23% to 1.72%, from cardiovascular disease ranged from 1.00% to 2.93%, and from respiratory disease ranged from -0.10% to 1.83%. The results were robust in sensitivity analyses. Conclusion: My study provides some information to support formulation of air quality control strategies and policies, and for updating air quality standards. Such information includes the overall and seasonal patterns of air pollutants and mortality in Hong Kong, as well as the excess risks of mortality associated with increase in PM2.5. Studies with individual data stratified for subgroups can be conducted in the future to investigate effect modification of lifestyle factors for the individuals and population. Book excerpt: The emergence of severe acute respiratory syndrome (SARS) in late 2002 and 2003 challenged the global public health community to confront a novel epidemic that spread rapidly from its origins in southern China until it had reached more than 25 other countries within a matter of months. In addition to the number of patients infected with the SARS virus, the disease had profound economic and political repercussions in many of the affected regions. Recent reports of isolated new SARS cases and a fear that the disease could reemerge and spread have put public health officials on high alert for any indications of possible new outbreaks. This report examines the response to SARS by public health systems in individual countries, the biology of the SARS coronavirus and related coronaviruses in animals, the economic and political fallout of the SARS epidemic, quarantine law and other public health measures that apply to combating infectious diseases, and the role of international organizations and scientific cooperation in halting the spread of SARS. The report provides an illuminating survey of findings from the epidemic, along with an assessment of what might be needed in order to contain any future outbreaks of SARS or other emerging infections.

However, little is known about the hazard of thirdhand smoke (THS) and secondhand smoke at home that comes from neighbourhood. This survey aims to investigate the association of respiratory symptoms with exposure to SHS at home from inside home, SHS at home from neighbourhood, THS at home, and SHS outside home in Hong Kong primary school students. The association of respiratory symptoms with different combinations of SHS and THS exposure was also investigated. Methods: This was a cross-sectional study conducted in 2010-11 with 20015 primary 4-6 (P4-6) students from 96 randomly selected schools in Hong Kong to collect self-reported data, using a purposely-designed questionnaire, from school-aged children on relevant demographics, respiratory symptoms and exposure to SHS and THS. To investigate the association of respiratory symptoms with SHS and THS exposure in never smoking and ever smoking primary school students, logistic regression analyses were conducted adjusting for potential confounders (age, sex, place of birth and number of bedrooms), and with mutual adjustment of various tobacco smoke exposures. Results: In never smoking primary students, compared with the unexposed group (0 day/week), the adjusted odds ratios (AORs) (95% confidence interval) were 1.04 (0.92-1.19) for exposure to SHS at home from inside home for 1-4 days/week and 1.22 (0.99-1.51) for 5-7 days/week. The corresponding figures were 1.16 (1.01-1.32) and 1.48 (1.07-2.04) for exposure to neighbourhood SHS, 1.44 (1.26-1.64) and 1.21 (0.84-1.77) for exposure to THS at home, and 1.02 (0.93-1.12) and 1.27 (1.08-1.49) for exposure to SHS outside home. Among all different SHS and THS exposure combinations, in both crude and adjusted models, exposure to more sources of SHS and THS was generally associated with higher odds of respiratory symptoms. Exposure to all 4 sources of SHS and THS was associated with an AOR of 1.86 (1.40-2.45) for respiratory symptoms. In ever smoking primary students, the sample size was too small to generate consistent results and most AORs were non-significant due to the low smoking prevalence. Conclusions: Respiratory symptoms were significantly associated with exposure to secondhand smoke at home from neighbours, thirdhand smoke at home and secondhand smoke outside home in Hong Kong never smoking primary school students. This study was the first to research the association of SHS exposure from neighbours with respiratory symptoms among children in Hong Kong and a strong association was observed. The association between SHS exposure and respiratory symptoms may be underestimated if the exposure from neighbours was ignored. These results support more stringent smokefree policies in Hong Kong.
