Cependant, il n'est pas rare que les praticiens soient confrontés à une insuffisance de volume osseux nécessitant des techniques chirurgicales de reconstruction osseuse. Toutefois, si la présence d'un volume osseux suffisant demeure indispensable, aujourd'hui, l'analyse préimplantaire a évolué en intégrant le concept d'une implantologie « guidée par la prothèse » ; les crêtes édentées étant fréquemment déformées et incompatibles avec la réhabilitation prothétique anticipée, de nombreuses techniques de greffes osseuses et de régénération osseuse guidée ont été proposées pour rétablir un volume osseux adéquat. Cependant, les techniques de greffes osseuses comportent des protocoles assez lourds nécessitant une expérience et une courbe d'apprentissage non négligeables, avec des risques opératoires et un taux de morbidité parfois élevé pour le patient. Grâce à l'avènement des technologies numériques et radiologiques et l'arrivée de nouveaux matériaux de greffe, les techniques d'augmentation osseuse se sont affinées avec des taux de succès indéniables. Aujourd'hui, l'analyse pré-implantaire permet d'anticiper le volume osseux nécessaire à une réhabilitation prothétique et de réaliser des augmentations de volume osseux plus prévisibles. Dans un premier temps, nous ferons un rappel sur les facteurs indispensables à respecter lors d'une analyse pré-implantaire, notamment le volume osseux nécessaire à un projet prothétique pré-établi. Puis nous décrirons les différentes techniques permettant d'augmenter le volume osseux et leur évolution. Le sujet étant vaste, nous limiterons notre exposé au secteur antérieur maxillaire. Nous terminerons par une discussion sur la fiabilité de ces greffes osseuses, les résultats obtenus d'après la littérature scientifique, ainsi que les perspectives que nous offrent le génie tissulaire.
