Book excerpt: Recently the Naval Blood Research Laboratory has been freezing red blood cells in a collection system which uses either a triple or a quadruple bag setup. Blood collection and red blood cell biochemical modification and freeze-preservation can be accomplished with this system. For the past 6 years the laboratory has been biochemically modifying liquid-stored red blood cells to increase their 2,3 DPG and ATP levels and thus improve the oxygen transport function of the preserved red blood cells upon infusion. Biochemically modified red blood cells, or 'rejuvenated' red blood cells as they are sometimes referred to, can be freeze-preserved or not as the situation demands. Red blood cells that have exceeded their mandatory shelf-life have been biochemically modified with a solution of pyruvate, inosine, glucose, phosphate, and adenine. The stored red blood cells are incubated with the 'rejuvenation' solution for 1 hour at 37 C. Biochemically modified red blood cells must be washed before transfusion whether or not they have been frozen to remove the additives used in the solution. This report describes a new blood collection system. The cost of processing can be reduced at least 50% the storage capacity in the -80 C mechanical refrigerator can be doubled, and the potential for contamination of the blood product is reduced. Biochemically modified freeze-preserved red blood cells have been used successfully in a number of important clinical situations.

Book excerpt: A method is described for the preparation and storage of baboon red blood cells, frozen, in the original polyvinylchloride (PVC) plastic collection bag. Up to four hundred fifty (450 + 45 ml) ml of blood is collected in a quadruple plastic bag system with a special adaptor port on the tubing connecting the primary 800 ml PVC plastic bag to the three transfer packs. The red cells are concentrated by centrifugation of the primary collection bag and all of the plasma is expressed into one of the integrally attached transfer packs. The red cell concentrate in the 800 ml primary collection bag with a hematocrit of 75 +/- 5 V% is stored with an empty integrally attached transfer pack. Glycerol solution is added through the collection line of the primary 800 ml collection bag. Three aliquots of glycerol solution are introduced with short equilibration periods between each addition. The red cells are then concentrated by centrifugation, the supernatant glycerol is transferred into the empty integrally attached transfer pack and discarded. The glycerolized packed red cells are then frozen. The procedure uses a high concentration of glycerol (40% w/v) and mechanical refrigeration at -80 C for the freezing and storage of the product.

Outdated red cells stored at 4 C for 36 to 38 days can be biochemically modified to increase the 2,3 DPG level to 150% of normal and the ATP level to 175% of normal; this procedure prepares outdated-rejuvenated red cells. Biochemically modified indated-frozen-thawed-washed red cells have improved oxygen transport function and biochemically modified outdated frozen-thawed-washed red cells have normal oxygen transport function. After the rejuvenation procedure, the red cells are concentrated, the supernatant removed, and the red cells glycerolized prior to freezing at -80 C.

Results of the U.S. Navy field- testing of the first generation frozen blood bank system in Vietnam between 1966 and 1968 have demonstrated the feasibility of a frozen blood bank system to supplement the liquid blood bank system deployed at a fixed medical facility, in this case at Danang, South Vietnam and aboard the hospital ships USS Repose and USS Sanctuary. Further research conducted after the Vietnam war resulted in marked simplification in red cell freezing methods and in the development of practical and simple methods to salvage outdated O-positive and O-negative red cells by biochemical treatment prior to freeze-preservation. The second generation integrated liquid-frozen blood bank system has been field-tested. Nonrejuvenated, indated-rejuvenated and outdated-rejuvenated cells are frozen with 40% W/V glycerol and stored at -80 C in the primary bag of a polyvinylchloride plastic multiple-bag collection system. After thawing, the red cells are washed with 1.5 liters of a crystalloid solution, composed of a sodium chloride-glucose-phosphate solution, pH 6.8. Platelet freezing methods also have been improved upon: 6 to 8 units of platelets are isolated from individual units of blood, pooled, and frozen with 6% DMSO and storage at -80 C in a polyvinylchloride plastic bag. After thawing, the platelets are washed with 250 ml of a solution composed of sodium chloride-glucose-phosphate, pH 5.0.
