Book excerpt: Tinnitus (ringing in the ears) is a prevalent and often debilitating disorder with approximately 10% of people (incorporating ages from children to the elderly) perceiving it continuously, and in 1-3% of the population it seriously affects the quality of life. The most common cause of tinnitus is hearing loss, and its prevalence has surged as a result from the various large-scale military actions in the Middle East in the last decade. Recent advances have been made in the area of behavioral animal models, in the understanding of human brain imaging aspects of tinnitus, and in addressing the long-range changes in human brain connectivity. Furthermore continued exploration of the three major animal models of tinnitus: salicylate-induced, noise trauma induced, and resulting from somatic interactions with the auditory system has further delineated the relative roles of cochlear activity vs. central auditory system changes. Evidence for the role of neural synchrony changes in tinnitus originates both from human EEG and MEG studies as well as from neuron pair-correlation studies in animals.

Even in the late 20th century, consciousness was considered to be beyond the reach of science. Now, understanding the neural mechanisms underlying consciousness is recognized as a key objective for 21st century science. The cognitive neuroscience of consciousness is a fundamentally multidisciplinary enterprise, involving powerful new combinations of functional brain imaging, computational modelling, theoretical innovation, and basic neurobiology. Its progress will be marked by new insights not only into the complex brain mechanisms underlying consciousness, but also by novel clinical approaches to a wide range of neurological and psychiatric disorders. These innovations are well represented by the contents of the present volume. A target article by Victor Lamme puts forward the contentious position that neural evidence should trump evidence from behaviour and introspection, in any theory of consciousness. This article and its several commentaries advance one of the fundamental debates in consciousness science, namely whether there exists non-reportable phenomenal consciousness, perhaps dependent on local rather than global neural processes. For example, repetitive transcranial magnetic stimulation (rTMS) over prefrontal areas has been extensively investigated as a treatment for patients with medication-resistant depression and has been shown to be associated with improvement of mood. Similarly, non-invasive stimulation techniques have been applied to various symptoms of Parkinson's disease such as bradykinesia and dyskinesias, with variables degrees of success reported. However, attempts to expand previously observed clinical improvements to other neurological disorders (e.g. Tourette's syndrome, autism, epilepsy) has been controversial. In trying to bypass potential confounding elements, researchers aim to target neural populations altered in disease to either increase or decrease their corrupted baseline activity. In addition, a complementary approach is to extend stimulation protocols that results enhanced behavior in healthy participants. This Frontiers Research Topic on non-invasive brain stimulation and enhancement of function tries to combine a series of articles from researchers who used non-invasive brain stimulation to aim improvement of either a motoric, cognitive or behavioral nature investigated behaviorally, physiologically or using brain imaging techniques in clinical populations. Investigation of the relation between enhancement of function in healthy populations and clinical improvement in patients with neurological or psychiatric disorders needs further consideration.
