Book excerpt: The dramatically increasing aging population of Hong Kong has elicited new risks and opportunities to facilitate a positive life for older adults. This book offers a holistic review of gerontological theories and literature, and constructs a conceptual framework of social support networks, coping and positive aging. In light of the implications of the convoy model of social support to depict an indigenous landscape of positive aging in Hong Kong, this is one of the very few empirical studies that adopts both quantitative research and qualitative research. The research consisted of a pilot study of in-depth interviews with 16 older Hong Kong Chinese and a main study surveying 393 older members of District Elderly Community Center. The results of the study indicate that family and peer support constitute the mainstay of support networks of the elderly, and that family and peer support are associated with positive aging. Moreover, the study shows that it is the depth of emotional closeness, namely, close interaction and intimacy with social partners that makes the greatest contribution to positive aging. Additionally, problem coping and emotion coping are found to mediate the relationship between social support networks and positive aging. There is potential in bringing more domestic helpers into elderly care and improving the service quality such that the goal of Aging in Place can be promoted in Hong Kong. Intended for researchers in social work, gerontology and positive psychology, it is also essential reading for graduates and social work professionals interested in this area. This book makes a valuable contribution to social gerontological research among Hong Kong older adults and the promotion of wellbeing in the elderly via the construct of positive aging in the culture of Chinese society.

Approximately one-quarter of community-dwelling Americans aged 65 and older are considered to be socially isolated, and a significant proportion of adults in the United States report feeling lonely. People who are 50 years of age or older are more likely to experience many of the risk factors that can cause or exacerbate social isolation or loneliness, such as living alone, the loss of family or friends, chronic illness, and sensory impairments. Over a life course, social isolation and loneliness may be episodic or chronic, depending upon an individual's circumstances and perceptions. A substantial body of evidence demonstrates that social isolation presents a major risk for premature mortality, comparable to other risk factors such as high blood pressure, smoking, or obesity. As older adults are particularly high-volume and high-frequency users of the health care system, there is an opportunity for health care professionals to identify, prevent, and mitigate the adverse health impacts of social isolation and loneliness in older adults. Social Isolation and Loneliness in Older Adults summarizes the evidence base and explores how social isolation and loneliness affect health and quality of life in adults aged 50 and older, particularly among low income, underserved, and vulnerable populations. This report makes recommendations specifically for clinical settings of health care to identify those who suffer the resultant negative health impacts of social isolation and loneliness and target interventions to improve their social conditions. This is a cross-sectional survey of 2870 community-dwelling persons aged 65 or older living in Switzerland. All persons received a multidimensional health risk appraisal questionnaire. Lubben Social Network Scale and MOS social support survey questions were used for measurement of social network and support. We examined l ) the association between social ties and levels of social support and 2) the association of social ties with demographic, socio-economic, and health-related characteristics based on linear and logistic regression models. Low levels of both emotional and instrumental support were independently related with the presence of low family and friendship ties. Factors related with low levels of overall social network were poor self-perceived health status, low educational level, rural living location (vs. urban), presence of depressive symptoms, and advanced age. Factors related with lower family ties but not with friendship ties were living location in an urban area and having fair or poor selfperceived health status. Factors related with lower friendship ties but not with family ties were the presence of limitations in activities of daily living, and low level of education. In conclusion, this study is evidence that a multidimensional health risk appraisal in older persons containing a social network module is a promising method for identifying persons at increased risk for social isolation, as well as for identifying the subcomponents of the social network and specific, potentially reversible factors associated with marginal social ties. This is a promising strategy for the development of intervention programs that attempt to improve social network resources. This difference is particularly notable given that the United States spends more on health care than any other nation. Concerned about this divergence, the National Institute on Aging asked the National Research Council to examine evidence on its possible causes. According to Explaining Divergent Levels of Longevity in High-Income Countries, the nation's history of heavy smoking is a major reason why lifespans in the United States fall short of those in many other high-income nations. Evidence suggests that current obesity levels play a substantial part as well. The book reports that lack of universal access to health care in the U.S. also has increased mortality and reduced life expectancy, though this is a less significant factor for those over age 65 because of Medicare access. For the main causes of death at older ages-cancer and cardiovascular disease-available indicators do not suggest that the U.S. health care system is failing to prevent deaths that would be averted elsewhere. In fact, cancer detection and survival appear to be better in the U.S. than in most other high-income nations, and survival rates following a heart attack also are favorable. Explaining Divergent Levels of Longevity in High-Income Countries identifies many gaps in research. For instance, while lung cancer deaths are a reliable marker of the damage from smoking, no clear-cut marker exists for obesity, physical inactivity, social integration, or other risks considered in this book. Moreover, evaluation of these risk factors is based on observational studies, which-unlike randomized controlled trials-are subject to many biases.
