Book excerpt: Improving and Accelerating Therapeutic Development for Nervous System Disorders is the summary of a workshop convened by the IOM Forum on Neuroscience and Nervous System Disorders to examine opportunities to accelerate early phases of drug development for nervous system drug discovery. Workshop participants discussed challenges in neuroscience research for enabling faster entry of potential treatments into first-in-human trials, explored how new and emerging tools and technologies may improve the efficiency of research, and considered mechanisms to facilitate a more effective and efficient development pipeline. There are several challenges to the current drug development pipeline for nervous system disorders. The fundamental etiology and pathophysiology of many nervous system disorders are unknown and the brain is inaccessible to study, making it difficult to develop accurate models. Patient heterogeneity is high, disease pathology can occur years to decades before becoming clinically apparent, and diagnostic and treatment biomarkers are lacking. In addition, the lack of validated targets, limitations related to the predictive validity of animal models - the extent to which the model predicts clinical efficacy - and regulatory barriers can also impede translation and drug development for nervous system disorders. Improving and Accelerating Therapeutic Development for Nervous System Disorders identifies avenues for moving directly from cellular models to human trials, minimizing the need for animal models to test efficacy, and discusses the potential benefits and risks of such an approach. This report is a timely discussion of opportunities to improve early drug development with a focus toward preclinical trials.

Book excerpt: Implantable technologies allow for a sustained control over the release of pharmaceuticals into the bloodstream thereby achieving a controlled concentration with the potential to minimise side-effects while increasing patient compliance. Significant progress has been made in various alternative implantable delivery technologies, notably in intraocular and subcutaneous devices. Despite success in research and clinical studies, long-term clinical efficacy may be more limited and different aspects related to drug development and commercialization using these technologies are not well understood or practiced in the commercial setting. This book provides a comprehensive and cohesive picture of the latest in the field while also outlining the opportunities and challenges in implantable technology. Implantable Technologies: Pepties and Biologic Drug Development is an ideal reference for any postgraduate or researcher interested in utilising implantable technologies and novel routes of drug administration. The book will also be of interest to those involved in formulation and clinical application for a wide array of disease areas in addition to more established paradigms such as diabetes and pain management.

Available in PDF, EPUB and Kindle. Book excerpt: The brain is the most complex organ in our body. Indeed, it is perhaps the most complex structure we have ever encountered in nature. Both structurally and functionally, there are many peculiarities that differentiate the brain from all other organs. The brain is our connection to the world around us and by governing nervous system and higher function, any disturbance induces severe neurological and psychiatric disorders that can have a devastating effect on quality of life. Our understanding of the physiology and biochemistry of the brain has improved dramatically in the last two decades. In particular, the critical role of cations, including magnesium, has become evident, even if incompletely understood at a mechanistic level. The exact role and regulation of magnesium, in particular, remains elusive, largely because intracellular levels are so difficult to routinely quantify. Nonetheless, the importance of magnesium to normal central nervous system activity is self-evident given the complicated homeostatic mechanisms that maintain the concentration of this cation within strict limits essential for normal physiology and metabolism. There is also considerable accumulating evidence to suggest alterations to some brain functions in both normal and pathological conditions may be linked to alterations in local magnesium concentration. This book, containing chapters written by some of the foremost experts in the field of magnesium research, brings together the latest in experimental and clinical magnesium research as it relates to the central nervous system. It offers a complete and updated view of magnesiums involvement in central nervous system function and in so doing, brings together two main pillars of contemporary neuroscience research, namely providing an explanation for the molecular mechanisms involved in brain function, and emphasizing the connections between the molecular changes and behavior. It is the untiring efforts of those magnesium researchers who have dedicated their lives to unraveling the mysteries of magnesiums role in biological systems that has inspired the collation of this volume of work.

Book excerpt: Schizophrenia is often associated with an inadequate response to pharmacological and non-pharmacological treatments. How to treat patients who have an unsatisfactory response to anti-psychotics, including clozapine - which is unequivocally the most powerful antipsychotic medication for this recalcitrant population - remains a clinical conundrum. A range of adjunctive medications have been tried with mixed results; there has also been renewed interest in the role of neuromodulatory strategies, electroconvulsive therapy, and cognitive and vocational approaches. Perhaps a bright spot for the future lies in the evolution of pharmacogenetic approaches for individualized care. In this book, leading experts from Europe, Australia and the Americas provide a timely appraisal of treatments for the most severely ill schizophrenia patients. This clinically focused book is informed by the latest research on the neurobiology and treatment of schizophrenia. It is comprehensive in scope, covering current treatment options, various add-on approaches, and a range of psychosocial treatments. The contributors are respected experts who have combined their clinical experience with cutting-edge research to provide readers with authoritative information on fundamental aspects of clinical care for schizophrenia.

At present, the conventional de novo drug discovery process requires an average of about 14 years and US$2.5 billion to approve and launch a drug. Drug repositioning can reduce the time and cost of this process because it takes advantage of drugs already in clinical use for other indications or drugs that have cleared phase I safety trials but have failed to show efficacy in the intended diseases. Historically, drug repositioning has been realized through serendipitous clinical observations or improved understanding of disease mechanisms. However, recent technological advances have enabled a more systematic approach to drug repositioning. This eBook collects 16 articles from 112 authors, providing readers with current advances and future perspectives of drug repositioning.

It is a field of study that describes the effects of drugs from the molecular to the behavioural level and requires integration and synthesis of knowledge from various disciplines including neuroanatomy, physiology, molecular biology, pharmacology and the behavioural sciences. The principal aims of this book are to provide students with a clear understanding of CNS disorders, and an appreciation of how basic and clinical research findings can be translated into therapeutics. After an introduction to the subject area, the remaining chapters are focused on reviewing the main psychiatric and neurological disorders that are covered in most courses. They are discussed in terms of their clinical symptoms, epidemiology, pathology, aetiology, underlying neurobiological and neurochemical mechanisms, pharmacotherapy, adjunctive non-pharmacological treatments, and clinical outcomes. Each chapter of the book is a ‘stand-alone’ chapter and is written in a clear, accessible style. Written by an author with many years teaching and research experience, this textbook will prove invaluable for students of pharmacology, pharmacy and the medical sciences needing a truly integrated introduction to this exciting field.
