Book excerpt: The skin is the first line of defense against chemical warfare agents including nerve agents and toxic industrial chemicals, providing a possible barrier or delay to systemic distribution. However, some chemicals act directly on the skin including vesicants sulfur mustard and corrosive compounds such as strong acids or bases, and do not have to gain access to systemic circulation to cause extensive skin damage. Early and rapid skin decontamination is extremely important following exposure to chemical warfare agents and toxic industrial chemicals because it decreases serious skin damage to the patient and, potentially, their doctor. This multi-authored international text pulls together a century of decontamination research and helps the reader expedite solutions that will decrease morbidity and mortality. Complete with dozens of hiqh quality photographs and illustrations, Skin Decontamination aids industrial hygiene, dermatology, occupational physicians and those involved in the public health arena.

Available in PDF, EPUB and Kindle. Book excerpt: The threat of domestic terrorism today looms larger than ever. Bombings at the World Trade Center and Oklahoma City's Federal Building, as well as nerve gas attacks in Japan, have made it tragically obvious that American civilians must be ready for terrorist attacks. What do we need to know to help emergency and medical personnel prepare for these attacks? Chemical and Biological Terrorism identifies the R&D efforts needed to implement recommendations in key areas: pre-incident intelligence, detection and identification of chemical and biological agents, protective clothing and equipment, early recognition that a population has been covertly exposed to a pathogen, mass casualty decontamination and triage, use of vaccines and pharmaceuticals, and the psychological effects of terror. Specific objectives for computer software development are also identified. The book addresses the differences between a biological and chemical attack, the distinct challenges to the military and civilian medical communities, and other broader issues. This book will be of critical interest to anyone involved in civilian preparedness for terrorist attack: planners, administrators, responders, medical professionals, public health and emergency personnel, and technology designers and engineers.

This book was released on 2017-01-27 with total page 184 pages. Available in PDF, EPUB and Kindle. Book excerpt: Surgical site infections are caused by bacteria that get in through incisions made during surgery. They threaten the lives of millions of patients each year and contribute to the spread of antibiotic resistance. In low- and middle-income countries, 11% of patients who undergo surgery are infected in the process. In Africa, up to 20% of women who have a caesarean section contract a wound infection, compromising their own health and their ability to care for their babies. But surgical site infections are not just a problem for poor countries. In the United States, they contribute to patients spending more than 400 000 extra days in hospital at a cost of an additional US $10 billion per year. No international evidence-based guidelines had previously been available before WHO launched its global guidelines on the prevention of surgical site infection on 3 November 2016, and there are inconsistencies in the interpretation of evidence and recommendations in existing national guidelines. These new WHO guidelines are valid for any country and suitable to local adaptations, and take account of the strength of available scientific evidence, the cost and resource implications, and patient values and preferences.

Available in PDF, EPUB and Kindle. Book excerpt: This book is a comprehensive volume dedicated to dermal absorption and decontamination. Given risks including chemical warfare and accidents with toxic industrial chemicals, studies involving dermal absorption and decontamination are of the utmost importance. Each chapter consists of a systematic review on an important dermal absorption and/or decontamination topic. Chapters address key issues such as the general acceptance of water/ soap and water as the gold standard of dermal decontamination despite the fact that it commonly provides incomplete decontamination and may even result in a “wash-in effect” (an increased penetration of contaminant). Decontamination with other substances (e.g. RSDL) is explored, and factors that affect dermal penetration (e.g. anatomical variation) are summarized. All systematic reviews are comprehensive and up-to-date. Dermal Absorption and Decontamination will be useful for many fields and disciplines. Firstly, information on dermal decontamination may be used by governments for military and national defense purposes in the event of chemical warfare. Additionally, this information would be of use to many production companies, whose employees are at risk of an accident with toxic industrial chemicals. Topics covered in the book will also be of use to pharmaceutical companies, especially those exploring transdermal delivery of drugs and is of great relevance to those in the medical field, particularly dermatologists.

Book excerpt: In the concept exploration/demonstration and validation phase of the Personnel/Casualty Decontamination System Skin Decontamination Kit (PCDS SDK) development program, it was shown that a safe and effective skin decontamination system against chemical warfare (CW) agents can be designed based on Ambergard XE-555 or Ambergard XE-556 resins developed by Rohm and Haas Company. The prototype PCDS SDK developed as part of this contract evolved into a novel soft- pack design. Each PCDS SDK consists of six applications of decontamination material in individual flexible pouches with its own pad for applying, spreading, and removing the decon material. A Special In-Process Review recommended that the program proceed to full-scale development. Keywords; Decontamination kits.

Available in PDF, EPUB and Kindle. Book excerpt: The objective of this book is to present a comprehensive picture, first of the fundamentals of general contamination of solid surfaces and water, and in the second part, to review the main practical procedures and means of applied decontamination used in the fields of activity. The most emphasis on radioactive contamination deals with decontamination of the operational facilities in nuclear power plants. Other special decontamination branches of current interest are also dealt with briefly. The art of decontamination is being enriched by the progress achieved in relevant scientific disciplines and employs these relative advances. The current volume discusses in detail the following trends in decontamination: Firstly, the development and use of new decontamination methods that are highly efficient, non-agressive to decontaminated materials, and economically feasible; secondly, the utilization of progressive elements of automation and robotics; thirdly, the development and use of such decontamination formulations that would minimize the volume of wastes and would produce wastes in a form in which they could be either easily further treated or safely disposed of without risk to human health or the environment; finally, the choice of suitable materials used both for the structural and the technological parts of nuclear installations with regard to their minimal contaminability and ease of decontamination.

Book excerpt: Current doctrine describes the use of the M291 Skin Decontamination Kit (SDK), 0.5% hypochlorite solution (household bleach diluted 1 to 10) and 1% soapy water solution to decontaminate skin exposed to chemical warfare agents. Reactive Skin Decontamination Lotion (RSDL) is a new product being considered and was recently approved by the FDA. This study directly compares the efficacy of these four decontamination products, in the clipped haired guinea pig model, following challenges by soman (GD), VX, and Russian VX (VR). The protective ratio (PR) was calculated from the derived median lethal dose-response curves established for each agent and non-decontaminated control animals. RSDL was found to be the most effective decontamination system, whereas the M291 SDK was the least effective.

Book excerpt: This Open access book offers updated and revised information on vessel health and preservation (VHP), a model concept first published in poster form in 2008 and in JVA in 2012, which has received a great deal of attention, especially in the US, UK and Australia. The book presents a model and a new way of thinking applied to vascular access and administration of intravenous treatment, and shows how establishing and maintaining a route of access to the bloodstream is essential for patients in acute care today. Until now, little thought has been given to an intentional process to guide selection, insertion and management of vascular access devices (VADs) and by default actions are based on crisis management when a quickly selected VAD fails. The book details how VHP establishes a framework or pathway model for each step of the patient experience, intentionally guiding, improving and eliminating risk when possible. The evidence points to the fact that reducing fragmentation, establishing a pathway, and teaching the process to all stakeholders reduces complications with intravenous therapy, improves efficiency and diminishes cost. As such this book appeals to bedside nurses, physicians and other health professionals.
