Book excerpt: Introduction : La souffrance psychique au travail est une situation touchant 17 % de la population salariée française en 2010. Les questionnaires de dépistage validés, comme le questionnaire de Karasek, ne sont pas adaptés à la consultation du médecin généraliste trop longs ou ne favorisant pas l'expression du patient. Quelles sont les questions les plus pertinentes du questionnaire de Karasek, pour la création d'un questionnaire simplifié, afin de dépister la souffrance au travail au sein d'une population active adulte consultant un médecin généraliste ? Méthode : Etude analytique transversale réalisée à l'aide d'auto-questionnaires anonymes remis aux patients salariés consultant dans les cabinets de médecine générale de la Communauté Professionnelle Territoriale de Santé Asclépios en Indre et Loire. Résultats : 237 questionnaires recueillis dont 213 analysés. 35,6 % des patients présentent une souffrance psychique au travail. Le modèle composite (conservant les 3 sous catégories du questionnaire de Karasek) est composé de 7 questions avec une sensibilité de 78 % et une spécificité de 83 % sur l'échantillon. Le modèle direct (ne tenant pas compte des sous catégories) est composé de 4 questions avec une sensibilité de 92 % et une spécificité de 79 % sur l'échantillon. Discussion : La souffrance psychique au travail est fréquente chez la population salariée. Les nouveaux modèles créés présentent de bonnes performances mais nécessitent d'être évalués sur une nouvelle population devant le risque de surmodélisation. Il apparaît nécessaire d'améliorer la formation des généralistes, les relations avec les autres professionnels de santé, et de trouver la place du dépistage de la souffrance psychique au travail dans la consultation de médecine générale afin de permettre une meilleure prise en charge.
