Book excerpt: Although there are population-based studies that find that sexual minorities are at increased risk for psychological distress and mental health disorders compared to heterosexuals, there have been few studies examining the way sexual minorities utilize mental health services within the U.S. health care system. With the reversal of the Defense of Marriage Act (DOMA) in 2013 and the Patient Protection and Affordable Care Act (PPACA), sexual minorities will have unprecedented access to health insurance coverage. The impact of this expanded coverage on mental health service utilization and cost is not known. This dissertation examines sexual minority mental health service utilization over three studies and two data sets. The first two studies examined sexual minority mental health service utilization using two different national data sets over several years: the National Health and Nutrition Examination Surveys (NHANES) from 2001-2010 and the Medical Expenditure Panel Surveys (MEPS) from 1996-2010. The third study used the MEPS to examine differences in out-of-pocket and total expenditures between individuals in same-sex couples compared to individuals in different-sex couples. This research found that sexual minorities were more likely to use mental health services compared to heterosexuals. In particular, sexual minorities were found to be primarily accessing this care in office-based visits and through psychopharmaceutical prescriptions. Despite this increase in access, sexual minorities were not more likely to receive minimally adequate care after initiating mental health services compared to heterosexuals. This increased utilization of care did translate into increased out-of-pocket mental health care expenditures and total mental health care expenditures for individuals in same-sex couples as compared to individuals in different-sex couples. This dissertation found that sexual minorities are accessing and using mental health services differently than heterosexuals. With the enactment of PPACA and the reversal of DOMA, sexual minorities should have unprecedented access to mental health services. Due to the limited research in this area, it remains unclear if there will be enough access to culturally competent mental health providers and how this access will impact overall access to already-scarce mental health services. Sexual minority and health policy researchers will need to seek novel data sets to examine the impact of this increased utilization on both mental health outcomes as well as health care expenditures.

Book excerpt: Researchers examining clinically-relevant trends for sexual minority women have found evidence of psychological distress and greater utilization of mental health services compared to heterosexually-identified women. However, the results of many research studies with this population have methodological limitations surrounding recruitment of participants, cross-sectional methods, and participant report of clinical services utilized. Few researchers have sought to analyze actual clinical presentation, service utilization, or clinical outcome of sexual minority women in a treatment-setting. This study is an in-depth clinical analysis of 215 sexual minority woman who sought services at a large Midwestern university counseling center over a period of two years and nine months. The analyses conducted allowed for the initial clinical presentation, service utilization, and clinical outcome of sexual minority women to be described and compared to comparative samples of service-seeking female college students. Sexual minority women were found to have a greater incidence of prior suicidality and prior counseling compared to heterosexual women at the same agency. Sexual minority women also reported greater symptoms of social anxiety, eating concerns, and family distress than heterosexual women at the same agency. Of seven examined predictors of service utilization, sexual minority identity label, age, and academic term of service initiation differentiated brief and extended service utilization from moderate service utilization. Prior use of counseling, psychological distress, racial-ethnic minority status, and time in the semester of initiation of services were not significant predictors of service utilization. Implications and limitations of these findings are presented, and future directions for research and intervention are discussed.

These disorders are among the most highly stigmatized health conditions in the United States, and they remain barriers to full participation in society in areas as basic as education, housing, and employment. Improving the lives of people with mental health and substance abuse disorders has been a priority in the United States for more than 50 years. The Community Mental Health Act of 1963 is considered a major turning point in America's efforts to improve behavioral healthcare. It ushered in an era of optimism and hope and laid the groundwork for the consumer movement and new models of recovery. The consumer movement gave voice to people with mental and substance use disorders and brought their perspectives and experience into national discussions about mental health. However over the same 50-year period, positive change in American public attitudes and beliefs about mental and substance use disorders has lagged behind these advances. Stigma is a complex social phenomenon based on a relationship between an attribute and a stereotype that assigns undesirable labels, qualities, and behaviors to a person with that attribute. Labeled individuals are then socially devalued, which leads to inequality and discrimination. This report contributes to national efforts to understand and change attitudes, beliefs and behaviors that can lead to stigma and discrimination. Changing stigma in a lasting way will require coordinated efforts, which are based on the best possible evidence, supported at the national level with multiyear funding, and planned and implemented by an effective coalition of representative stakeholders. Ending Discrimination Against People with Mental and Substance Use Disorders: The Evidence for Stigma Change explores stigma and discrimination faced by individuals with mental or substance use disorders and recommends effective strategies for reducing stigma and encouraging people to seek treatment and other supportive services. It offers a set of conclusions and recommendations about successful stigma change strategies and the research needed to inform and evaluate these efforts in the United States.

Book excerpt: Prior studies have noted significant health disparities experienced by sexual minority individuals with military histories compared to their heterosexual peers. Often the minority stress model, which posits sexual minority individuals experience unique chronic stressors that affect their well-being and fuel health disparities, is used to conceptualize these differences. Results from several studies have identified that fears or experiences of anticipated stigma and experiences of discrimination related to one's sexual orientation with health care providers are significant barriers to health care utilization for LGB+ people. However, these studies have concentrated on civilian samples, veterans who served primarily before Don't Ask, Don't Tell (DADT) was repealed, or with individuals who primarily use VHA care. Further, most studies have not examined a more comprehensive array of minority stressors as potential predictors of healthcare utilization. Therefore, the current study utilized a cross-sectional online survey of sexual minority individuals who have served in the military since DADT's repeal to examine the direct and indirect impact of minority stress during military service on health care utilization. To accomplish these goals, cisgender sexual minority adults with military service history after the repeal of DADT were recruited from a Qualtrics panel to participate in an online survey examining the relations between minority stress during time in service and utilization of healthcare services. Data were collected and analyzed regarding LGB+ individuals' demographics, military history, healthcare need, resilience, and minority stress during time in service, including experiences with discrimination, harassment, violent and nonviolent victimization, anticipated stigma, internalized heterosexism, and concealment of one's sexual minority status. Results showed that distal and proximal stressors did not account for significant variance in healthcare utilization after accounting for demographics and chronic health conditions. Instead, chronic physical and mental health conditions, bisexual/pansexual orientation, and resilience predicted healthcare utilization. Resilience did not moderate the relation between minority stress and healthcare utilization but was an independent predictor of healthcare utilization. Gender moderated the relation between both anticipated stigma and healthcare utilization and internalized heterosexism and healthcare utilization, such that higher levels of these stressors predicted less utilization in men and more utilization in women. The results illustrate the impact of health need and use of integrated healthcare systems in shaping adherence to recommend preventative screening and vaccinations in sexual minority veterans. The results demonstrate the role of stigma, heterosexism, and gender in determining how sexual minority service members and veterans receive care and suggests the need for different strategies when working with men and women veterans, with a need to encourage men who experienced sexual stigma to receive preventative healthcare as recommended. The results also highlight the need to explore the role of resilience in this population and the subpopulation of bisexual/pansexual women as they may have experienced protective factors that led them to engage in greater healthcare utilization. Lastly, while minority stress during time in service did not predict healthcare utilization, current minority stress or minority stress in healthcare settings may be a worthwhile area to examine in relation to healthcare utilization. Work in these areas can help to decrease the disparities in healthcare utilization between LGB+ veterans and their heterosexual peers.
