Often these behaviors are either ineffective at reducing accidents or associated with negative outcomes such as depressed mood. The Compensatory Driving Behavior Scale was created to clarify self-regulatory driving behaviors related to this process. The scale yielded a five-component solution assessing Comfort, Vision Comfort, Frequency, Avoidance, and Difficulty Adapting associated with driving in challenging situations. These components demonstrated relationships with age, health, changes in driving abilities, extraversion, emotional stability, and external locus of control. Older drivers reported additional self-regulatory behaviors, including increasing attention, defensive driving, and increasing adherence to the laws. Reasons for these adaptations varied from age to concern for others. A re-conceptualization of the SOC model, focused on independence through driving rather than mobility, follows.

Book excerpt: "Having accurate self-awareness of driving ability may be one important factor to enable older drivers to adopt appropriate self-regulatory behaviors and maintain safe driving performance in their everyday living for as long as possible. Nevertheless, some methodological issues and knowledge gaps related to this topic exist. The overall objective of this thesis was to contribute evidence towards older drivers' self-awareness of their driving ability in a naturalistic driving environment. The first study compared and contrasted how the standard on-road driving evaluation (SODE) and the naturalistic driving observation (NDO) approaches influence older adults' driving performance using a reflective literature review. This study highlighted that, during the SODE, older drivers' performance may be negatively impacted by test anxiety and the unfamiliar environment, but changes in their driving capacity over time can be measured in this structured context. On the contrary, using the NDO approach, older drivers can adjust their driving behavior and environment based on their self-awareness of driving ability and self-regulatory modifications; by reducing the demands of their driving environment, their driving performance could be stable over time despite changes in their driving capacity (Manuscript #1). Based on the previous study, a validated NDO approach, the electronic Driving Observation Schedule (eDOS), was adopted to evaluate older drivers' performance in their everyday driving environment. Some improvements to its scoring system were required and 13 experts in driving rehabilitation were consulted using a two-round on-line survey. A weighted maneuver/environmental complexity score was created to represent the complexity of the driving maneuvers and environment in order to control the fact that the driving environment during each eDOS differs from each other. In addition, a weighted eDOS total score was generated to improve the scoring of older adults' driving performance by accounting for the different risk levels of each driving error type, the maneuver and environment complexity in which they occur (Chapter 4).The next study examined the accuracy of older drivers' self-awareness of driving ability and its associated demographic and clinical factors. The accuracy of self-awareness is defined by the correspondence between one's perceived and actual ability. Perceived driving ability was assessed using the Perceived Driving Ability [PDA] questionnaire, a valid and reliable tool and the weighted eDOS score was used to represent everyday driving performance. By comparing the two scores, 108 older drivers were classified into 3 groups based on the accuracy of estimation of their driving ability: under-estimation (19%); accurate (29%); over-estimation (53%). An ordinal regression model showed that older drivers who over-estimated their driving ability had better visuo-motor processing speed and fewer comorbid conditions (Manuscript #2). A longitudinal study was administered to further examine older drivers' self-awareness of their changes in driving ability over time (n=60). Results indicated that approximately one-third of the older drivers did not detect their declining driving ability over one year. Participants who had worse driving performance at the second session, regardless of their perceived changes in driving ability (Manuscript #3). The findings from this thesis indicate that many older drivers over-estimated their driving ability and did not perceive their declining driving performance over one year. Future studies are needed to examine the relationship between older drivers' accuracy of self-awareness and crash risk, as well as to create intervention programs for enhancing their accuracy of self-awareness of driving ability. " --

Purposes: The aims of the thesis were to: 1) replicate Blanchard's findings on the associations between driver perceptions and self-regulatory practices in older drivers; and 2) extend this investigation by examining driving over a longer monitoring period in the winter.

Certain illnesses that are more common in older adults can affect their capacity to function independently. These include the capacity to make medical decisions, live independently, manage finances, to name a few. Healthcare professionals, especially psychiatrists are often entrusted with the responsibility of assessing an older adult’s capacity to perform one or more functions. This makes it imperative for them to be cognizant of these issues, understand the need for these evaluations, and be able to conduct them in a comprehensive manner. Another way of protecting an older person’s rights and facilitating a life based on their own decisions even after they lose decision making capacity is Advanced Health Care Planning (AHCP). Health care professionals are required to initiate a discussion about AHCP with their patients and their families and review it periodically. Lastly, the older adults incarcerated in prisons is a group that is growing in numbers. They have unique needs at the intersection of the geriatric and forensic services, but are often marginalized by both services. The combination of poor quality of life and increasing costs makes the care of older adults in the criminal justice system makes this topic an important public health concern. There is a pressing need for better training of prison staff in issues of geriatric psychiatry. Assessment of criminal responsibility and competence to stand trial in aging offenders are other complex but under-studied issues. This proposed book will provide a comprehensive view of ethical, medicolegal, and forensic issues that will be useful in clinical practice. There will be three sub-sections, each focusing on ethical, medicolegal and forensic issues respectively. The first section will focus on ethical issues. Its first chapters will provide an overview of the how age and the process of aging influence decision-making and introduce unique ethical dimensions to clinical care. This will be followed by a discussion of the concepts of informed consent and capacity evaluation. The next chapters will focus on common scenarios that arise in the care of elderly patients and offer a practical approach to understanding and managing them. These will include assessments of the capacity to make medical decisions, the capacity to live independently, manage finances, drive a vehicle, have sexual relations etc. A chapter on ethical issues specific to dementia will outline issues related to diagnostic disclosure and genetic testing. Research ethics issues in geriatric psychiatry will also be outlined. The next section of the book will focus on surrogate decision making in an older adult who has been deemed to lack the capacity to serve one or more functions independently. The first chapters in this sub-section will focus on patient directed advance health care planning tools, namely, living will and power of attorney. This will be followed by an overview of default surrogate making. Guardianship will subsequently be covered. A separate chapter will cover the issue of elder abuse and discuss an approach to assessing it. The last section of the book will cover forensic issues in geriatric psychiatry. The first chapter will discuss aging older adults in the criminal justice system from an epidemiological perspective. The growing numbers of incarcerated older adults, their illness burden, the challenges in the diagnosis and management of neurocognitive disorders in the prison setting will be elucidated. The following chapter will discuss competence to stand trial with reference to elderly offenders. This will be followed by a discussion of the concepts of medical reprieve, compassionate release as well as model programs and policies currently in the works for older incarcerated adults.

Book excerpt: This collection considers ways in which societal contexts influence aging by influencing self-regulatory processes. No one doubts that the social contexts in which individuals develop exert strong influence on life trajectories. Those born into environments that provide high quality education, supportive social relations, and economic assets do better in old age than those born into environments bereft of such resources. The extent of this influence, however, is only beginning to be revealed. Recent research shows that life experiences influence basic brain structures (e.g. the effect of musical training on neural organization) and functions (e.g. inflammatory processes), and that social embeddedness may even protect against Alzheimer's disease. Similarly, education increasingly appears to have a "real" effect on neural integrity. Thus, societal contexts may not simply open or close doors for individuals, but may influence self-regulatory processes at the most basic levels of functioning. Although social structures are generally seen as the independent variables that affect individual aging, it is also possible to think of a lifetime development of self-regulatory processes leading to behaviors in old age that can have impact on and modify societal structures. Two parts of this book consider self-regulation as the dependent variable, asking how social contexts influence cognitive, emotional, and self-regulatory processes. The third part reverses the question, treating self-regulation as the independent variable and retirement and physical health as dependent variables. In this part, consideration is given to how the effectiveness of self-regulation influences physical and economic outcomes in old age.
