Book excerpt: In Manitoba, government policy is for public health nurses to screen families with newborns within one week post-discharge for a number of risk factors associated with poor child developmental health, including a question on a past or current history of intimate partner violence (IPV) between parenting partners. The purpose of this study was to examine differences in the developmental trajectories of mothers and their children from the prenatal period to 5-years post-delivery based on the IPV screen response. Administrative databases housed at the Manitoba Centre for Health Policy provided data for this study. Manitoban women giving birth to a live singleton infant from January 1, 2003 to December 31, 2006 were included in analyses. Outcomes assessed included sociodemographic covariates, maternal prenatal morbidities and complications, birth outcomes, maternal postpartum health, child postpartum health, postpartum child welfare organization involvement, and children's readiness for school at kindergarten entry. Descriptive statistics and logistic regression were used to examine differences in outcomes of interest based on IPV screen response (i.e., negative IPV screen, positive IPV screen, not screened for IPV). In the study population, 66.7% of the sample was screened for a history (past or current) of IPV between parenting partners. Among women who were screened, 2.1% screened positive for IPV. Findings indicated that a positive IPV screen was associated with increased maternal prenatal morbidities (e.g., mental health problems, hospitalizations), as well as more adverse birth outcomes (e.g., low birthweight, preterm birth). In the 5 years post-delivery, a positive screen for IPV at birth was associated with poorer maternal and child health, increased child and families services contact, and children being less ready for school at kindergarten entry relative to those with a negative IPV screen. Similar patterns of adverse outcomes were noted among women (and their children) who were not screened for IPV (vs. women screening negative for IPV) in the early postpartum period. Incorporating IPV screening into routine prenatal care, rather than assessing IPV experiences after birth, may help to better identify families in need of support and, ultimately, improve pregnancy outcomes and the longer-term trajectory of women and their children.

Book excerpt: BACKGROUND: Intimate partner violence (IPV) is a worldwide epidemic that is often overlooked, ignored, and underreported. However, it greatly affects the health of women, especially in pregnancy. Healthcare providers and other staff may be in the optimal positions to screen and to identify victims of IPV. The objective of this project is to implement an evidencebased IPV screening tool and protocol for all pregnant and postpartum women, visiting the Women’s Health Department at Waianae Coast Comprehensive Health Center (WCCHC). METHODS: Based on a literature review, the Abuse Assessment Screen (AAS), a five-item tool that assesses for physical, emotional, and sexual abuse, was implemented in the Women’s Health department from August to November 2014. In addition, an updated IPV screening and intervention protocol was implemented. Data was collected prior to and after the implementation of the new screening tool and protocol. Descriptive statistics and trend analysis was completed. OUTCOMES: The AAS, used to screen pregnant women during initial OB visits, revealed an overall IPV rate of 14.6% (6% current IPV, 9% history of IPV), when screened by APRNs. The APRNs had an overall 90.2% screening rate. The perinatal case managers revealed a 6.7% current IPV rate and 10.8% history of IPV in pregnant women, with an overall 63.2% screening rate. Postpartum screening by APRNs revealed an overall 56.5% screening rate, with a 9.8% IPV rate. The most common interventions for women who screened positive for IPV were being counseled directly by the provider and being given a phone resource card, which includes the IPV shelter numbers and local IPV information line. A small percentage of women were referred to Behavioral Health. CONCLUSION: Routine IPV screening for all women seeking prenatal and postpartum care is critical in order to identify and help victims of IPV. Use of the AAS and the new screening and intervention protocol has increased the identification of victims of IPV. Providers should continue to screen for IPV and provide services to pregnant and postpartum women.

OBJECTIVE: To examine evidence on the performance of screening procedures and interventions in the primary care setting in reducing harm from family and intimate partner violence for children, women, and elderly adults. DATA SOURCES: MEDLINE(r), PsycINFO, CINAHL, Health & Psychosocial Instruments, ERIC, AARP Ageline, and the Cochrane Controlled Trials Register, reference lists of systematic reviews, and experts. STUDY SELECTION: Included studies had English-language abstracts, were applicable to U.S. clinical practice, described abuse and neglect in women, children, or elderly adults, were conducted in or linked to primary care, obstetrics/gynecology, or emergency department settings, and included a clinician in the process of assessment or intervention. DATA EXTRACTION: We extracted selected information about study design, patient samples and settings, methods of assessment or intervention, and clinical endpointsand applied a set of criteria to evaluate study quality. DATA SYNTHESIS: No studies directly addressed the effectiveness of screening in a healthcare setting in reducing harm, or described the adverse effects of screening and interventions. All instruments designed to screen for child abuse and neglect were directed to parents, particularly pregnant women. These had fairly high sensitivity but low specificity. Several brief instruments designed to identify women with intimate partner violence compared well to longer previously validated instruments. We found few studies of screening for elder abuse and neglect. A randomized controlled trial with 15-years follow-up indicated that nurse home visits during pregnancy and for 2-years postpartum for low-income women improved abuse and neglect outcomes for children. Studies of interventions for children of other ages, women who are not pregnant, and elderly adults are lacking. CONCLUSIONS: Screening and interventions for child abuse are directed to parents during prenatal and postpartum periods. Several brief screening instruments have been tested for women, but interventions are lacking. Few instruments and no interventions were identified for elderly adults.

Under the rubric of violence against women the two overlapping issues have received increasing attention. Much of the response to these forms of violence has focussed either on advocacy, providing adequate health and legal services and ensuring safety of victims once violence is disclosed, or on screening for violence in the context of health services. When the word "prevention" is used, it is usually in reference to secondary and tertiary prevention - helping women get out of violent situations and preventing further violence. Little attention has been given to primary prevention - addressing the root causes with the goal of reducing the number of new cases. Recent hallmark publications have called for increased investment in primary prevention. As part of the Global Campaign for Violence Prevention, WHO is giving increased attention to the primary prevention of intimate partner violence and sexual violence. Preventing intimate partner and sexual violence against women: taking action and generating evidence is an important new tool for violence prevention researchers, practitioners and advocates provides a planning framework for developing policies and programmes for the prevention of intimate partner and sexual violence. This guide outlines the nature, magnitude, risks and consequences of intimate partner and sexual violence. It outlines strategies to prevent these forms of violence against women and describes how these can be tailored to the needs, capacities and resources of particular settings. It also emphasizes the importance of integrating scientific evaluation into all prevention activities in order to expand current knowledge of what works. The manual describes interventions of known effectiveness, those supported by emerging evidence, and those that could potentially be effective, but have yet to be sufficiently evaluated for their impact. For instance, school-based programmes to prevent violence occurring in "dating relationships" have been shown to be effective in randomized trials in the USA and Canada. Evidence is emerging for the effectiveness of empowerment approaches which use microfinance combined with gender-equality training or are based on communications and relationship skills training such as the Stepping Stones training package, which has been widely used in low- and middle-income countries. Strategies that aim to prevent intimate partner and sexual violence through reducing the harmful use of alcohol also show promise. A six-step framework for planning, implementing and evaluating such prevention programmes and policies is presented. This document is primarily aimed at policy-makers, programme planners and donors from public health and related sectors who seek to advance the prevention of intimate partner and sexual violence against women. In addition to the principal audience, other interested parties will include those working in other government sectors such as education, child welfare, social care, criminal justice and departments of women or gender equality; advocates from civil society organizations; local authorities; environmental and urban planners; and researchers."

The National Institute of Justice and the Centers for Disease Control and Prevention cosponsored the survey through a grant to the Center for Policy Research. The survey consists of telephone interviews with a nationally representative sample of 8,000 U.S. women and 8,000 U.S. men about their experiences as victims of various forms of violence, including intimate partner violence. The survey compares intimate partner victimization rates among women and men, specific racial groups, Hispanics and non-Hispanics, and same-sex and opposite-sex cohabitants. It also examines risk factors associated with intimate partner violence, the rate of injury among rape and physical assault victims, injured victims' use of medical services, and victims' involvement with the justice system. Research on intimate partner violence has increased dramatically over the past 20 years. While greatly enhancing public awareness and understanding of this serious social problem, this research has also created much controversy and confusion. Findings of intimate partner victimization vary widely from study to study. Some studies conclude that women and men are equally likely to be victimized by their partners, but others conclude that women are more likely to be victimized. Some studies conclude that minorities and whites suffer equal rates of intimate partner violence, and others conclude that minorities suffer higher rates. In addition, there are many gaps in the scientific literature on intimate partner violence, such as the level of violence committed against men and women by same-sex intimates. Little empirical data exist on the relationship between different forms of intimate partner violence, such as emotional abuse and physical assault. Finally, little is known of the consequences of intimate partner violence, including rate of injury and victims' use of medical and justice system services. This Research Report addresses these and other issues related to intimate partner violence. The information presented in this report is based on findings from the National Violence Against Women (NVAW) Survey, a national telephone survey jointly sponsored by the National Institute of Justice (NIJ) and the Centers for Disease Control and Prevention (CDC). The survey, which was conducted from November 1995 to May 1996, consists of telephone interviews with a representative sample of 8,000 U.S. women and 8,000 U.S. men. Survey respondents were queried about their experiences as victims of various forms of violence, including rape, physical assault, and stalking by intimate partners. Victimized respondents were asked detailed questions about the characteristics and consequences of their victimization, including the extent and nature of any injuries they sustained, their use of medical services, and their involvement with the justice system. This Research Report also summarizes the survey's findings on victimization rates among women and men, specific racial groups, Hispanics and non-Hispanics, and opposite-sex and same-sex cohabitants. It examines risk factors associated with intimate partner violence, rates of injury among rape and physical assault victims, injured victims' use of medical services, and victims' involvement with the justice system. Although this report focuses on women's and men's experiences as victims of intimate partner violence, complete details about men's and women's experiences as victims of rape, physical assault, and stalking by all types of assailants are contained in earlier NIJ and CDC reports. Because of the sensitive nature of the survey, state-of-the-art techniques were used to protect the confidentiality of the information being sought and to minimize the potential for retraumatizing victims of violence and jeopardizing the safety of respondents.

If experienced, both physical and emotional abuse can cause adverse consequences for mothers and infants. One of those prevalent consequences of IPV is depression. Latinas have been found to experience higher rates of IPV compared to their Caucasian counterparts and depression at 2 to 3 times higher rates than the national average, but are vastly underrepresented in the literature on IPV. The purpose of this study is fourfold. It identifies rates of IPV screening during the perinatal periods. It also examines rates and types of IPV in Latina primary care patients across the perinatal periods, and examines the degree to which IPV rates are accounted for by patient characteristics and type of prenatal care. Lastly, this study investigates the relationship between IPV and depressive symptoms during the perinatal periods. The participant set is 296 low -income Latina primary care patients. Rates of IPV in this population ranged from 7% to 9% throughout the perinatal periods. While 3%-6% of women reported experiencing intimate partner physical abuse, 13%- 23% of women reported experiencing intimate partner emotional abuse. Thus, women reported significantly higher rates of emotional abuse than physical abuse throughout the perinatal periods. Women with higher IPV scores (HITS) were more likely to have higher depression scores (EPDS) throughout this time. Rates of IPV were significantly, positively correlated with rates of depressive symptoms at each trimester and postpartum. Women who preferred their care in Spanish and with greater parity were more likely to have higher rates of IPV (HITS). Both physical and emotional abuse were significantly, positively correlated with depression levels (EPDS) throughout the perinatal periods.
