The computer survey included questions on IPV and other health risks. Computer-generated risk reports were attached to patients' medical charts for review by their physician. Participants in the control arm received standard medical care. Data were collected by audio taping the medical visits and an exit survey of patients. The results reveal that the overall prevalence of IPV in the family practice was high; 14.6% in the pencil and paper survey and 13.7% in the RCT study. The RCT demonstrated that, compared to standard medical care, the computer-assisted screening resulted in a 1.8-fold increase in the likelihood of physician and patients discussing IPV risk and the detection of women at risk of IPV increased by a factor of 2.5-fold. Further, RCT participant patients showed acceptance of the computer-assisted screening, after its use. The interviewed physicians perceived computer-assisted screening as most feasible for periodic health exams and follow-up visits, given the existing logistical limitations. Overall, physicians expressed general acceptance of the computer screening. The results provide impetus for improvements in clinical practice, medical training, policy and research to address the issue of IPV. The use of the screening tool may be enhanced by continued research into its generalizability by inclusion of multiple family practice clinics and various primary care settings. The overall aim of the dissertation research was to advance knowledge on the phenomenon of computer-assisted screening for intimate partner violence (IPV) within a Canadian family practice context. A series of inter-linked studies (quantitative and qualitative) was conducted to answer three research questions: (1) what is the effectiveness of computer-assisted screening for increasing physician-patient discussions on and detection of IPV risk? (2) what is the acceptance of computer-assisted screening among patients? and (3) what is the feasibility of computer-assisted screening from the perspectives of family physicians? To answer the first two questions, a randomized controlled trial (RCT) was conducted at an academically-affiliated family practice clinic in Toronto with adult female patients in a current or recent intimate relationship (n = 314). The RCT planning was facilitated by studies in the pre-trial phase (a pencil and paper survey of patients, and a psychometric scale validation). To answer the third question, in-depth qualitative interviews were conducted with the trial physicians (n = 10).

The National Institute of Justice and the Centers for Disease Control and Prevention cosponsored the survey through a grant to the Center for Policy Research. The survey consists of telephone interviews with a nationally representative sample of 8,000 U.S. women and 8,000 U.S. men about their experiences as victims of various forms of violence, including intimate partner violence. The survey compares intimate partner victimization rates among women and men, specific racial groups, Hispanics and non-Hispanics, and same-sex and opposite-sex cohabitants. It also examines risk factors associated with intimate partner violence, the rate of injury among rape and physical assault victims, injured victims' use of medical services, and victims' involvement with the justice system. Research on intimate partner violence has increased dramatically over the past 20 years. While greatly enhancing public awareness and understanding of this serious social problem, this research has also created much controversy and confusion. Findings of intimate partner victimization vary widely from study to study. Some studies conclude that women and men are equally likely to be victimized by their partners, but others conclude that women are more likely to be victimized. Some studies conclude that minorities and whites suffer equal rates of intimate partner violence, and others conclude that minorities suffer higher rates. In addition, there are many gaps in the scientific literature on intimate partner violence, such as the level of violence committed against men and women by same-sex intimates. Little empirical data exist on the relationship between different forms of intimate partner violence, such as emotional abuse and physical assault. Finally, little is known of the consequences of intimate partner violence, including rate of injury and victims' use of medical and justice system services. This Research Report addresses these and other issues related to intimate partner violence. The information presented in this report is based on findings from the National Violence Against Women (NVAW) Survey, a national telephone survey jointly sponsored by the National Institute of Justice (NIJ) and the Centers for Disease Control and Prevention (CDC). The survey, which was conducted from November 1995 to May 1996, consists of telephone interviews with a representative sample of 8,000 U.S. women and 8,000 U.S. men. Survey respondents were queried about their experiences as victims of various forms of violence, including rape, physical assault, and stalking by intimate partners. Victimized respondents were asked detailed questions about the characteristics and consequences of their victimization, including the extent and nature of any injuries they sustained, their use of medical services, and their involvement with the justice system. This Research Report also summarizes the survey's findings on victimization rates among women and men, specific racial groups, Hispanics and non-Hispanics, and opposite-sex and same-sex cohabitants. It examines risk factors associated with intimate partner violence, rates of injury among rape and physical assault victims, injured victims' use of medical services, and victims' involvement with the justice system. Although this report focuses on women's and men's experiences as victims of intimate partner violence, complete details about men's and women's experiences as victims of rape, physical assault, and stalking by all types of assailants are contained in earlier NIJ and CDC reports. Because of the sensitive nature of the survey, state-of-the-art techniques were used to protect the confidentiality of the information being sought and to minimize the potential for retraumatizing victims of violence and jeopardizing the safety of respondents.

Although 84% of women tell friends and family about IPV, only 21% tell doctors, nurses or other healthcare providers about it. A literature review revealed five themes related to barriers to screening for IPV: lack of time, lack of standardized protocols and tools, lack of knowledge, lack of privacy and provider discomfort. An adaptation of the Johari Window, a tool used for self-awareness, was used to examine these themes. This is a descriptive qualitative study where eleven providers in the tri-county area of Michigan were interviewed to determine their perceived barriers to screening for IPV. Demographic and interview questions were adapted from the literature. Data analysis was completed by first analyzing interviews for themes, then analyzing the themes in the context of the Johari Window. Interviews revealed barriers and facilitators to screening which fit into six themes: patient's unwillingness to disclose, lack of privacy, provider discomfort, lack of time, a lack of standardized protocols and tools, lack of knowledge. The patinpatient'singness to disclose was the most prevalent theme in this study and was not identified in previous studies. The themes were analyzed using the Johari Window framework. Awareness of barriers to screening for IPV can allow providers to assess patient's individually and intervene appropriately when attempting to increase disclosure in primary care." -- From page iv.

Too often this preventable form of violence is repetitive in nature, occurring at multiple points across the lifespan. The prevalence of intimate partner violence is on the higher end of this spectrum in East Africa, with in-country demographic and health surveys indicating that approximately half of all women between the ages of 15-49 in Uganda, Kenya, and Tanzania having experienced physical or sexual abuse within a partnership. It is now widely accepted that preventing intimate partner violence is possible and can be achieved through a greater understanding of the problem; its risk and protective factors; and effective evidence-informed primary, secondary, and tertiary prevention. To that end, on August 11-12, 2014, the Institute of Medicine's Forum on Global Violence Prevention, in a collaborative partnership with the Uganda National Academy of Sciences, convened a workshop focused on informing and creating synergies within a diverse community of researchers, health workers, and decision makers committed to promoting intimate partner violence-prevention efforts that are innovative, evidence-based, and crosscutting. This workshop brought together a variety of stakeholders and community workers from Uganda, Kenya, and Tanzania to engage in a meaningful, multidirectional dialogue regarding intimate partner violence in the region. Preventing Intimate Partner Violence in Uganda, Kenya, and Tanzania summarizes the presentations and discussion of the workshop.

Although the clinician's role in identification and intervention is considered a professional and legal responsibility, the effectiveness of these efforts is unclear. OBJECTIVE: To examine evidence on the performance of screening procedures and interventions in the primary care setting in reducing harm from family and intimate partner violence for children, women, and elderly adults. DATA SOURCES: MEDLINE(r), PsycINFO, CINAHL, Health & Psychosocial Instruments, ERIC, AARP Ageline, and the Cochrane Controlled Trials Register, reference lists of systematic reviews, and experts. STUDY SELECTION: Included studies had English-language abstracts, were applicable to U.S. clinical practice, described abuse and neglect in women, children, or elderly adults, were conducted in or linked to primary care, obstetrics/gynecology, or emergency department settings, and included a clinician in the process of assessment or intervention. DATA EXTRACTION: We extracted selected information about study design, patient samples and settings, methods of assessment or intervention, and clinical endpointsand applied a set of criteria to evaluate study quality. DATA SYNTHESIS: No studies directly addressed the effectiveness of screening in a healthcare setting in reducing harm, or described the adverse effects of screening and interventions. All instruments designed to screen for child abuse and neglect were directed to parents, particularly pregnant women. These had fairly high sensitivity but low specificity. Several brief instruments designed to identify women with intimate partner violence compared well to longer previously validated instruments. We found few studies of screening for elder abuse and neglect. A randomized controlled trial with 15-years follow-up indicated that nurse home visits during pregnancy and for 2-years postpartum for low-income women improved abuse and neglect outcomes for children. Studies of interventions for children of other ages, women who are not pregnant, and elderly adults are lacking. CONCLUSIONS: Screening and interventions for child abuse are directed to parents during prenatal and postpartum periods. Several brief screening instruments have been tested for women, but interventions are lacking. Few instruments and no interventions were identified for elderly adults.
